Kirkwood Celebrates Renovations with Open House

a good cup of coffee—“just like home.” For atime, his wife was apatient in the roomnext to Mr. Blizzard’s.When she was able,the two of them couldwalk from one roomto the other, “likebeing at home.” On December 10th, Mrs. Blizzard“passed away in that next room,” he said.

With warmer weather forecasted, Mr. Blizzard looksforward to going outside, he says. A certifiednursing assistant wheeled him around the groundsone day; he and one of our CNAs aim to planttomatoes and herbs by the terrace outside hisroom.

OnAugust 12 when we moved our Stevens PCUpatients to the Forlines PCU, she was ready at4:30 a.m.—the staff encouraged her to wait a fewmore hours before making the eight-mile trip to

Caldwell Hospice and Palliative Care Board of Directors

Caldwell Hospice and Palliative Care held an “openhouse” at Kirkwood on February 25. Patients, theirfamilies, and the community got a first-hand look atthe main house, the carriage house, offices, and thepatient care unit, all of which underwent extensiverenovation from mid-August to mid-November 2010.With the completion of this latest project, we cannow report on the benefits to patients and familiesof our construction project at the Robbins Centerand our renovations at Kirkwood.

To date, the occupancy rate for both facilities isbetween 90 and 100%, which shows thatconstructing a second facility addressed a real needexperienced by our patients and their families.

Even with three times the number of beds, thedemand for rooms remains greater than the supply.“We stay full almost all the time,” said one of ourmedical social workers. “People have been amazedthat we filled up immediately and have pretty muchstayed full.” Another of our medical social workersreported that a family member expressed apprecia-tion for the new facility in Hudson and said, “Weneed another facility to help more people so theydon't have to wait!”

We offer two levels of patient care at both facilities:general inpatient (acute and respite care) andresidential hospice care.

The opportunity to offer residential hospice care wasa major reason for constructing the additionalfacility. According to one of our medical socialworkers, having residential care as an option was a“lifesaver” for two of the families with whom sheworks. In the second half of 2010, we served acombined 18 residential patients and 156 generalinpatients.

Mr. Morris Blizzard (pictured at right) was admittedto the Forlines PCU as a residential patient soonafter it opened. “I couldn’t ask for any better,” hesaid. He begins his day with the morning paper and

Among terminally ill patients under hospice care,end-stage cardiac patients are our most fragilebecause any one of four significant problems canarise so quickly: chest pains, shortness of breath(SOB), edema (swelling), and anxiety.

We need to resolve these symptoms immediatelyso that patients feel better, physically and emotion-ally (for example, chest pains and SOB canheighten anxiety, or vice versa, thus making thesituation worse). We developed a general plan thatour hospice nurses follow and a symptom relief kit,based on the accepted standard treatment for eachproblem.

Each cardiac patient’s personal doctor oversees thetreatment plan because that doctor knows thepatient and can tailor the plan and the symptomrelief kit to the patient’s specific needs. Thismethod of providing care ensures that hospicenurses have specific orders to relieve patients’symptoms.

The Cardiac Care at Home program helps hospiceend-stage cardiac patients gain confidence inrecognizing and responding to the problems associ-ated with their condition. It might include ourrecommending solutions and/or changes to thepatient’s daily routine and habits; for example,incorporating “rest breaks” rather than trying toaccomplish a task all at once. A clear benefit is thatpatients can stay at home, with fewer emergencyroom visits and fewer hospitalizations. This matters

because heart failure isthe leading cause ofhospital admissions inthe United States forpeople 65 years andolder.

Cardiac Care at Home isnot limited to resolvingmedication concerns.Patients can choose tohave a medical social worker, a chaplain, and/or acounselor assigned to them—this service will be ofparticular benefit if they’re experiencing anxiety.They can have a Caldwell Hospice CNA help themwith their daily tasks in order to conserve theirenergy. For example, if shortness of breath is anissue, without having someone help the patientget to the shower, it might take two to three hoursto complete this “simple” task.

