Botulinum toxin type A is a proven drug that has been used since the 1980s in health care to treat muscular diseases. In recent times, it has been increasingly in use for cosmetic purposes — to reduce visible lines and wrinkles.Today, an injection treatment using botulinum toxin type A ‘Botox’ is a very common non-surgical cosmetic treatment. In recent years, millions of such treatments were carried out worldwide. It is the most commonly chosen aesthetic injection treatment owing to its high safety and good results. Further, it is also a relatively quick and simple treatment. Today there is no better non-surgical treatment to remove and prevent wrinkles, especially in the forehead, between the eyebrows and around the eyes. In many cases, the botulinum toxin is used as an alternative to surgery (e.g. forehead lift), thus often delaying the need for surgery.Every time you laugh, get angry, or concentrate, you tighten your facial muscles causing wrinkles and facial lines. Because the elasticity of the skin decreases with age, these lines tend to remain. Botulinum toxin helps to prevent this. The botulinum toxin is injected in small amounts into facial muscles which blocks nerve impulses to selected muscles and causes them to relax — this allows the overlying skin to appear smooth. Except the targeted muscles, other nerve functions in the skin are not in the least affected. The result is that lines and wrinkles are softened and become less visible or disappear completely.The common treatment areas are between the eyebrows, crow’s feet around the eyes and the horizontal lines on the forehead. Other treatment areas include those which are characterized by overactive musculature (bruxism / teeth grinding), excessive sweating (hyperhidrosis) and tension headaches. A good result of this treatment depends on injecting the correct amount of botulinum toxin in the right place.

When is the botulinum toxin treatment appropriate?• If you have marked lines between your eyes that make you look tired, angry or sad.• When facial muscles cause wrinkles around your eyes and forehead.• In cases of asymmetry in the face.As a rule, the treatment is for already existing wrinkles and lines. What happens before the treatment?

You meet our Doctor or Nurse for a personal consultation where you review your requirements. Based on this, they will recommend the appropriate treatment and informs you of the benefits and risks. Then you take a joint decision on the treatment to be performed.

The Botox treatment can usually be performed in direct connection with the consultation, but we prefer that you give yourself some time for reflecting on the newly gathered information. In addition to the consultation, we also review your health status and possible medications. Generally, we suggest to avoid taking Blood-thinners medicines and herbal remedies before an injection treatment.

Also, if you are pregnant or breastfeeding, we do not recommend treatment with Botox or fillers.

How is the wrinkle treatment performed?

A clinician injects a small amount of botulinum toxin using a very thin needle in the area one wishes to treat. The total treatment takes between 10–20 minutes.

What happens after the treatment?

As a rule, the treatment requires no convalescence; you can go back to work or to a party in the evening, if you wish. However, sometimes, some sensitive areas of the face have the tendency to bruise, so it is good to be prepared for this.

You should avoid rubbing or massaging the treated area for about 4 hours after the treatment — it can make botulinum toxin spread to the nearby muscles that you do not want to treat. One should also take it easy for approximately 48 hours after the treatment and avoid any kind of physical exercise.The result of the treatment will not be seen immediately — the effect sets in after a few days and it may take up to 3 weeks before the full effects are visible. In the first meeting, we book a return visit in order to assess the treatment’s performance and determine if a larger dose of the product is required to achieve the desired effect.The effect of Botox is not permanent. The results last for about 3–6 months; however, the treatment may be repeated after this time. In order to maintain the desired effect, the treatment should be repeated 2–3 times per year. Studies have shown that the effect lasts a little longer with each subsequent treatment.The Medical Products Agency in Sweden has approved the pharmaceutical company Allergan’s cell-based potency assay for two different types of muscle relaxants. The approval from the Swedish Medical Products Agency is specific to Allergan’s muscle relaxant. This approval does not include muscle relaxants produced by other manufacturers, which will continue with their animal-based tests.

Q: How does BOTOX® work?A: BOTOX® blocks signals from the nerves to the muscles. Due to this, the injected muscles cannot contract. This reduces the wrinkles and gives a more relaxed and soft look. BOTOX® is most often used on forehead lines, crow’s feet (lines around the eye), and frown lines. A point to be noted: wrinkles caused by sun damage and gravity will not respond to BOTOX®.

