Preface The new summer 2016 editions of Casino City’s Gaming Business Directory, Casino City’s Gaming Business Directory CD and the GamingDirectory.com online service have all been completely revised. Since publication of the last edition we have reviewed and updated every section of the directory. The result is print and CD editions of the summer 2016 directory that include thousands of changes in gaming industry property and owner contacts. As the volume of information we gather has grown, we have continued to differentiate the print, CD and online versions of our directories. Our GamingDirectory.com online service now provides direct links to well over 10,000 public LinkedIn® profile pages of executive contacts. As with previous editions of Casino City’s Gaming Business Directory, this print edition provides a concise version of the international gaming properties section, with full property and contact details available on Casino City’s Gaming Business Directory CD and through our online subscription service at GamingDirectory.com. We continue to use icons to differentiate Native

American I, under construction t and track �gaming facilities.

The creation of the new summer 2016 editions of Casino City’s Gaming Business Directory, Casino City’s Gaming Business Directory CD and the GamingDirectory.com online service as well as all the underlying databases and processes, was a massive effort involving many disciplines by a team of very dedicated people. We’re proud of the effort put forth by every employee of Casino City Press.

Steve Bloom, Vice President of Engineering, has overall responsibility for the software engineering efforts required to create Casino City’s Gaming Business Directory and CD including database design, data-entry applications and data extract procedures, in addition to managing the design and layout of the book and CD. Andrea Mullaney, Director of Marketing, created the cover artwork for the directory and CD. Tom Yee, Content Manager, leads the directories content team, which was responsible for all data collection and update efforts. Introduction Casino City’s Gaming Business Directory CD provides in-depth information about the gaming industry worldwide. Gaming properties, property owners and executive contacts are all included. Detailed information on more than 7,000 gaming properties is provided, including comprehensive coverage of casinos, card rooms, racinos, horse tracks, casino cruises and cruise ships in more than 150 countries worldwide, plus dog tracks, jai-alai frontons and off-track betting facilities in North America. More than 930 gaming property owners are identified together with the more than 4,000 properties they collectively own and operate. Nearly 25,000 gaming property and property owner executive contacts are included. Contacts are classified into 40 departments including senior management, casino operations, finance, legal counsel, compliance, internal audit, risk management, public relations, marketing, player development, slots, table games, poker, keno, bingo, sportsbook, racebook, racing operations, mutuels, simulcasting, cage, hotel, retail operations, food, beverage, human resources, security, surveillance, facilities, property design, purchasing, MIS, conventions, special events, entertainment and wardrobe. All this information is provided in both Excel spreadsheets and comma-separated values format files. The information can be used for a variety of purposes, including targeted direct-mail campaigns, fax broadcasts, and importing into software such as GoldMine, ACT!, Microsoft Outlook and WinFax Pro. Sample Microsoft Word mail-merge template files for both letters and mailing labels are included. Casino City’s Gaming Business Directory CD also includes the current and 26 previous editions of the print version of Casino City’s Gaming Business Directory as PDF (Portable Document Format) files. You can view or search these documents on your PC using the Adobe Acrobat Reader.

CD User Interface All the information available on Casino City’s Gaming Business Directory CD is accessible through an easy-to-use interface. The interface operates using a standard Web browser, and provides direct single-click access to all features from the CD home page. The layout of the CD user interface home page is shown in Figure 1. The CD home page features four major functions:

1. Access all data files provided on the CD and associated documentation. 2. View and search any print edition of Casino City’s Gaming Business Directory on your PC. 3. Use GamingDirectory.com to perform customized searches and download updated data

files. 4. Create mailing labels and letters using templates and Word mail-merge.

Each of these areas links to pages of more detailed information, and additional links are provided to information on importing data, the complete user guide, related products, terms of use and contact information. On a Windows personal computer, the interface is launched automatically when Casino City’s Gaming Business Directory CD is inserted into the CD drive. If you want to use the interface on an Apple or other non-Windows computer, you must open the index.html file located in the CDInterface folder on the CD.

