WHY THIS PROJECT?

An Introduction to Odisha and Healthcare in the State

Our Camps were held in Sambalpur and Bhawanipatna (Kalahandi district) in

Odisha State in India.

Odisha (formerly Orissa) is among the poorer states in India. Odisha is home to a

large tribal populace, or Adivasis (known officially as Scheduled Tribes in India),

and Scheduled Castes. The western districts such as Kalahandi have a majority

population of SC/ST. Typically, the districts and its people tend to be the poorest,

with very little access to healthcare. Some statistics on the state of Odisha

Population: 36,706,920

513,897 blind people

1,798,639 blind Eyes (Bilateral+Unilateral)

996,446 cataract blind eyes

343,602 new blind eyes due to cataract/yr.

Currently doing 86,386 cataract surgeries per year

To match the Incidence Odisha need to do 257,216 more cataract surgeries

per year. 3 times more….

Trilochan Netralaya and Dr Shiva

The second reason for picking Odisha and these camps for our maiden project was

Dr Shivaprasad Sahoo and Trilochan Netralaya (TN). In TN, The Vision Mission has

found an ideal complementary partner for achieving its objective. Dr Shivaprasad

has worked tirelessly and selflessly over the years in trying to eradicate curable

blindness in the region. Dr Shiva, as he is commonly known, has performed over

35,000 cataract surgeries in his career – a staggering number by any standards. He

has a team of equally dedicated staff at his establishment who match his diligence

and sense of duty.

TN has existing camps in Sambalpur and Kalahandi, and therefore also possesses

the methods to bring in people for camps. Often, poor patients have to be brought

in from a radius of 150 kms from the site of the camp. They are illiterate and would

have no money even to pay for transport. That makes the task of educating them

about the benefits of such a surgery, and bringing them over to the camp site quite

a challenging one in itself. Flying in from Singapore directly, we would be unable to

do this. TN, on the other hand, is very well equipped to do the same.

On the flipside, what TN lacks is something that we have in abundance: Namely,

expertise of first world doctors who are well qualified, and are able to look at a

variety of problems related to the eye; money and donors.

We have also engaged with Dr Shiva and TN in the past and were familiar with

their work and methods.

THE ONE WEEK CAMP: A RUN THROUGH

Day 1: We landed in Sambalpur, Odisha on Sunday

9th February 2014, via train taken from Howrah.

After a quick freshen-up, we went down to our base

of operation at Trilochan Netralaya. The first day was

spent at TN’s more remote site at a rustic,

countryside facility – called Betty’s Ashram. Over 45

cataract surgeries completed.

Day 2: Post-Op Viewing and Care at Betty’s Ashram.

All patients were fine with no complications with

good vision posteroperatively. The patients were

given breakfast, and then sent off in TNs buses and

vans back to their locations. Went over to the main

site in Sambalpur town where many patients had

come down for the clinic. Dr Shiva had called down

the more unique cases for whom he desired a second

opinion from the Singapore doctors.

Day 3: More surgeries conducted at Sambalpur. Left

for Bhawanipatna in Kalahandi district later in the

afternoon, the site of our third camp.

Day 4: Close to 76 surgeries conducted in

Bhawanipatna in a single day. Also saw some clinic

patients. Left back for Sambalpur later in the day.

Day 5: More surgeries performed taking the grand

total to 300. Had an interaction with students from

Master of Social Work programme, Sambalpur

University. Discussions also with Dr Shiva and team;

brainstormed on what more we could do and how we

could take this initiative forward.

Left by train to Bhubaneswar in the evening.

HOW WE HAVE HELPED

Free Cataract Surgeries

300 people in Sambalpur and Kalahandi

districts spent years of their life suffering

without the benefit of proper sight,

because they did not have the equivalent

of $30 to cure themselves. They can now

see! The donors money helped fund not

just the surgery, but also the following:

- Travel costs for bring the patients to the camp, and then back to their village

- Stay and Accommodation – for the purpose of rest and recuperation. Given

that the villages are a 100 kms away, it would not be possible for the

patients to go back home and them return 3 days later for their Post-Op

checkup. Therefore, facilities need to be in place to ensure they can stay at

the camp.

- Food – during their stay at the camp

- Lenses

- Cost of nurses and other staff

- Cost of equipment, medicines

Outpatients and Other Ophthalmic Assistance

Dr Shiva also has an outpatient clinic facility where patients with a wide range of

conditions are seen by himself and another doctor, Dr Shushant. He took the

opportunity to bring back some of the more complex cases - from various

subspecialties such as neuro-ophthalmology, pediatric ophthalmology, cornea,

glaucoma, uveitis and retina - for a second opinion by our team and discussion for

further management. Majority of the patients could be managed in the local setting,

although there were a handful of patients who required referral to tertiary centers

in Bhubaneswar.

Raising Awareness. Sowing the Seeds for Future Work

The more unexpected parts of the trip included our interactions with young and

enthusiastic people. The doctors, nurses and support staff , the students from

Sambalpur University, and various others that we met, showcased a great amount

of energy to get things done. The Vision Mission team was inspired by Dr Shiva and

his team of dedicated staff. The doctors in the team were also very impressed and

humbled by the high level of skill exuded by Dr Shiva in managing complicated

cataracts. What is needed now is backing in the form of correct training, mentoring

and funding so that TN can provide holistic high-quality ophthalmic care beyond

just cataracts. It goes without saying that

much more can be accomplished in future

mission trips in the form of training the staff,

and augmenting their skillsets. It did get us

thinking that the setting up of some form of

tele-medicine facility in those areas could

greatly enhance the quality of service (more on

that in the section on Future Projects).

FUTURE PROJECTS

Over a few rounds of discussions with Dr Shiva, and other people on the ground, we worked out the

following possible future initiatives:

(1) Tele-Medicine for Sambalpur: Try to setup mechanisms using the state of the art in tele-

medicine facilities, such that international experts will be able to assist the doctors and

institutes in Odisha through remote means. If this succeeds, then regular assistance can be

provided, if necessary, on a weekly basis.

(2) Providing sub-speciality expertise. Sambalpur still needs to have regular visits by sub-

speciality ophthalmologists to help deal with specific and unique cases. This includes doctors

having expertise in glaucoma and phacoemulsification.

(3) Funding a further 500-1000 surgeries per year. To ensure the good work does not stop, and

to ensure that we can forever eradicate curable blindness from the region, we wish to try

and find the means to commit to regular surgeries of the order of 500-1000 atleast. We wish

to consider increasing such numbers in the near future.
