Foreword Dear colleagues We know that safe workplaces “don’t just happen”, and that the challenges require every individual to play their part. Those of you who work offshore must therefore think HSE in every work situa- tion. This means that you must take responsibility for safety and health, and ensure that your workmates do the same. Working on one of Statoil’s offshore installations involves many obligations, and I require the following of you where HSE is con- cerned: You must: be familiar with and obey the relevant regulations and procedures evaluate all risk factors before you carry out a task check that you have all the necessary documentation before starting a job take your time to carry out all work in a safe manner always use the mandatory protective equipment ensure that your workplace is clean and tidy contribute to a good working environment be familiar with the procedures and special duties in an emergency situation report accidents, near misses and hazardous conditions Checklists have proved to be an efficient tool in ensuring that impor- tant safety considerations are not forgotten or overlooked when planning and carrying out work. This booklet does not cover all aspects of some of the operations described - it contains important checklists for a number of selected operations only. Everyone who carries out/prepares for a task must have this book on them at all ti- mes and use it as a final check before/during/after a job. The comple- te requirements are described in governing documents. This booklet is available on all EPN’s offshore installations, and I expect you to use it and go through the checklists whenever necessary. Work safely! Planning the job

I can confirm that:

temporary job stoppage

I can confirm that:

1 the work site is clean and tidy

2 surfaces at a height have been cleared of all material

3 waste and scrap have been sorted accordingly

4 drains have been cleaned

5 gas cylinders have been closed (oxygen and acetylene bottles)

6 air and water to power tools and equipment/ hoses have been shut off

7 electrical tools/equipment have been turned off

8 cables and hoses have been coiled and hung up

9 equipment exposed to the elements has been secured against the weather

10 barriers have been removed (if they are no longer necessary)

11 the work site is safe for all personnel

12 the WP has been signed by the supervisor in charge and the area/system supervisor after theyhave inspected the work site

13 the WP is returned due to the stoppage

14 an incident report (RUH) has been completed ifrelevant

Pre-job checks

I can confirm that:

1 the description of the job is accurate2 lighting is adequate in the work area 3 the area has been defined/declared gas-free

4 equipment that could discharge/leak gas has been defined/identified

5 the appointed person is familiar with the location and use of the nearest first aid equip-ment, fire alarm, emergency shower, fire extinguisher, ESD 2 switch and telephone

6 the workplace is clean and tidy, with adequate access

7 escape routes are clear of obstacles alternative escape routes must have signs

8 equipment/tools are in order: they have been approved, secured and checked

9 signs and barriers are in place10 the WP is valid. The work site has been inspected

by a qualified supervisor and the area/system supervisor. The WP documents will be kept clearly visible at the work site

11 a toolbox talk has been held and all possible risks have been considered/discussed

Breathing apparatus

I can confirm that:

1 the need for breathing apparatus has been considered2 another person/colleague will always be

present when breathing air is used3 portable/transportable compressed air cylinders:

a. are checked before useb. are filled from an air bank/compressor which

satisfies statutory requirementsc. the straps on the back frame have been checked

and are OKd. the straps on portable cylinders/fittings have

been tightened and are securee. the mask has been seal tested without the

demand regulatorf. when the demand regulator is connected and the

cylinder valve opened, the pressure is above 270 barg. the warning tone sounds when the pressure drops

below 50 barh. the breathing apparatus is ready for use when the

above-mentioned checks have been carried out, the demand regulator fitted to the mask and the cylindervalve opened

4 fixed breathing apparatus will be checked before use:

a. the system (e.g. instrumentation air) to which the apparatus has been attached has been approved for use as breathing air

b. hoses and connections for breathing air meet the requirements of the installation

c. the necessary filtering equipment for breathing air is fitted

d. the mask has been seal tested without the demand regulator

Barriers, signs

I can confirm that:

1 the requirements specified on pages iv-viii of this booklet have been met

2 no-entry barriers are clearly indicated through red/white chain/band and a red sign with white text

3 warning barriers are clearly indicated through yellow/black chain/band and a yellow sign with black text (in exceptional cases, barrier tape or yellow and black tape may be used instead)

4 radioactivity is clearly indicated through yellow/black chain/band. When radioactivity warning signs are also in place, this means no entry

5 scaffolding is used to cordon off an area - e.g.hazardous openings, height differences, etc.