CD Data Table Names and Descriptions The names of the five data tables provided on Casino City’s Gaming Business Directory CD and an overview of the contents of each data table are provided below in Table 1. Appendix A provides a complete listing of the columns included in each data table. Appendix B provides detailed descriptions of the information provided in each column. Each of these tables is provided in both Excel and comma-separated values formats. Information about the specific formats provided and where they are located on Casino City’s Gaming Business Directory CD is included in the CD Files and Folders section of this manual. Table 1: CD Table Names and Descriptions

GamingProperties contains one row for each gaming property included in Casino City’s Gaming Business Directory. Properties covered include casinos, card rooms, racinos, horse tracks, dog tracks, jai-alai frontons, casino cruises and cruise ships. Available information for each property can include: property name; mailing and physical addresses; hours; main, fax, and toll-free telephone numbers; games offered; restaurant, entertainment venue and attraction names; number of gaming machines, table games, poker tables and bingo seats; whether there is a sportsbook and racebook or simulcasting; casino and convention square footage; number of hotel rooms or cabins, restaurants, entertainment venues, parking spaces and employees; website and e-mail addresses; whether or not the property is affiliated with a Native American tribe or First Nation; the type of property; and a principal property contact name and title. The principal property contact provided is typically the general manager.

GamingPropertyContacts contains one row for each gaming property contact. The contacts are classified by department responsibility. Contact name and title are provided, as well as complete information about the property with which the contact is affiliated. A responsibility order is provided based on the contact’s title.

PropertyOwners contains one row for each gaming property owner included in Casino City’s Gaming Business Directory. Available information for each owner can include: owner name; mailing and physical addresses; main, fax, and toll-free telephone numbers; number of gaming properties owned; stock symbol for publicly traded owners; website and e-mail addresses; whether or not the owner is a Native American tribe or First Nation; and a property owner contact name.

PropertyOwnerContacts contains one row for each gaming property owner contact. The contacts are classified by department responsibility. Contact name and title are provided as well as complete information about the property owner with which the contact is affiliated. A responsibility order is provided based on the contact’s title.

PropertyOwnerProperties contains one row for each gaming property owned by a property owner included in Casino City’s Gaming Business Directory. Property owner name is provided as well as complete information about the gaming property with which the owner is affiliated.

CD Files and Folders The information contained on Casino City’s Gaming Business Directory CD is organized into a number of folders. Most of the folders contain a complete set of the data included on the CD in a specific data format. There are also several portable document format (PDF) files contained in the root folder. Microsoft Word Mail-Merge One common use of Casino City’s Gaming Business Directory CD is the production of mailing labels or letters for executive contacts. Word mail-merge is designed to perform both of these tasks, and you will find sample Word mail-merge template files on the CD that should help simplify the process for you. Information about the sample mail-merge template files can be found in Appendix G. This section documents how to perform a mail-merge with Microsoft Word 2000. If you want to create mailing labels, template files are provided for Avery 5160 style labels (30 1-by-25/8-inch labels per sheet), Avery 5161 style labels (20 1-by-4-inch labels per sheet), and Avery 5162 style labels (14 11/3-by-4-inch labels per sheet). Open the template file Avery5160Labels.doc, Avery5161Labels.doc or Avery5162Labels.doc contained in the Templates folder. Then select Mail Merge on the Tools menu to open the Mail Merge Helper dialog box. Since you have already opened a mailing label template file, you can go directly to step two and click on Get Data and then on Open Data Source. Select the data table file you wish to use to create mailing labels and click OK. In step three you merge and format the labels. As part of this step you may select the Query option to choose which contacts to use. The Query option allows you to sort or select records according to criteria you specify. For example, you can select properties with a certain minimum number of table games or hotel rooms. If you want to specify more complex criteria, such as selecting contacts with particular departmental responsibilities, you must use a more sophisticated query tool like Microsoft Query, whose usage is explained in the next section. To create a letter, follow a process similar to the one outlined above for mailing labels, but use the file SampleLetterTemplate.doc. After selecting the appropriate data source for your letter, edit the contents of the sample letter so it has the contents you desire before performing the merge. You can easily delete the body of the letter and type or copy and paste new material from other documents. You can use any of the columns in the selected data table to customize your letter. For example, you can insert the first name of the recipient into a sentence in the body of the letter to make it more personal, or you can include specific information about the property, such as the number of gaming machines, in your letter. Word uses the term “field” to refer to data table columns. Use the Insert Merge Field button on the Mail Merge tool bar to insert columns from the data table. Your letter can also include different words or sentences based on the value of columns in the data table. Just press the Insert Word Field button on the Mail Merge tool bar and select the If…Then…Else… option. Sometimes you may want to create letters or mailing labels for a group of executives that you cannot easily specify using the information on the CD and the Query facility provided by Word. In this situation consider using GamingDirectory.com to create a custom data extract to use with Word mail-merge that contains just the executives you require. The GamingDirectory.com data selection facilities are designed to handle certain types of executive selection criteria not easily specified using Word Query options and the information available on the CD.

