Kaiser Permanente Thriving Schools (KP TS) is a multi-year, multi-strategy national initiative to promote wellness for students, staff and teachers in K-12 public schools using a variety of strategies. Kaiser Permanente and the Alliance for a Healthier Generation (HG) Healthy Schools Program are partnering to help schools create healthier school environments by expanding the Thriving Schools Program to hundreds more schools.

The purpose of the Kaiser Permanente Thriving Schools Action Plan Grants program is to provide grants ($1,000-$5,000) to schools to promote health and wellness policy, environment and systems change efforts.

Complete applications are due by 5pm Pacific Time on November 16, 2018.

Who is eligible:

Schools enrolled in/partnering with HG’s Healthy Schools Program in partnership with the Kaiser Permanente Thriving Schools program.

Schools/school districts that have been awarded grants in previous cycles may only apply in the current cycle if they have completed their grant activities AND completed the final on-line grant survey provided by Community Partners.

Guidelines:

The applicant must demonstrate how the grant will:

Support a Healthy Schools Program Action Plan action step (s). Promote sustainable changes within the topic areas below:

o Increase student physical activity during the school dayo Improve student participation in the school meal programo Increase access to healthy foods outside of the school meal program o Improve staff wellness

Change policy systems and environment to position health and wellness as an integral part of the school climate and culture.

Support implementation of local/district, state and federal school food and physical activity policies.

Grant funds may be used to support:

Programs addressing the following high impact strategies:o Physical Activity Strategies

Increase physical activity during daily recess in elementary schools Increase the proportion of PE classes where students spend at least 50% of class

time being moderately to vigorously active

Increase minutes of weekly PE in elementary school Increase years of PE required in middle/high school Promote walking/biking to school Institute physical activity breaks in the classroom

o Nutrition Strategies Increase proportion of competitive foods/beverages sold in school meeting

USDA Smart Snacks in School standards Increase the availability of fruits and vegetables outside of the cafeteria Increase participation in the school breakfast and lunch programs Implement cafeteria alternatives designed to promote increased school meal

participation (kiosks, grab and go, reimbursable vending options, etc.)o Staff Wellness Strategies

Increase staff access to healthy food on school campuses Increase staff access to physical activity on school campuses Promote staff social emotional wellbeing

Equipment and training/professional development that will support a sustained change at the

school.

Grant funds are not allowed to support:

Salary support for any school employee. One time activities. Activities or items covered under the school’s general fund. Curricula. Bottled water. Requests for fitness monitors (e.g. Fitbit, Jawbone, pedometers). Incentives such as water bottles, t-shirts, or other incentive or promotional items.

Required Documentation:

Action Plan Grant Budget. Click here for the budget template. You must upload your Healthy Schools Program Action Plan for your school. If a school district is applying on behalf of more than one school, the school district must submit the Healthy Schools Program Action Plans for each school in one PDF document.

If an organization will serve as the fiscal agent (e.g. 501c3 related to the school such as PTA, school/district foundation):

A copy of the school/school district's current tax exempt determination letter from the Internal Revenue Service.

A letter from the Chief Financial Officer or a Certified Public Accounting Firm, indicating that the school/school district has been granted tax exemption.

A copy of the statue or enabling legislation establishing the school/school district.

Grant Logistics:

Proposed range for grants: $1000 - $5000 (per school). Grant period: 10 months to spend grant Prefer to grant funds to a 501c3 related to the school (PTA, school/district foundation) or funds

can go directly to the school. A school district may apply for multiple schools within the district. Each school must have a

Healthy Schools Program Action Plan. The request must support a common Action Plan Action Step at all schools. If schools within a school district are working on different Action Steps, each school must submit a separate application.

Proposal must be supported by the school principal/administrator or school district leadership if submitting a proposal for more than one school.

Identify project lead and administrative lead (if different). Only one application per school/district per grant cycle. Schools/school districts that have been

awarded grants in previous cycles may only apply in the current cycle if they have completed their grant activities AND completed the final online grant survey provided by Community Partners.

The HG Program Manager for your school(s) must assist in the development of the application and review and approve the final proposal. Please start working with your Program Manager as soon as possible.

Preference will be given to schools/school districts that have not received funds in a previous grant cycle.

All grants will be administered by Community Partners, a partner organization with Kaiser Permanente and Ad Lucem Consulting.

If awarded a grant, Grantees will agree to: o Complete a survey describing how grant funds were used. o Submit photographs and accompanying signed releases for promotional and

informational materials, if requested. o Notify KP TS about any planned media (e.g. press releases, articles), acknowledge KP TS

in media, and share media with KP TS.o Show successes, if requested, to demonstrate progress made toward creating a healthy

school environment.

Grant Request

Grant awards are between $1,000 and $5,000 per school. School districts applying on behalf of more than one school should submit a total request for all schools combined. Grant funds can be used for, but not limited to, professional development, workshops for teachers and administrators, school nutrition programs, physical activity equipment, and more.