Anyone can refer a patient to Caldwell Hospice’sCardiac Care at Home program. We’ll follow up onthe referral by requesting an order for admissionfrom the patient’s attending physician. The patientis always in charge; our focus remains centered onthe individual. The patient and family will determinewhat the plan of care will be, and it will be tailoredspecifically to the patient’s needs.

We will miss Avis S. Corpening, retired Caldwell Hospice and Palliative CareBoard member, whose memories span this organization’s almost 30-yearhistory. Avis liked to say that she was “familiar with hospice from everydirection.” She was a member of our very first volunteer training class. Inthose early years, she served as one of our volunteer nurses, aftercompleting a “hospice course” through Catawba Memorial Hospital (nowCatawba Valley Medical Center) and CVTI (now CVCC). She contributedunselfishly and humbly. According to Avis, “I just went where they sent me.”

Understanding Your Suicide Grief: Permission to Heal

In counseling terminology, “complicated grief”refers to situations—such things as divorce,estrangement of siblings, extreme remorsefollowing an argument prior to the loved one’sdeath—that make families’ grief after the loss of aloved one harder to absorb. One of the mostpowerful and least-discussed circumstances followsthe death of a loved one by suicide. Shame, guilt,and embarrassment discourage many people fromtalking at all, even though family members need totalk, as part of coming to terms with the loss, theemotions, the profound changes that result fromthe death and will follow them for years to come. People can go for years not discussing a death bysuicide with anyone outside the immediate family—if they even discuss it within the family. Friendsand neighbors may be reluctant to mention theperson, the person’s death, or memories of theperson, afraid they might hurt anyone’s feelings bybringing up a “sore subject.” The silence becomesdeafening, and the emotional wounds may neverheal.

Rev. Marcus Ollis, pastorof St. John’s LutheranChurch in Hudson, knowssuicide survivors’ compli-cated grief very well.When his church hostedan eight-week griefsupport program withCaldwell Hospice’schaplain Dr. Steve Butleras the lead facilitator, thetwo ministers learnedthat every participant was a suicide survivor. Theprogram brought healing at a level that neither onehad anticipated. One assignment, writing a letter tothe person who had died, was especially helpful.“The participants broke through their isolation andministered to each other,” Rev. Ollis said. By theprogram’s conclusion, they were more able to talkand remember in a healthy way.

Survivors’ shame—fear of how others will react tothe death—is one of the first responses, Rev. Ollissays. Guilt is common, as survivors wonder whatthey missed, what they might have done differ-ently, etc. Survivors feel anger and betrayal,wondering why their loved one did this; they mayfeel abandoned by the loved one, too. It can behard to remember the person’s life apart from theperson’s death, as if the death is all that matters.

Rev. Ollis says a breakthrough takes place whensurvivors are able to talk, when they are “given

permission” to express feelings, tell memories, andgrieve in their own way. Some family membershave been grieving for years without talking toothers. He recommends that friends and familymembers “check in” with survivors more frequentlythan they would if it were a “regular” death.

Suggestions for friends, co-workers, and familymembers:

v Surround the survivors with love and concern.

v You don’t have to say anything—just be thereand listen; it can help you to know what to say.

v Be sensitive to what’s happening in their lives.

v Tell your memories of the person who died—funny stories, favorite sayings, shared experi-ences. Family members may be starved to hearsomething good about—and unrelated to thedeath of—their loved one.

Advice to the survivors:

v Ask for help because you’re not alone.

v You may be feeling alienated, but there is help.

v Your pain can be lessened.

v You can learn ways to grieve in a healthy way.

v There are people who care, who want to help,who will listen without judging.