Using Microsoft Query with Microsoft Word Mail-Merge Using Microsoft Query with Word mail-merge enables you to easily construct sophisticated queries to select an appropriate set of Gaming Business Directory data. Select Mail Merge on the Tools menu to open the Mail Merge Helper dialog box, click on Get Data under Step Two after setting up the main mail-merge document, and then click on Open Data Source. The Open Data Source screen will appear. Click on the MS Query button to select data using the Microsoft Query tool. Microsoft Query is an optionally installed component of Microsoft Office, so you will only be able to use it if it has been installed. The first screen presented by Microsoft Query is the Choose Data Source screen. If the Gaming Business Directory data you are using is in comma-separated values format you must select “<New Data Source>” and click OK. Data downloaded from GamingDirectory.com is always in comma-separated values format. The Create New Data Source screen will appear if you select <New Data Source>. You must provide a name for the data source, select “Microsoft Text Driver (*.txt, *.csv)” and click Connect. The ODBC Text Setup screen will then be presented. Uncheck “Use Current Directory” and press the Select Directory button to open the Select Directory screen. Select the drive where the directory containing the data is located, select the folder containing the data, and click OK to close the Select Directory screen. Click OK to close the ODBC Text Setup screen and then click OK to close the Create New Data Source screen. On the Choose Data Source screen, select the newly created data source name if you have created a new data source for comma-separated values data. Select MS Access Database to use Access database files on the CD or select Excel Files to use Excel spreadsheets on the CD. Click OK once you have chosen a data source. If you indicate a database or spreadsheet data source, a Select Database or Select Spreadsheet screen will appear. Select the drive where the database or spreadsheet is located, select the folder containing the database or spreadsheet, select the specific database file or spreadsheet if required, and click OK. The Query Wizard Choose Columns screen will now be displayed. Select the Gaming Business Directory table you wish to use, click the “>” button to select all columns in the table, and click Next. The Query Wizard Filter Data screen is displayed. Select a column to be used to filter the data. For example, select the Responsibilities column within the GamingPropertyContacts table if you wish to select contacts based on departmental responsibility. Next, select an operator such as “Contains” and enter a value such as “Security.” To specify additional criteria for the same column, simply select “and” or “or” depending on whether either or both criteria must be satisfied, and specify the criteria, such as “Contains,” “Security or Surveillance.” Thus you can easily request contacts that have either Security or Surveillance responsibilities. To filter based on another column, select it and specify the desired criteria. Once all criteria are specified, click Next. The Query Wizard Sort Order screen will appear. Specify any desired sort order and click Next. The Query Wizard Finish screen will be displayed. We recommend checking “View data or edit query in Microsoft Query” before pressing the Finish button if you have specified a query involving multiple columns.

Importing Contacts into the ACT! Contact Manager You can import data provided on Casino City’s Gaming Business Directory CD and data downloaded from the GamingDirectory.com website into the ACT! contact management system. This section outlines the data import process for the ACT!2000 version of this software. First you should select the data you wish to import into ACT! If you only wish to import a portion of the contacts contained in one of the Gaming Business Directory files, you must first use a tool other than ACT! to create a file that includes just the contacts you want to import. If you want to import a portion of the GamingProperties, GamingPropertyContacts, PropertyOwners or PropertyOwnerContacts tables, then we recommend using the GamingDirectory.com website to select and download the appropriate information. Using GamingDirectory.com you can easily select contacts based on a variety of criteria including geographic location, department, and type of gaming facility. Once you have Gaming Business Directory data ready to import you should open the ACT! database into which the contacts are to be imported. If you want to create a new database for the contacts, use the ACT! menu system to select File > New > ACT Database and provide a name for the new database. If you want to import contacts into an existing database, we recommend you create a backup copy of the database before proceeding so you can restore the original database if necessary. Open the ACT! database, select File > Backup, provide the name of the location where the backup .zip file should be saved, make sure all of the Options tab items are checked, and click Start. You can restore the ACT! database to the backup version by selecting File > Restore, providing the location and name of the backup .zip file, and clicking Start. When ACT! is importing contact records into a new empty database and it encounters a match with the first and last name of a previously imported contact, it normally prompts you to determine if the new contact should be merged with the existing contact record, added as a separate contact record, or skipped and not added. When importing Gaming Business Directory contacts into a new database we recommend that you disable these duplicate checking prompts so that all contacts are added automatically without any intervention. Use the ACT! menu system to select Edit > Define Fields > Advanced Tab and uncheck the “Enable duplicate checking” option for a newly created database. If you have previously imported contacts from the GamingProperties, GamingPropertyContacts, PropertyOwners, or PropertyOwnerContacts tables into ACT! we recommend using the PropertyId, OrganizationId and ContactId values for duplicate checking when importing subsequent updates downloaded from GamingDirectory.com. Use the ACT! menu system to select Edit > Define Fields > Advanced Tab. In the drop-down lists under the heading “Match duplicates using,” select the ACT! field used to hold PropertyId or OrganizationId in the “Match on” drop-down list. Next select the ACT! field used to hold ContactId in the first “Then on” drop-down list and select <None> in the second “Then on” drop-down list. You use the ACT! import wizard to add Gaming Business Directory contacts to an ACT! database. Use the ACT! menu system to select File > Data Exchange > Import. You will then be prompted for the type and name of the file to be imported. If you are importing contact data downloaded from the GamingDirectory.com website, you should select a file type of “Text - delimited” since the file being imported is in comma-separated values format. If you are importing contacts from the CD, you should either select a file type of “dBase III-V” and use the

