What is a Stretch mark

- Narrow streak-like lines• On the surface of the skin.• Also known as stria or striae.• Often red or purple at first, • Gradually fading to a silvery-white colour. • 1-10mm wide• A few cms long.

• Stretch marks occur in dermis when the skin is stretched considerably over a short period of time.

• This rapid stretching causes the dermis to break in places and allow the deeper layers of the skin to show through,

What is a Stretch mark

- 90% of women- after pregnancy- after a fast growth + puberty hormonal changes- after corticoids (Cushing syndrome, corticosteroid trea tments,

derived use of topic corticoids to bleach dark skin t ypes)- after bodybuilding (steroids, muscle growth, diuretics)

Common used treatments

Young red strech marks: Lasers to erase microvascularisation (KTP, PDL, Cop per Bromide laser)

White old corrugated SM:• Dermabrasion• Peeling (TCA)• Near infrared lasers: 1064 nm, 1320 nm, 1450 nm, 1550 nm• Fractional ablative lasers: CO2, ERBIUM• Mesotherapy

So why Carboxytherapy?

The same idea :

• create a mid dermis trauma to initiate a healing proces s (collagen, healing factors...)

• optimize the healing process with more oxygen and mo re on-site vascularisation

3 Effects

The borh effect: • in the presence of CO2, the binding affinity betwee n haemoglobin and oxygen

is lowered • --> more oxygen is released on site.(Temperature level may improve the O2 release)

• Neovascularization and blood flow increase has been demonstrated by laser doppler and biopsies

• --> more oxygen, more healing factors released on site.

• 4 to 10 sessions• 2 to 4 weeks between sessions• 80 to 150 cc/min flow (high flows)• intradermal injection• 30G needle with bevel up• skin disinfection prior to any injections• injections along each mark, 2 cm spaced• end point: popcorn effect, warmth feeling. redness after

deflation .• painful technique (emla, nitrous oxyde. ..)• no downtime• no post injection treatment or cream (hemoclar homeo pathy to

prevent reduce hematomas )

• After injection– Pop-corn effect, 1 to 2 minutes after injection– Erythema, lasts from 5 to 10 minutes after injectio n– Warmth sensation 5 to 10 minutes after injection– Then back to normal appearance

My feelings after several hundreds sessions

• Complementary to other modalities (combine+++)• Less results on thin white superficial SM• Better results on old, deep, corrugated SM• Better results on dark skin types• Good candidates see results after a few sessions (red uced

depth)• Low investment for the physician • Reduced cost for the patient.• It never completely deletes SM: only improvement.

My feelings after several hundreds sessions

Although we have nothing perfect to remove SM, Carbo n Dioxide Therapy should be considered as a possible modality f or treating old deep stretch marks.

• It is not invasive. • It has a low profile of risk.• It is not expensive.
