The samples of the above articles can be held at the office of the undersigned-
1) The technical specification of each item mentioned in a separate sheet and all bidder must submit material safety data sheet along with their quotation. Financial bid will be evaluated after only qualifying the technical specification.
2) Price quoted should be inclusive of all taxes and charges of delivery at the said destination. No separate delivery charge will be paid.
3) The rates quoted by the bidder will remain valid for one year from the date of opening of bid. 4) Each bidder shall submit only one quotation. 5) The tender would be evaluated separately for each item. 6) Supplies are to be made to the office of the undersigned within the 10 days of issuing work order. (at the DTC,
Howrah in District Hospital Campus Howrah) 7) The department reserves the right to reject any or all quotations at its own discretion without assigning any reason
thereof. 8) The rate should be quoted in Indian Rupees ( both in words and figures) only. Corrections, if any , shall be made by
crossing out and rewriting with proper attestation. 9) Self attested copies of valid IT, PT, Trade License are to be attached with the quotations. 10) The successful tenderer must furnish a Security deposit of Rs. 10,000/ ( ten thousand only) of Bank Draft or pay
order favoring “District Health & Family Welfare Samity, RNTCP, Howrah. The security money will be discharged after the date of completion of the performance obligation. Security Deposit will be forfeited for non performance or part performance of the supply within the given time schedule. In case of part performance of work order, for un-perform portion, the quotation inviting authority may have the right to procure the balance quantity from the next bidder with inviting quotation or from open market and in the case the difference in cost arising due to difference in rate quoted by 1st and 2nd lowest bidder may be adjusted against the unpaid bill/s of the tenderer who failed to perform total performance obligation.
11) Payment shall be made after receiving goods in good condition and with requisite quality. 12) Bills in duplicate along with recipient challans is to be furnished to the office of the undersigned for payment. 13) A “Purchase Committee’, constituted by the District Health & Family Welfare Samity (RNTCP) will conduct the
whole tender process. 14) The undersigned reserves the right to accept or reject any or all bidders without assigning any reason whatsoever. 15) The selected suppliers may be asked to produce the sample of the goods selected prior to placement of Order for
supply, to technical committee to ensure its quality and specification 16) If sub-standard or non-specified items are supplied, the undersigned reserves the right to reject the order and to
forfeit the security money. 17) The bidder himself or his authorized representative must be present at the time of opening of the quotation.

1) Price quoted should be inclusive of all taxes and charges of delivery at the said destination No separate delivery charge will be paid.
2) The rates quoted by the bidder will remain valid for one year from the date of opening of bid. 3) Each bidder shall submit only one quotation.
4) The quotation would be evaluated separately for each item. 5) Supplies are to be made to the office of the undersigned within the 10 days of issuing work order (at the
DTC, Howrah in District Hospital Campus Howrah) 6) The department reserves the right to reject any or all quotations at its shown discretion without
assigning any reason thereof. 7) The rate should be quoted in Indian Rupees ( both in words and figures) only. Corrections, if any , shall
be made by crossing out and rewriting with proper attestation. 8) Self attested copies of valid IT, PT, Trade License are to be attached with the quotations. 9) In case of delayed servicing (servicing after 48 hrs. of lodging complaint ) and inability to repair the
fault , the charge of servicing from external agencies if required will be deducted from the AMC value. 10) Payment shall be made after receiving goods in good condition and with requisite quality. 11) Bills in duplicate along with recipient challans is to be furnished to the office of the undersigned for
payment. 12) A “Purchase Committee’, constituted by the District Health & Family Welfare Samity (RNTCP) will
conduct the whole tender / quotation process. 13) The undersigned reserves the right to accept or reject any or all bidders without assigning any reason
whatsoever. 14) The selected suppliers may be asked to produce the sample of the goods selected prior to placement of

order and to forfeit the security money. 16) The bidder himself or his authorized representative may be present at the time of opening of the