dBase III format files provided in the dBase folder on the CD or select a file type of “ACT! 3.x, 4.x, or 2000” and use the ACT!2000 files provided in the ACT folder on the CD. There are a variety of factors that will dictate whether it is better to use the dBase III or ACT!2000 version of the data when importing data from the CD. You can only use the ACT!2000 version of the data if it contains all of the information you wish to import. ACT! has limitations on the number of fields so the ACT! version of the data does not contain as much information as the dBase III version of the data. Appendix D documents the limitations on information available in the ACT!2000 version of the data. Additionally, you cannot import data from an ACT! database when it is only available read-only as is the case for files on the CD. This means you must first copy ACT! files from the CD to your computer’s hard drive and change their file attributes so they are no longer read-only before you can import data from them. There are two advantages to importing data from an ACT! database. First, the mapping of fields is simpler. Second, the issues that can arise when importing phone numbers with country codes into ACT! described later in this section are avoided. Once you have specified the file type and file name to import click Next. You should be careful not to click Finish at this point. On the next Import Wizard screen, check Contact Records. If you have selected a file type of “Text - delimited” you should also click Options and uncheck the “Yes, import the first record” option. This prevents the comma-separated values header record from being imported as a contact record. Clicking Next takes you to the subsequent Import Wizard screen. Check the “Don’t use predefined map” option on this screen and click Next. You should now be presented with the Import Wizard Contact Map screen, which you must use to specify how to map Gaming Business Directory column names (shown in the left column) to ACT! fields (shown in the right column). Table 3 shows the names of the Gaming Business Directory columns used in Text-delimited and dBase input formats and the corresponding ACT! field names. When the names do not match exactly, you must explicitly define the mapping. For example, you should locate PostalCode in the first column, click the right column for that row, and select Zip from the dropdown list showing all ACT! fields. This will map the PostalCode input column or field to the Zip ACT! field. When importing from an ACT! database the fields shown in Table 1 will already match directly to the ACT! field names. The mappings shown in Table 3 are suggested mappings. In some cases you may want to alter the mappings if the suggested use does not match your normal use of the fields. For example, the Phone and E-mail provided for Gaming Business Directory contacts are for the company, and not for the individual contact. If you normally keep personal phone or e-mail addresses in the ACT! Phone and E-mail Address fields, then saving company information in these fields may be inappropriate. Doing so would make it hard to merge new Gaming Business Directory contact data into existing contact records since the normal merge process would then replace older personalized data with newer company data for an individual contact. You must decide if you would rather import the TitleLong column or the more compact Title column into ACT! as the Title field. We generally recommend using the shorter Title column. Please note that the columns shown in the table are not present in all Gaming Business Directory files. For example, the PhoneExtension column is only present in the Suppliers and SupplierContacts tables. We recommend that you import the PropertyId or OrganizationId and ContactId columns into ACT! We will change the name of a property contained in the Company field to match a newer preferred property name. Similarly, the name of an individual can change due to a change in marital status or a spelling correction. If the PropertyId or OrganizationId and ContactId fields are included in your ACT! Database, then subsequent updates can be merged based on the more stable PropertyId or OrganizationId and ContactId values. Please note that importing telephone numbers may work differently than you would expect. ACT! will frequently add a country code to the beginning of an imported telephone number based on the contact’s country even if a country code is already present in the telephone number. When this occurs the country code will be duplicated at the beginning of the imported telephone number. Once you have completed the field mappings, click Finish. The data will then be imported into the existing or newly initialized ACT! database. The time required to complete the import will vary based on the number of contacts being imported. Normally only a few minutes are required, but importing a large file such as GamingPropertyContacts, which contains more than 20,000 contacts, may take over an hour. After the import is complete, be sure to restore any settings you altered for duplicate checking to the values in effect before the import was performed. If you want to import a portion of the GamingProperties, GamingPropertyContacts, PropertyOwners or PropertyOwnerContacts tables, then we recommend using the GamingDirectory.com website to select and download the appropriate information. Once you have Gaming Business Directory data ready to import, you should open the GoldMine database into which the contacts are to be imported. If you want to import contacts into an existing database, we recommend you create a backup copy of the database before proceeding so you can restore the original database if necessary. Although GoldMine does not provide a built-in backup utility, a discussion of backup strategies is provided in the GoldMine Help Index under “backing up data.” You can create a backup by copying all files of type *.D* in the folder where the existing database is contained. You can restore the GoldMine database to the backup by restoring the saved files. You use the GoldMine import wizard to add Gaming Business Directory contacts to a GoldMine database. Use the GoldMine menu system to select Tools > Import/Export Wizard > Import Contact Records. Always select “Import a new file.” If you are importing contact data downloaded from the GamingDirectory.com website, you should select a file type of “ASCII File” since the file being imported is in comma-separated values format. If you are importing contacts from the CD, you should select a file type of “DBF File” and use the dBase III format files provided in the dBase directory of the CD. Once you have specified the file type and file name, click Next. Provide the name of the file to be imported and click Next. The Import File Profile screen will now be displayed. If you are importing an ASCII File, the screen will appear as shown in Figure 2 below. You should provide meaningful labels for the columns in the input file that you plan to import into GoldMine before clicking Next. To change a Label Name, right-click on the label name, enter the new label name, and click OK. Label names must be 10 or fewer characters in length. The values shown in “Preview Record: 1” match the column names shown in Appendix A. When these names are 10 or fewer characters in length, we recommend using these values. When they are more than 10 characters in length, we recommend substituting the shorter column names shown in Appendix C.

