EFFECTIVE 8D PROBLEM SOLVING REPORTING Date : 17-18 September, 2014 Time : 9.00 am - 5.00pm Venue : FMM NS Branch, Seremban 2 INTRODUCTION The 8D (Eight Disciplines) Problem Solving Process is a team oriented and structured problem solving methodology that is mainly used to identify, correct and eliminate recurring problems. It focuses on the origin of the problem by deter- mining root causes and estab- lishes permanent corrective and preventive actions. 8D is a problem-solving methodology for prod- uct and process improvement. It is structured into eight disciplines, emphasizing team synergy. The team as whole is better and smarter than the quality sum of the individuals. Each discipline is supported by a checklist of assessment questions, such as "what is wrong with what", "what, when, where, how much". It always is a common mistake of problem solving teams to jump to conclusions and make decisions based on feelings and non- verifiable observations. The team must stay focused on the facts and what solutions can be linked to these facts. The 8 Disciplines (8D) process is a problem solving tool popularly used in responding to customer returns, suppliers and internal issues. Its effectiveness stems from the fact that it helps to incorporate all the important aspects of problem solving, i.e., the containment of the problem, root cause analysis, problem correction, and problem prevention to prevent recurrence. OBJECTIVES At the end of the program the participants will be able to: Understand the problem solving tools including tech- niques, charts and diagrams Prepare for the 8D process Establish a problem solving team Write the problem statement and problem description Develop an Interim Containment Action Define and verify the Root Cause and Escape Point Choose and verify Permanent Corrective Actions Prevent Recurrence Celebrate success CONTENT 1.
