FUSION LABS EMPLOYEE BONUS REFERRAL PROGRAM I. Purpose The purpose of the Employee Referral Bonus Program is to incent employees (Full time equivalents (FTEs)) to submit perspective qualified job candidates for employment with Fusion Labs. Hired referrals are required to successfully complete the probationary period prior to the incentive award to be given. II. Eligibility and Participation Applicant ‐ Applicants are persons not currently employed at Fusion Labs. Referral Bonus Amount The Referral Bonus Program is operated under the authority of the CEO. Under these guidelines, an employee may receive multiple bonuses, the total of which may not exceed $3,000 in any fiscal year. FUSION LABS EMPLOYEE BONUS REFERRAL PROGRAM

I. Purpose

The purpose of the Employee Referral Bonus Program is to incent employees (Full time equivalents (FTEs)) to submit perspective qualified job candidates for employment with Fusion Labs. Hired referrals are required to successfully complete the probationary period prior to the incentive award to be given.

II. Eligibility and Participation

Applicant ‐ Applicants are persons not currently employed at Fusion Labs.

Referring Employee ‐ All employees in FTE equivalent positions are eligible to receive a referral bonus with the exception of the following:

Employees whose regular, recurring jobs include the recruitment ofemployees.

Selecting manager/supervisor or other persons associated with the selection of the candidate.

All HR Department employees Family members of a Fusion Labs employee, or owner

III. Referral Bonus Amount

The Referral Bonus Program is operated under the authority of the CEO.

Under these guidelines, an employee may receive multiple bonuses, the total of which may not exceed $3,000 in any fiscal year. A bonus for a hired referral that successfully completes probation is $1,000.

The Referral bonus will be paid after the referral has successfully completed the ninety-day probationary period. Bonus payments may be paid in a lump sum, or monthly after the successful completion of the new employee probationary period.

IV. Referral Bonus Program Administration

The Referral Program operates in accordance with the following requirements:

Referral bonuses are awarded at the discretion of management with CEO final approval authority.

The employee making the referral will complete a form provided by Human Resources (HR) prior to the candidate interviewing, or no later than upon the applicant submitting an application for employment. The purpose of the form is to identify to whom the incentive award is to be paid.

Employees selected for eligible positions must successfully complete the probationary period of three months

Both the referring employee and the referred candidate must be employed by Fusion Labs when a referral bonus is eligible for payment.