Once the initial record is selected as the current contact record, use the GoldMine menu system to select Edit > Delete Contact. Make sure all items are selected, and click Delete. When GoldMine imports contact records, it always creates a new contact record for each imported contact. This is done even if the contact has the same name, company name, address, and phone number as an existing contact record. After the import is complete, you can merge

previously existing and new contact records and purge duplicate records using the GoldMine Merge/Purge Wizard. To launch the wizard, use the GoldMine menu system to select Tools > Merge/Purge Records > Merge/Purge Wizard. On the initial Wizard screen, check “Merge/Purge using new criteria” and click Next. On the Select a Filter/Group screen, select “All Contact Records” and click Next to display the Merge/Purge Criteria screen. If you have imported updates to previously imported Gaming Business Directory contacts that included a ContactId, we recommend that you perform a merge/purge operation based on the combination of ContactId and either PropertyId or CompanyId as appropriate. Click New on the Merge/Purge Criteria screen to identify new criteria for selecting records that will be merged or purged. Select the field that contains the ContactId, check Exact Match, assign the criteria a weight of 30 and click OK. Click New on the Merge/Purge Criteria screen again, select the field that contains the PropertyId or CompanyId, check Exact Match, assign the criteria a weight of 30 and click OK. Verify that the Qualifying Weight is set to the default value of 50 and click Next. When both criteria are satisfied for a pair of contact records, the calculated weight will be 30+30=60, which exceeds the Qualifying Weight required to identify a match. On the Merge Method screen, check “Keep record that was created first,” uncheck all other items, and click Next. On the Purge Method screen select “Delete the duplicate record” and click Next. On the Ready to Merge/Purge screen, click Finish. The GoldMine Process Monitor window will appear and show the status of the merge/purge process. You may also want to perform a merge/purge operation for contacts that have matching Contact and Company fields. Simply repeat the process outlined above using the Contact and Company fields.