Q: How is a BOTOX® procedure performed?A: Getting BOTOX® only takes a few minutes and doesn’t require anaesthesia. BOTOX® is injected with a fine needle into specific muscles with only minor discomfort. It generally takes 3–14 days to take full effect, and it is best to avoid alcohol starting at least one week before the procedure. It is also advisable to stop taking aspirin two weeks before the treatment in order to reduce bruising.

Q: What are the side effects of BOTOX®?A: Temporary bruising is the most common side effect of BOTOX®. Headaches, which disappear in 24–48 hours, can occur, but this is rare. A small percentage of patients may develop eyelid drooping. This usually resolves within three weeks. Drooping usually occurs when the Botox moves from the injection site, so it is advisable not to rub the treated area for 12 hours after the injection or to lie down for at least 4 hours.

Q: Who should not receive BOTOX®?A: People who are pregnant, breastfeeding, or suffer from a neurological disease should not use Botox. Since Botox doesn’t work for all wrinkles, you should consult with a doctor first.

Q: How safe are wrinkle-relaxing injections? How often should you get BOTOX® after your first injection? How many times can you have BOTOX®?A: BOTOX® is very safe when injected by an experienced physician, dentist or nurse. It is recommended to wait for at least 3 months between Botox treatments. However, the body’s immune system is capable of developing antibodies to the Botox molecule which can render it ineffective at some point in the future.

Q: Is BOTOX® painful? I have a very low pain threshold. A: BOTOX®, itself, is not painful. The pain is associated with the needle stick. The smaller the needle, the better it is tolerated. When a volume of fluid is injected under the skin, it temporarily stimulates some of the nerve fibres. The threshold for these occurrences differs from person to person.

Q: How many BOTOX® treatments are needed before you see the desired results, does it hurt, and what is the healing period and aftercare? A: When injected appropriately, the results will become noticeable after the first treatment. It takes 3–7 days before the muscle relaxing effects become visible. The maximum effect occurs at about day 10–14. The effect then lasts for about 3–6 months.

Pain and discomfort depends on one’s tolerance to needle injections around the face. There is no typical ‘healing period and aftercare’.

Q: Who can perform BOTOX®? A: BOTOX® is a prescription drug. It is administered by a physician, a dentist or a nurse.

Q: Is it true that the more I have Botox, the less I need to use it? A: This has been a topic of discussion among many physicians. Muscles that are not used tend to atrophy and weaken over time. Therefore, the muscles that are continually injected with BOTOX® may fall into this category and produce a similar effect. This might result in using less BOTOX® over time.

Q: What happens if I discontinue BOTOX® after the first time or after several times? Will my forehead look worse than before I started treatment? Some people tell me once you start using it, you can never stop. A: The wrinkles of your forehead should return to their original appearance when the BOTOX® wears off. Discontinuing the BOTOX® treatment will not worsen your wrinkles.

The reason ‘you can never stop’ using the BOTOX® is because after seeing the disappearance of your wrinkles, you would ‘never want to see the wrinkles’ again. You can stop the treatment at any point without facing any undesirable consequences (other than the return of the wrinkles, of course).

Q: I’m 25 years old and am starting to notice fine lines around my eyes. Am I too young to have BOTOX®? A: No, but be warned; once you start, you are bound to be ‘hooked!’

Q: I have heavyset brows and I don’t want to resort to surgery. Can BOTOX® be used for a ‘mini-lift’? A: When injected in the correct area under the eyebrows, BOTOX® can raise the eyebrows (‘mini-lift’) to a certain degree. However, there is a relative risk — the same injection could inadvertently cause the upper eyelid to fall.

Q: Does BOTOX® diminish horizontal forehead wrinkles? A: Yes, when injected into the frontalis muscle of the forehead (the one that lifts your eyebrows), the horizontal wrinkles can be diminished.

Q: Can you inject BOTOX® in numerous places at the same time?A: Yes, BOTOX® is frequently given in multiple areas during one session.

Q: Will I have a frozen appearance after BOTOX®? I fear I won’t be able to move my forehead. A: It all depends on where and how much BOTOX® is injected. There are those who want their foreheads ‘frozen’. Your desired BOTOX® effects should be discussed with your doctor prior to undergoing the treatment. The ‘where and how much’ can be individualized based on your wishes.

Q: What are the risks and complications of BOTOX®? A: The main risks and complications of BOTOX® are associated with relaxing a muscle that was not intended to be injected. This can include a myriad of symptoms depending on which muscle is inadvertently weakened.