6 signs are standard, e.g. for:a. alternative escape routesb. information7 the need for barriers both above and below the

workplace has been considered8 barriers will be removed as soon as the job is

completed

3 the nurse has been notified of work with chemicals (type/quantity/use/who/what/where) – when the work requires a work permit

4 the labelling on chemical packaging is in accordance with the regulations. If in doubt, I will contact my supervisor

5 I know where the nearest eye-wash station/emergency sho-wer is located and I have checked that there is water in it

6 I will wear the necessary perso-nal protective equipment

7 if chemicals are transferred to another appro-ved container, I will ensure that this is also labelledin accordance with the regulations

8 containers for toxic waste are available9 the area is cordoned off and signs are in place10 when the work is finished, the remaining

chemicals will be returned to the original contai-ner. All containers are regarded as hazardous waste if they are not cleaned before disposal

11 an RUH will be written for all undesirable inci-dents - e.g. if the chemical is not labelled in accordance with the regulations (in such cases, the chemical will be returned to land)

Electrical equipment – use of

loose equipment

I can confirm that:

The equipment has been examined visually beforethe work commenced and the following were checked:1 the equipment/tool is complete, the protective

devices are in place, all bolts are in place, and covers and enclosures are in place

2 rubber cables are free of cuts and damage3 plugs are complete4 equipment used outdoors (outside offices and

the living quarters) is marked with the year’s colour

5 faulty equipment is placed in a designated area, or the electrician is contacted

during use:6 the nearest fixed outlet will be used7 cables/leads

a. are not laid through doorwaysb. do not cross fire safety barriers without the

implementation of compensating measuresc. are hung off on S hooks8 when using an adapter consisting of EX-appro-

ved plug and non-EX-proof socket (industrial type), non-EX proof electrical tools are plugged into the adapter at the workplace (max. length of adapter is one metre)

9 the last connection is at the voltage source

Electrical equipment

Personal safety when working on low-voltageplant

A.1 Rotating equipment driven by low-voltagemotors

I can confirm that:

before the work commences:

1 the electrician has disconnected circuit breakers/electrical feeders in the switchboard andchecked that voltage cannot be supplied from other sources

2 the electrician has installed a multilock and the electrical department’s padlock, and has logged this

3 a personal padlock is in place and logged4 voltage testing at the workplace has been

carried out to ensure zero voltage if the task involves work on an electrical installation

5 a test start has been made to ensure that the equipment does not start

while carrying out the work:6 if I stop working before the job is complete, I will

replace my personal padlock with the depart-ment padlock, or hand over the key to my perso-nal padlock to my relief

after finishing work:7 I will remove my personal padlock in the

switchboard and complete the log8 the electrician has checked that the equipment

is ready to be energised

9 the electrician has removed the electrical department’s padlock and multilock

10 the electrician has reconnected circuit bre-akers/electrical feeders ready for energising and carried out logging

A.2 Equipment fed from 230V/400V distributi-on panels

I can confirm that:

before the work commences:1 the electrician has disconnected circuit bre-

akers/electrical feeders in the switchboard and checked that voltage cannot be supplied from other sources

2 the electrician has erected a temporary sign: ‘DANGER Do not operate. Work ongoing’ and attached it to the circuit breakers/-electrical feeders (padlock or other additional safety device can be used)

3 voltage testing at the workplace has beencarried out to ensure zero voltage if the task involves work on an electrical installation

after finishing work:4 the equipment is checked and ready to be

energised5 the electrician has removed signs and any

other additional safety device on the circuit breakers/electrical feeders

6 the electrician has reconnected the circuit bre-aker/electrical feeders if energising is necessary

Entry into containers and tanks

(WR1154/WR1145)

I can confirm that:

1 the requirements specified on pages iv-viii of this booklet have been met

2 an SJA has been carried out3 the entry permit is valid4 isolations have been carried out and lines have

been blanked off5 an entry guard has been designated and knows

how to use the breathing apparatus with buddy mask6 speedy evacuation is possible7 the necessary rescue equipment is ready8 cables/extension leads are earthed and connected

to an approved earth fault protector9 personnel are aware that all use of electricity is

forbidden in tanks where a combustible atmospheremay arise (e.g. clearing away drilling mud, sediment etc). EX-certified pneumatic lighting is permitted.