Camelot Manor CNA Jeffrey McElrath has been namedCaldwell Hospice’s “Above and Beyond” recipient for fourthquarter 2010. Going “Above and Beyond”

Caldwell Hospice VolunteersCaldwell Hospice offers, as a service to its patientsand their families, specially trained 11th Hourvolunteers who are available to be with themduring what will be, most likely, their final 24 to 48hours. We asked two of our 11th Hour volunteersto explain what they do and why their presencematters.Reba Barlow: “When I go toa home, I sometimes findpeople tired and worn outfrom being caregivers. Theysee the hospice volunteer as anew face who will offersupport and a good listeningear. The families need to talkabout their loved ones, tellstories about them.” In one case, Reba explained, “a young man and hiswife were visiting his mother at a nursing home.He looked scared and confused. After talking tohim and getting a feel for where he was in hisgrieving, I asked if he had told his mother good-bye; he said no. He walked over to her, held herhand, and talked to her for several minutes. I thinkhe needed that time with her. After the son left theroom, I went to the patient’s bedside, and shedied, as I held her hand.” The rewards are many for our volunteers, Rebasaid: “When you see the faces of family membersas their loved one dies, when you can get a hugfrom a family member because you cared enoughto be there, when they say ’thank you’ and can’tthank you enough for being there, when they trustyou enough to bring you into their circle of grief, itis rewarding in ways you can never explain. So Isay to you who might be thinking of becoming an11th hour volunteer, take the training. Go sit withone family, and you will see what a difference youcan make to that one family.”

Joyce Roberts: When shearrives, Joyce introducesherself to the patient, family,and everyone in the room.She asks whether there’sanything she can do and letsthem know how long she isscheduled to stay because,Joyce says, “Families needreassurance that I’ll be therefor the allotted time.” Sometimes, the patient or a family member willwant to talk about worries or concerns. At othertimes, talk isn’t necessary. “It’s important to havea hospice presence—to be a comfort—whatevercomfort you can be, even by sitting in the cornerand not saying a thing.”

Most of the time, Joyceshares the 11th hourvigils with patients inlong-term-care facilitiesor hospital rooms. “I’mright here,” she says tothem. “You’re not byyourself,” and she will tellthem over and over, ifshe suspects they needthe reassurance. Somepatients are facing theend of life alone, withoutfamily nearby, and forJoyce, this strengthensher resolve to “be there.”

Reba and Joyce recognize their experiences asbeing life-changing. At this vulnerable time, Joycesaid, patients or their family members “sometimesreach for you.” They may initiate the hug because“they think you, representing hospice, havesomething special.”

Caldwell Hospice Volunteers in Action

The volunTEENS who serve Caldwell Hospice’spatients and staff are young people with a purpose.Their assignments include holiday caroling in long-term-care facilities and our patient care units,creating cards and other crafts for patients, bakingcookies for patients, helping with yardwork andgrounds maintenance at both facilities.

Our South Caldwell High School volunTEENS club“adopted” Brockford Inn in Granite Falls, and ourWest Caldwell High School volunTEENS clubadopted Grandview Villa. They make monthly visitsto play Bingo or enjoy “movies and popcorn” withthe residents. Some of the students have carriedtheir end-of-school-year parties, with refreshments,to share with the residents.

Our volunTEENS say it’s almost like being withfamily members, and the residents say our teenshelp them feel young.

We are proud of the work these young people dofor us.

Welcome, New Volunteers! VolunTEENS Play Bingo

v Yardwork and maintenance: Help with planting, mulching, pruning, and watering at Kirkwood inLenoir and the Robbins Center in Hudson.

v Vigil Music by the Bedside: Bring musical talent or ability to operate the CD player; receive specialtraining to provide music to our patient care unit patients at the end of their lives.

v PCU receptionists: Front desk, Forlines PCU, 8 a.m. to 8 p.m. Monday through Friday—welcomevisitors, show them to patients’ rooms, answer the phone, accept Caldwell Hospice employment appli-cations, introduce family members to the PCU, give tours for families, groups, etc.

v Activities for residential patients: We received an activities cart and an art cart at the ForlinesPCU; now, we need volunteers to organize activities, plan pastimes, and spend time with our residen-tial patients.

v Hairdressers: Our patients need volunteers who can cut and style hair, trim beards, etc. for patientsin their homes or in the PCUs because nothing makes people feel better than having their hair styled!

her new home. One of thebenefits of being a residentialpatient, she said, is that “I’msure to eat.” Quality of life isanother. Mrs. Frye’s medicalsocial worker took her on anafternoon excursion, recently,visiting her old neighborhood,her hairdresser, and otherfriends.

