Clinical Data on Capsular Fibrosis after Breast Reconstruction and Subcutaneous Mastectomy Summary. In 1971-1980 a total of 3670 silicone implants were used in breast augmentation, subcutaneous mastectomy, and breast reconstruction. Capsular fibrosis (Baker I I-IV) was found in 19 %, 54 %, and 64 %, respectively, after irradiation in 84 % and without irradiation in 43 %. Of the patients with light skin, 47 % had palpable fibrosis; with dark skin complexion, 12 %. Double lumen prostheses with 25 mg cortisone lowered fibrosis in 109 patients from 54% to 15% after subcutaneous mastectomy and from 64% to 14.3% after reconstruction. There was a high complication rate of 19 % thinning skin, making check-ups and eventual exchange of the implant necessary. In den Jahren 1971- 1980 wurden 3670 Silikonimplantate bei Brust-Augmentationen, subcutanen Mastektomien und Brustwiederaufbau verwendet. Eine Kapselbildung fund sich bei respektive 19 %, 54 % und 64 % beim Wiederaufbau nach postoperativer Bestrahlung bei 84 %, ohne Bestrahlung bei 43 %. Hellh~utige Patientinnen hatten in 47 %, dunkelhfiutigc Patientinnen in 12 % eine Kapselfibrose. Doppelwan- dige Prothesen mit 25 mg Cortisonzusatz bei 109 Patientinnen senkten die H/iufigkeit yon 54 auf 15 % bei der subcutanen Mastektomie und yon 64 auf 14,3 % beim Wiederaufbau. Die hohe Komplikationsrate von 19 % Hautausdtinnungen erfordert konsequente Kontrollen und gegebenenfalls ein Auswechseln des Implantates. Breast asymmetry has been classified as congenital or acquired. Included in the congenital group is the amazon syndrome, whereas, for example, postburn or postmastectomy asymmetry belongs to the acquired group. Fundamentally there are two methods of treatment: reduction of the larger breast or augmentation of the smaller breast. With severely hypoplastic breasts the implant must be buried under a transposed latissimus dorsi muscle flap. A myocutaneous lat. dorsi flap is only necessary when additional skin is required to accommodate a prosthesis. Key words: Breast asymmetry - Reduction - Augmentation - Latissimus dorsi flap.
