Attention Deficit /Attention Deficit Hyperactivity Disorder

The ADD/ADHD diagnosis should be made by a licensed psychologist or other mental health

professional qualified by academic course work, supervised training, and experience. An acceptable

ADD/ADHD report should have the following elements:

1. The report should be current—within the last three years.

2. The report should be typed on letterhead, dated and signed by the professional

making the diagnosis, and otherwise legible.

3. The evaluation must include a specific diagnosis. 4. The evaluation should be comprehensive and contain the following:

Evidence of current impairment.

Alternative diagnoses or explanations have been ruled out.