This section outlines the data import process for Outlook 2000 (version 9.0). First, you should select the data you wish to import into Outlook. If you only wish to import a portion of the contacts contained in one of the Gaming Business Directory files, you must first use a tool other than Outlook to create a file that includes just the contacts you want to import. If you want to import a portion of the GamingProperties, GamingPropertyContacts, PropertyOwners or PropertyOwnerContacts tables, then we recommend using the GamingDirectory.com website to select and download the appropriate information. Using GamingDirectory.com, you can easily select contacts based on a variety of criteria including geographic location, department, and type of gaming facility. Once you have Gaming Business Directory data ready to import into Outlook, you should select the Outlook folder into which the contacts are to be imported. If you want to create a new folder to hold the contacts, right-click on the parent folder that should contain the new folder and select the New Folder menu item. Enter the name for the new folder, select “Contact Items” from the “Folder Contains” drop-down menu, and click OK. You use the Outlook import wizard to add contacts to a contacts folder. Use the Outlook menu system to select File > Import and Export. Select “Import from another program or file” as the action to perform and click Next. Then select “Comma Separated Values (Windows)” as the file type to import from and click Next. Provide the file name of the file to import, check “Allow duplicates to be created,” and click Next. Note that if you use the Browse button to select the file to import you will need to enter *.* in the file name input area for Outlook to display the complete list of files in the currently selected directory. On the next screen select the destination folder and click Next. Click “Map Custom Fields” to open the “Map Custom Fields” screen that shows available Gaming Business Directory columns on the left and Outlook contact fields on the right. Importing Contacts into WinFax Pro You can import data provided on Casino City’s Gaming Business Directory CD and data downloaded from the GamingDirectory.com website into a WinFax Pro phonebook. This section outlines the data import process for WinFax Pro version 10.0. First, you should select the data you wish to import into WinFax Pro. If you only wish to import a portion of the information contained in one of the Gaming Business Directory files, you must first use a tool other than WinFax Pro to create a file that includes just the data you want to import. If you want to import a portion of the GamingProperties, GamingPropertyContacts, PropertyOwners, or PropertyOwnerContacts tables, then we recommend using the GamingDirectory.com website to select and download the appropriate information. Using GamingDirectory.com, you can easily select data based on a variety of criteria including geographic location, department, and type of gaming facility. Once you have Gaming Business Directory data ready to import into WinFax Pro, you should create a new phonebook to contain the imported information unless you wish to use an existing phonebook. To create a new phonebook, use the WinFax Pro Message Manager file menu to select New and then Phonebook. This will launch the phonebook creation wizard. To launch the import phonebook wizard, use the WinFax Pro Message Manager file menu to select Import and then Phonebook. Once the wizard has started, select ASCII (Text file) as the format of the external phonebook to import and press the Next button. To select a file to import from the CD, click the Select button, make sure “Files of type: text files (.txt)” is selected, use the Look-in pull-down menu to select the CommaSeparatedValues folder on the CD, select the desired file from the folder, and press the Open button. Alternatively, you can enter the location of a file you have downloaded using GamingDirectory.com. Once you have selected a file to import, press the Next button. On the following screen, make sure “Columns are separated by a Comma(,)” is selected, uncheck the “Import the first row of data” checkbox, and press the Next button. You must now specify the WinFax phonebook fields that correspond to Gaming Business Directory data columns. The column labeled “ACSII file preview” on the screen shows the names of the Gaming Business Directory columns present in the file you are importing. For each column name shown in Table 6, you should use the “phonebook fields” pull-down menu on the screen to select the corresponding phonebook field name shown in the table. When you are finished specifying the mapping, press the Next button. Finally, select the phonebook into which the information should be imported and press the Finish button to perform the actual import.

Casino City’s Gaming Business Directory CD user interface includes a Web page that links to gaming property cross-reference lists (see Figure 3). There are lists by size and property type. There are lists of properties ranked by size by the number of gaming machines, table games, gaming positions, hotel rooms and poker tables. There are property type lists for Tribal Gaming – Class III, Casino-Hotels, Riverboat Casinos and many other property types. Also included among the property types are facilities that are under construction or in the planning phase. A given property may be included in more than one of the property type lists. Appendix A: Data Table Column Summary and Row Order The following table summarizes the columns containing data in each of the five data tables provided on Casino City’s Gaming Business Directory CD. Within each data table, the columns are ordered in the same sequence as they are listed in the table below. Columns without a checkmark are still present in a data table, but do not contain any actual data. The data tables are provided in eight distinct data formats. There are exceptions to the information provided in this section for three of these formats. Appendix C describes the exceptions for dBase III, Appendix D describes the exceptions for the ACT! contact manager, and Appendix E describes the exceptions for the GoldMine business contact manager. Descriptions of the contents of each data table column can be found in Appendix B. The Type column indicates whether the data in a column is of data type text or data type number. If you convert data from one format to another, you should be sure the appropriate data type is specified for each column during the conversion process. Otherwise, the wrong data type may be used by default. Phone The main telephone number of the company. Additional detail on the format of telephone numbers can be found in Appendix C.