Q: I had BOTOX® injections two weeks ago. After one week my left eyelid began to droop. How long will it last? A: Occasionally some of the BOTOX® can migrate beyond the targeted area and affect another muscle. It sounds like the

muscle that lifts your eyelid was affected in this way. Typically, a drooping eyelid as a side effect of BOTOX® will last a couple of weeks, depending on how much of the Botox migrated into that muscle. As with the intended effects of the BOTOX®, this side effect will eventually wear off as well.

Q: Are there reports of BOTOX® causing double vision and blurry vision and at what time after the injections? A: This would depend completely on where the BOTOX® is injected. Double vision and blurred vision are the two known temporary side effects of the Botox injected around the eyes.

Q: I had BOTOX® a few days ago and don’t like the results. What can I do? How do I reverse the effects of Botox? A: All you can do is wait for the BOTOX® to wear off. The effects of Botox can last up to 6 months.

Q: Is BOTOX® toxic? Will it affect my health after several treatments? I want to use it only for cosmetic purposes on my forehead.A: All drugs and/or medications, when taken at certain high dosage levels, can produce toxic effects. When used appropriately, BOTOX® should not have a toxic effect on your general health, even after several treatments. The amount required to effectively reduce the wrinkles on your forehead is far below any toxic level of the drug.

BOTOX® and Other Medications

Q: What vitamins and herbs affect the BOTOX® treatment? A: I am unaware of any vitamin or herb that has a direct effect on the effectiveness of BOTOX®. However, certain zinc supplements have been studied as products that might enhance the effects of Botox. There are many vitamins (e.g., vitamin E) and herbs (e.g., clove, garlic, ginger, ginseng, and liquorice) that show anticoagulant or blood-thinning effects. Since BOTOX® is injected, avoiding these products prior to the injections may help reduce the possible bruising associated with these injections.

Questions & Answers about Fillers

Q: What are the most common filler treatments?A: Above all, we treat the nasolabial folds, corners of the mouth, lips and cheekbones.

Q: What is the purpose of fillers?A: A filler is a translucent gel that is developed chemically to mimic the body’s own hyaluronic acid. A filler is injected into the wrinkles and folds in the face in order to smoothen them out. Fillers can give your lips more volume and a clearer outline or lip line. These are the most commonly known treatment areas of fillers. However, dermal fillers can also sculpt cheekbones, giving the cheeks an increased volume and the skin more moisture and elasticity by using a product known as booster.

Furthermore, unknown to many people fillers can be used to sculpt and create new contours and shapes; for example, by carrying out small adjustments to the nose. There are very many different applications, but these are the most commonly known ones.

Q: What should you consider before the filler treatment?A: It is important to go to a serious practitioner, i.e. a well-trained doctor, dentist or nurse who uses safe and quality products.

The most important thing to consider is the training received by the treatment provider performing the treatment — whether he/she has passed the stipulated training. It is necessary to check if the injectors are always careful to ask the client about their medical history, for example, about allergies, medical conditions, etc. If the customer is ill, he / she should not be injected.

Q: What is the process when clients approach you?A: New clients will always undergo a consultation, wherein we review several aspects with them such as the difference between our treatments, the products we use and the options. We look at the mirror with the patient and ask the patient to demonstrate what he or she wants. Then, we take a photo of the area so that we can later compare the results — the before-and-after look. The injection treatment:Some clients prefer an anaesthetic ointment (pain relieving ointment) that can be applied before the treatment. Q: What are the risks and side effects? How will I look after the treatment?A: Following the injection, some redness and swelling may occur at the injection site and in some cases you may notice bruising. Most side effects are mild to moderate and are transient as well. All injections entail the risk of infection; so be sure to follow the recommendations you receive and stay hygienic.

Q: How long is the durability of fillers?A: The preparations are made using the body like substance hyaluronic acid and this material is broken down naturally. It is the reassurance of preparations but also creates limitations for sustainability. With Juvéderm® having a unique cross-linking technology, which forms a network-like structure, the general durability is 6–15 months.

Q: Will it hurt?A: No, the treatment is relatively quick and usually does not call for anaesthesia or any specific pain relief treatment. The filler preparation itself contains the pain reduction element. The exception, however, is the treatment of lips, which have abundant nerves; we will supplement this area with external topical anaesthesia if you wish to make the treatment experience more gentle.

Q: How much does the treatment cost?A: The amount of preparation required to treat the area determines the treatment price. An injection dose costs 2600 kronor including the follow-up visits.