10 fixed radioactive sources (measuring equipment) are in the closed/covered position

11 the tank has been cleaned, measured for gas and approved for entry

12 barriers and signs are in place13 escape routes are known and clear of obstacles14 noise-reducing measures have been implemented15 the appointed person:

a. has been trained in the use of the gas monitorb. will carry a UHF radio if they cannot maintain visual

contact with the entry guardc. will use the breathing apparatus and any necessary

personal protective equipment

Entry guard (WR1154/WR1145)I can confirm that:

1 I have completed my training

2 I will wear the entry guard vest

3 I have participated in/reviewed the SJA

4 I know where the nearest emergency telephone/fire alarm are located

5 radio contact with the CCR has been established and the appointed person will be observed; the radio will be used in the event of impaired visual contact

6 the appointed person and I have agreed on the stop signal

7 rescue equipment is nearby and ready for use (e.g. safety line, rescue harness, EX-proof flashlight, breathing apparatus with buddy mask and UHF radio). The equipment has been checked as follows:

a. the straps and back frames have been inspected and tested, mask and buddy mask visually inspected and are OK, sealing test of the masks has been carried out, cylinder pressure is over 270 bar and the warning tone sounds when the pressure in the demand regulator falls below 50 bar

b. the breathing apparatus can be used when the above-mentioned checks have been carried out, the demand regulator is fitted to the mask and the cylinder valve is fully open. NB! check that there is positive pressure in the mask before use

8 involved personnel know the location of the escape routes

9 I will not take an active part in the work and will always be present during entry operations

10 my position as entry guard is just outside the entry point, from where I will maintain an overview of the

Fire guard’s duties (WR1154)

I can confirm that:

1 I have undergone the necessary fire guard training

2 I will wear the fire guard vest

3 I have participated in/reviewed the SJA if relevant

4 I have addressed communication/notificationby ensuring that:

a. I know where the nearest fire alarm/telephone are located

b. the appointed person and I have agreed on the stop signal

c. radio contact with the CCR is established in accordance with the WP

5 I have established emergency response by ensuring that:

a. a fire extinguisher is ready to useb. I know where the nearest fire hydrant/fire hose is

locatedc. the nearest fire hydrant/fire hose are in orderd. the location of the release button for the deluge

system/ESD 2 is knowne. involved personnel are familiar with the escape

routes

involved personnel during entry operations11 I will monitor the work situation and the surroun-

dings in such a way that the work can be stopped in the event of a change in circumstances that calls for this

12 I will stop the work in the event of an alarm on the portable gas monitor

13 in connection with interruptions/work breaks I will ensure that:

a. the equipment/workplace are securedb. the CCR/area operator are notified if safety sys-

tems have been disconnected so that these systems can be reset

14 in the event of an undesirable incident I will:

a. notify the appointed person and CCR and activate the alarm

b. commence rescue without exposing myself to undue risk

c. ensure that the equipment/workplace are secured15 in the event of other emergency situations I will:

a. notify the appointed person and stop the workb. ensure that the equipment/workplace are securedc. muster in accordance with emergency procedures

b. the safety systems will only remain out of action after obtaining clearance from the area supervisor

c. as fire guard I will remain in place during heat treatment until the object(s) has/have cooled down to 200oC

d. ignition sources that could cause a fire at a later stage will be turned off/not be left unattended

9 in the event of fire, I will:

a. notify the involved personnel, activate the alarm and inform the CCR

b. commence rescue without exposing myself to undue risk

c. turn off electricity and equipmentd. commence firefighting/ cooling downe. remove gas cylinders10 in the event of another emergency situation,

I will:

a. notify the involved personnel and stop the workb. turn off electricity and equipment and secure the

work sitec. muster in accordance with the emergency

procedures

End of hire period and return to base:
