The aim of the study was to make a survey of the bladder and bowel situation in adulthood. MATERIAL AND METHODS There were 152 children and adolescents with MMC, born before 1980, who had at least 2 assessments by the urologist at the Rehabilitation Centre in Gothenburg. Of them, 100 individuals were randomly chosen and 66 (30 males, 36 females), with a median age of 38 years (range 28-51), accepted to participate in a telephone interview. RESULTS Upper secondary school was passed by 75% of the individuals, 68% were employed and 35% were married or lived with a partner. Surgery of the urinary tract was performed in 35% of the patients and of the intestine in 14 %. CIC was done by 70% and enema used by 14 %. However, during daytime 60% of the participants had episodes of urinary leakage and 50% episodes of faecal leakage. Pads were used in 70% of the patients while anticholinergic treatment was registered in 12%. In 1/3 of the adults there was no urological follow-up at all or in a frequency of less than once every 5 years. Seventeen % had support by urotherapist, while none had support regarding bowel-emptying regimen. CONCLUSIONS In spite of high incidence of incontinence and despite surgical procedures of the urinary tract in one third of the cases, few adults with MMC had regular follow-up by urologist and urotherapist. Skin derived stem cells (SKPs) are neural crest derived progenitors that retain the ability to differentiate into a variety of mesenchymal and neuronal cell types and could be extremely useful for bladder augmentation. We therefore tested the ability of SKPs to differentiate into bladder smooth muscle cells (BSMcell), a mesenchymal cell type, by exposing them to microenvironments that stimulate proliferation or differentiation of BSMcells. MATERIAL AND METHODS Rat SKPs were injected into ex vivo organ cultures of either mouse embryonic bladders or adult rat bladders exposed to mechanical strain. Alternatively, SKPs were exposed to different patterns of mechanical strain in vitro. Expression of smooth muscle specific markers was analyzed by immunofluorescence microscopy (SMactin, desmin, SMmyosin) and real time PCR (SMactin, SMmyosin, SM22, calponin, myocardin). RESULTS SKPs that were injected into embryonic (E12.5) mouse bladders integrate into the developing muscle and urothelial layer and express SMactin and desmin. SKPs injected into stretch injured adult bladders primarily integrate into the urothelium and the interstitial spaces; few SKPs express BSMcell specific markers. Exposure of SKPs to sustained or sinusoidal patterns of stretch modifies expression of BSMcell specific markers. CONCLUSIONS SKPs can differentiate into BSMcells when provided with adequate microenvironmental cues such as the embryonic bladder and could potentially be used for bladder augmentation surgery.
