High Level Summary Outcomes

4

•As per the charts on slide two the CCG spends £30.8m on 11,336 emergency admissions for patients 75 and over across all providers.

•One of the key outcomes linked to the £5/head investment is to reduce avoidable emergency admissions.

•The work done by practices/localities as a result of the £5/pt investment will contribute towards the overall target the CCG has to reduce admissions by 15% over the next two years.

•For practices in Ipswich and East Suffolk CCG this would equate to

– 1 admission avoided per week per practice – Over the course of a year this would reduce emergency admissions

for the over 75’s by almost 1,900.
