End-Of-The-Year-Tip

As we approach the end of the school year, it is an important to address critical issues that can provide a proactive approach for the upcoming school year. PBIS/Culture/Climate teams can use this action planning

template to target and address school-wide, classroom and individual student behavior management support.

Priority (H, M, L)

Objective Activity Who When

Prepare students for next grade/teachers

Prepare teachers for new students

Prepare students for transition to new school

Teach/Pre-Correct expectations and routines for end of school year

Review and reinforce expected behaviors

Arranged events to celebrate successes of all students and staff

Survey staff on status of school-wide PBIS (SAS)

Review and evaluate office referral and/or discipline data for year

Revise/evaluate PBIS/Tiered Fidelity Inventory (TFI) action plans for year

Prepare proactive transition plans for at-risk (high risk) students
