This directory provides a list of the community providers for VNSNY CHOICE Managed
Long Term Care (MLTC) and VNSNY CHOICE Total members. The VNSNY CHOICE
network is comprised of hundreds of qualified professionals and long term care services
throughout New York. All of our providers have met strict licensure requirements before
they became a provider in the VNSNY CHOICE network.
How Do I Find A Provider in the Directory?
Many of the providers in our directory have been organized according to county and
specialty. A few services that will be delivered to or provided directly in your home,
such as home delivered meals or transportation, are listed according to specialty.
The directory also indicates which providers offer wheelchair accessible facilities or
services for the hearing impaired or visually impaired. Please look for these symbols
throughout the directory. The symbol legend is listed at the end of the Table of Contents.
Do I Need to Use Plan Providers?
VNSNY CHOICE MLTC and VNSNY CHOICE Total uses a network of community
providers to deliver many of the services that you need. Please talk to your Care
Coordinator about the type of services that should be included in your plan of care.
With a few exceptions, you must receive all services that are covered by VNSNY
CHOICE MLTC and VNSNY CHOICE Total from one of the providers in our provider
network. You do not need to use plan providers in the following situations.
As a VNSNY CHOICE MLTC member, you can choose your own doctor.
You do not need to use a plan provider in an emergency. In an emergency, please
call 911 or get to the closest hospital, emergency room or doctor’s office. Once
the emergency has passed, please contact VNSNY CHOICE as soon as possible
so that we can alter the services you are receiving to best address the changes in
your health needs.
If you need a service that is covered by VNSNY CHOICE MLTC or VNSNY
CHOICE Total, but there is not a network provider with the specific expertise that
you require, we will find an appropriate provider, even if the provider is out of the
network.
If you are receiving health care services from an out-of-network provider at the
time of your enrollment you may continue to see this provider until you complete
the course of treatment for up to 90 days.
If you are in the process of receiving treatment for a health problem and your
provider terminates its relationship with VNSNY CHOICE MLTC or VNSNY
CHOICE Total, you may continue to see this provider to complete the course of
treatment for up to 90 days.

Can I Choose My Own Provider?
You may choose a new provider at any time. The easiest way to change providers is to
ask your Care Coordinator for help. On your own, you can choose any dentist,
optometrist or podiatrist listed in the provider directory. If you make a change, please tell
your Care Coordinator so that he/she can continue to coordinate your care.
Can the List of Plan Providers Change?
Yes, VNSNY CHOICE MLTC or VNSNY CHOICE Total may add or remove a plan
provider from the directory. This provider directory includes all provider additions and
changes received at the time of publication. To make certain that a particular provider is
in the VNSNY CHOICE network, please call Member Services at the number below or
go to our website at vnsnychoice.org and use our online provider search tool.
How Often Will I Receive A New Provider Directory?
We will send you an updated provider directory every year. However, please call
Member Services at the number below if you ever need another copy of your provider
directory.
