Don’t wait – integrate!

What is GP2GP?

GP2GP enables patients’ electronic health records to be transferred

directly, securely, and quickly between GP practices.

Improving patient care as GPs have full and detailed medical records

available to them for a new patient’s first consultation.

When does GP2GP happen?

A successful GP2GP electronic health record transfer takes place when:

• a patient joins a GP practice as a full General Medical Services

registration

• there’s a successful PDS (Personal Demographics Service) match

• the new and old practice are both in England

• and both practices are live with GP2GP

Smartcards are essential when registering new patients.

Why use GP2GP?

GP2GP Achieving the lifelong electronic health record

Legal - The General Medical Services GP Contract 2014 places a

contractual requirement to utilise GP2GP whenever possible, for

the transfer of patient records between practices.

Ethical - Patients expect GPs to receive their medical records at the

earliest opportunity. Receipt and integration of the GP2GP electronic

health record fully supports this expectation.

Policy - GP2GP supports the Health Secretary’s objective that ‘patients

should have compatible digital records so their health information can

follow them around the health and social care system’.

GP2GP facts

GP2GP Achieving the lifelong electronic health record

More than 5 million

electronic health

records transferred

to patient’s new GPs

between 2007 and

2015

95% of England’s GP practices are now live

with GP2GP GP2GP is supported

by the BMA & RCGP

Joint GP IT

Committee

Around 50% of all electronic health record

transfers are integrated the same day they are

received by practices

Benefits for practices and patients

6

Full electronic health record available

for the patient’s first and subsequent

consultations ensures continuity of care

for ongoing medical conditions

Safer prescribing provided by access to

the patient’s current and past medication, and to any recorded allergies and adverse reactions

Time savings gained from access to

referral letters, investigation results,

and incoming documentation from

secondary care, out of hours services,

emergency departments

GP2GP Achieving the lifelong electronic health record

No need for patients to provide a detailed

account of their previous medical

history to their new clinician

GP2GP version 2.2 – launching summer 2015

GP2GP Achieving the lifelong electronic health record

Version 2.2 includes a number of benefits for practices and patients:

• Large messaging

• Reduction in paper printing

• Full electronic health record integration for returning patients (like

students and seasonal workers)

LARGE MESSAGING

Records exceeding 5 megabytes or

having more than 99 attachments

will be transferrable.

BENEFITS

Patients with complex health

conditions usually have larger

electronic health records, and the

ability to transfer their medical

information through Large

Messaging supports continuity of

care, and will reduce the time and

effort required by the new practice

to assess medical histories and

prioritise care for these patients.

Large messaging

GP2GP Achieving the lifelong electronic health record

Reducing paper printing

REDUCING PAPER PRINTING

Sending practices will be able to

see if any attachments need

printing because they were not

delivered successfully.

BENEFITS

Sending practices will save time and

energy if they do not need to print

parts of the electronic health record

to send onto the new patient’s GP.

Receiving practices will see a

significant reduction in time-

consuming scanning and attachment

of documents for new patients.

RETURNING PATIENTS

Enables record integration for

patients returning to practices they

have previously been registered at.

BENEFITS

Being able to receive and fully

integrate the electronic health record

for these patients ensures the rich

electronic health record is

maintained when the patient returns

to a previous practice, and supports

continuity of care.

Benefits practices with high numbers

of students in higher education, or

seasonal workers.

Returning patients

GP2GP Achieving the lifelong electronic health record

“Increased safety through

knowledge of allergies, previous

history. Seamless transition of

medical treatment for ongoing

conditions.”

“Everything transfers across in one

quick step and is automatically filed

into the relevant sections of a

patient's record. At first I thought “it

can't be as simple as that?” But it

really is. I think it's fantastic!”

“Gives the clinician a background to the patient's medication history so when

patient attends the GP is more aware of what care will be needed and can

process any follow-up care that may be needed which could (without GP2GP

transfer) get overlooked/stalled until paper notes reach us.”