Fax The fax telephone number of the company. Additional detail on the format of telephone numbers can be found in Appendix C.

FaxDigits The digits of the fax telephone number of the company. The number begins with the country code (the digit one within the North American calling area) and excludes any punctuation characters such as parentheses, dashes or blanks. This form of the fax number is suitable for use with fax services such as efax.com.

VenueList A list of the entertainment venues at a gaming property. The facility type may be included within parentheses following the venue name. Multiple venues can be listed, separated from each other by a comma followed by a space.

GamingMachines The number of slot machines, video lottery devices and similar devices available at a gaming property. A value of zero indicates that no devices are available or that the number of devices is not available.

Responsibilities An executive contact’s areas of responsibility, or departments. Multiple departments can be listed, separated from each other by a comma followed by a space. A complete list of department values can be found in Appendix D.

ResponsibilityOrder A number representing the responsibility order of an executive contact within a company. If there is a single executive contact for a gaming property or gaming property owner, then that executive will always have a responsibility order value of 1. If there are two or more executives, then one executive will have a responsibility order value of 1 and the other executives will have responsibility order values of 2, 3, and so on. Executives with titles believed to indicate a greater level of responsibility are assigned smaller responsibility order values, so sorting executives in ascending order by their responsibility order value will place the highest ranking executives first. For example, a president is always given a smaller responsibility order value than a vice president, and a vice president is always given a smaller responsibility order value than a manager. If two titles are viewed as representing the same level of responsibility, they will be assigned different but adjacent responsibility order values. Responsibility order values can be used to select a specific number of executive contacts based on their relative level of responsibility.

PropertyOwnerName The name of a gaming property owner or a management company responsible for a gaming property. Multiple property owner and management company names can be listed, separated from each other by a comma and a space, in the GamingProperties and GamingPropertyContacts tables.

PropertyId The identification number used for a gaming property by Casino City Press.

A sample letter template named SampleLetterTemplate.doc can also be found in the Templates folder on the CD. The sample letter template formats the address heading section of the letter in a fashion similar to that used in the mailing label template. However, the TitleLong field is used rather than Title field and the StateLong field is used rather than the State field. Also, the Country is always placed on a separate line since the limitation of a maximum of seven address lines necessary for the mailing labels can be relaxed within the heading of a letter. A bookmark named Recipient is also created using the name line of the address so it can easily be used again in the salutation line. If you wish to examine the details of the heading of the letter template, simply select the heading portion of the sample letter from within Word, right-click on your mouse while the mouse pointer is positioned over the selected text, and select the Toggle Field Codes menu item.

License Agreement

This is a legal agreement between the end user (“You”) and Information Technology Systems, Inc., its affiliates and subsidiaries (collectively “Casino City Press”). This Agreement is part of a package (the “Package”) that also includes a CD-ROM disc (collectively, the “CD-ROM”) and certain written materials (the “Documentation”).

BY INSTALLING THE CD-ROM, YOU ACKNOWLEDGE THAT YOU HAVE READ ALL OF THE TERMS AND CONDITIONS OF THIS AGREEMENT, UNDERSTAND THEM, AND AGREE TO BE BOUND BY THEM. YOU UNDERSTAND THAT, IF YOU PURCHASED THE PACKAGE FROM AN AUTHORIZED RESELLER OF CASINO CITY PRESS, THAT RESELLER IS NOT AN AGENT OF CASINO CITY PRESS AND IS NOT AUTHORIZED TO MAKE ANY REPRESENTATIONS, CONDITIONS OR WARRANTIES, STATUTORY OR OTHERWISE, ON BEHALF OF CASINO CITY PRESS NOR TO VARY ANY OF THE TERMS AND CONDITIONS OF THIS AGREEMENT.

If You do not agree to the terms of this Agreement, promptly return the entire Package to the place You obtained it for a full refund.

LIMITED LICENSE: You are entitled to use this CD-ROM for Your own use, but may not sell or transfer reproductions of the CD-ROM or Documentation to other parties in any way. You may use one copy of the CD-ROM on a single terminal connected to a single computer. You may not network the CD-ROM or its contents or otherwise use it or its contents on more than one computer or computer terminal at the same time.