To staff an additional facility meant hiring moreemployees. Between June and December 2010, weadded 31 employees; 24 of them were relateddirectly to our opening the Forlines PCU. Two of thenew employees, our dietary staff, prepare threemeals a day for patients in the Forlines PCU. “Thepatients love the food, even when it is pureed!” oneof our direct-care staff members said. “Most havecommented on how lucky they feel to have CaldwellHospice, and how good it is to have a facility inHudson.”

The convenience of having facilities in southernCaldwell County and in Lenoir is a benefit. “Whenwe have beds available on both units, patients andfamilies request a specific location, closest to theirown home,” said one of our medical social workers.

What does residential care mean to our patients?According to Mrs. Frye, who counts staff membersas her friends, “I wouldn’t advise anybody to goanywhere else. They are so good up here!” Tosomeone with doubts about moving to one of ourresidential rooms, Mr. Blizzard’s advice would be:“Go! You’ll like it!” I couldn’t have said it bettermyself. —Cathy Swanson

Caldwell Hospice Board Member Spotlight: Dr. Robert Belk

When Dr. Robert Belk came to Lenoir in 1973, heintended to practice medicine in cardiology and“cure people,” as he says. He was familiar with theconcept of hospice care, but nothing concrete. In1981 as the Caldwell County Hospice steeringcommittee was forming, he became involved. Rev.Parker Williamson, his pastor at First PresbyterianChurch, Lenoir, asked him to represent the medicalcommunity and help it understand the need for ahospice program in the county.

How instrumental was Dr. Belk to gaining commu-nity support for Caldwell Hospice? “Very,” saidBoard President Parker Williamson. He was “highlyrespected by the medical community,” and “Bobbelieved in this hospice; his faith and his confi-dence in it won the medical community.” Parkerreports that Dr. Belk “went to Medical Societymeetings, and when it was his turn to do theprogram, he would bring in a hospice speaker; as aresult, we started getting referrals.”

Thus began the treasured relationship between Dr.Belk and our hospice staff, our patients, and theirfamilies. Dr. Belk has remained the bridge betweenour organization and the people we serve fornearly 30 years.

Dr. Belk calls his Caldwell Hospice tenure, first as avolunteer and then as its full-time medical director,a rewarding experience. He also admits that it

“gets difficult at times.” This is a small communityand the Caldwell Hospice staff takes care of peoplewho know each other. Part of the job is to tellfamilies that their loved one is dying and leavethem with a different kind of hope.

One of the best things Caldwell Hospice does, in Dr.Belk’s estimation, is to help bring families backtogether. He’s impressed with Caldwell Hospice’sstaff members and their involvement with patients.They couldn’t work here if they didn’t care somuch, he says. And they care enough to try tofulfill patients’ wishes and needs.

Dr. Belk wants the community to know that “weappreciate all that you do” to help Caldwell Hospiceserve people who are dying. He says, “It’srewarding to be there with people who aren’t goingto get well.”

In his life away from Caldwell Hospice, Dr. Belkgoes home every day to Donna Belk, his wife ofnearly 43 years, and their beloved dogs: “we’vehad Golden Retrievers forever.” The Belks have twosons, one daughter, and five grandchildren: threethree-year-olds, one five-year-old, and one seven-year-old.

Another of Dr. Belk’s passions is sailing, which “wetry to do three to four times a year—on theChesapeake Bay as much as possible and Tampa agood bit.”