If your organization has purchased a GamingDirectory.com corporate subscription, then during the subscription term only, your organization may network the CD-ROM to install the contents of the CD-ROM on a network server for use on more than one computer or computer terminal. Such use must be restricted to organization personnel performing work on behalf of the organization.

OWNERSHIP; COPYRIGHT: Title to the CD-ROM and the Documentation, and patents, copyrights and all other property rights applicable thereto, shall at all times remain solely and exclusively with Casino City Press and its licensors, and You shall not take any action inconsistent with such title. The CD-ROM and Documentation are protected by United States, Canadian and other applicable laws, and by international treaty provisions. Any rights not expressly granted herein are reserved to Casino City Press and its licensors.

OTHER RESTRICTIONS: You may not cause or permit the disclosure, copying, renting, licensing, sublicensing, leasing, disseminating, or otherwise distributing the CD-ROM or the Documentation by any means or in any form, without the prior written consent of Casino City Press. If You modify, enhance, supplement, create a derivative work from, adapt, or translate the contents of the CD-ROM, the results You create shall be subject to the same restrictions as the original CD-ROM.

LIMITED WARRANTY: Casino City Press warrants for a period of ninety (90) days following original retail purchase of the Package that the CD-ROM is free from substantial errors or defects that will materially interfere with the operation of the CD-ROM as described in the Documentation. If You believe You have found any such error or defect in the CD-ROM during the warranty period, (i) if You are in the United States, call Casino City Press at 617-332-2850 between the hours of 9:00 a.m. and 5:00 p.m. Monday through Friday (Eastern Time), holidays excluded, and provide the Product ISBN number; or (ii) if You are outside the United States, send Your original CD-ROM disc to Casino City Press, 95 Wells Avenue, Newton, Massachusetts 02459, United States, together with dated proof of purchase, the Product ISBN number, a brief description of such error or defect, and the address to which it is to be returned. If You have a problem resulting from a manufacturing defect in the CD-ROM, the entire liability of Casino City Press and Your exclusive remedy for breach of this limited warranty shall be replacement of the CD-ROM, within a reasonable period of time and without charge, with a corrected version of the CD-ROM. GENERAL: This Agreement constitutes the entire understanding between Casino City Press and You with respect to the subject matter hereof. Any change to this Agreement must be in writing, signed by Casino City Press and You. Terms and conditions as set forth in any purchase order which differ from, conflict with, or are not included in this Agreement, shall not become part of this Agreement unless specifically accepted by Casino City Press in writing. You shall be responsible for and shall pay, and shall reimburse Casino City Press on request if Casino City Press is required to pay, any sales, use, value added (VAT), consumption or other tax (excluding any tax that is based on the net income of Casino City Press), assessment, duty, tariff, or other fee or charge of any kind or nature that is levied or imposed by any governmental authority on the Package.

EXPORT AND IMPORT COMPLIANCE: In the event You export the CD-ROM or the Documentation from the country in which You first received it, You assume the responsibility for compliance with all applicable export and re-export regulations, as the case may be.

GOVERNING LAW; ARBITRATION: This Agreement shall be governed by, and any arbitration hereunder shall apply, the laws of the Commonwealth of Massachusetts, U.S.A., excluding (i) its conflict of laws principles; (ii) the United Nations Convention on Contracts for the International Sale of Goods; (iii) the 1974 Convention on the Limitation Period in the

Any dispute, controversy, or claim arising out of or relating to this Agreement or to a breach hereof, including its interpretation, performance, or termination, shall be finally resolved by arbitration. The arbitration shall be conducted by three (3) arbiters, one to be appointed by Casino City Press, one to be appointed by You, and a third being nominated by the two arbitrators so selected, or if they cannot agree on a third arbitrator, by the President of the American Arbitration Association (“AAA”). The arbitration shall be conducted in English and in accordance with the commercial arbitration rules of the AAA. The arbitration, including the rendering of the award, shall take place in Boston, Massachusetts, and shall be the exclusive forum for resolving such dispute, controversy, or claim. The decision of the arbitrators shall be binding upon the parties hereto, and the expense of arbitration (including without limitation the award of attorney’s fees to the prevailing party) shall be paid as the arbitrators determine. The decision of the arbitrators shall be executory, and judgment thereon may be entered by any court of competent jurisdiction. Notwithstanding anything contained in this Paragraph to the contrary, Casino City Press shall have the right to institute judicial proceedings against You or anyone acting by, through, or under You, in order to enforce the rights of Casino City Press hereunder through reformation of contract, specific performance, injunction, or similar equitable relief.
