INTRODUÇÃO
Jovem de 15 anos referenciada à consulta deCadiologia Pediátrica por sopro cardíaco, sem
sintomatologia associada.Analiticamente sem alterações relevantes
nomeadamente eosinofilia. O electrocardiogramarevelou ritmo sinusal, com frequência de 75pulsações por minuto, dilatação bi-auricular edeficiente progressão de ondas R nas derivaçõesprécordiais.
O ecocardiograma mostrou dilataçãobi-auricular, ventrículo esquerdo (VE) dilatadode configuração esférica, com função sistólicaglobal conservada, ventrículo direito (VD) aformar o ápex cardíaco por posicionamentoanterior ao ventrículo esquerdo em forma de“dedo de luva” (Fig. 1e 2). As paredes de ambosos ventrículos não mostravam hipertrofia oualterações da ecogenicidade. O estudo Dopplerdo fluxo da câmara de entrada do ventrículoesquerdo apresentava padrão restritivo.
Realizou cateterismo cardíaco que mostrouhipertensão pulmonar ligeira, padrão restritivo doventrículo esquerdo (curva em dip and plateaucom pressão telediastólica do VE=16mmHg),ventrículo direito de morfologia atípicacircundando o ápex do VE (Fig. 3).
Na sequência do estudo etiológico demiocardiopatia realizou ressonância magnéticacardíaca que confirmou a presença de dilatação
INTRODUCTION
A15-year-old girl was referred for PediatricCardiology consultation for a heart murmur,
with no associated symptoms.Laboratory tests showed no relevant
alterations, including eosinophilia. Theelectrocardiogram revealed sinus rhythm, heartrate of 75 bpm, biatrial dilatation and poor Rwave progression in the precordial leads.
Echocardiography showed biatrial dilatation,a dilated left ventricle (LV) with a sphericalgeometry, preserved global systolic function, andthe right ventricle (RV) forming the cardiac apexdue to its position anterior to the LV, and shapedlike the finger of a glove (Figs. 1 and 2). The wallsof both ventricles showed no hypertrophy oralterations in echogenicity. Doppler flow study ofthe LV inflow tract showed a restrictive pattern.
Cardiac catheterization showed mildpulmonary hypertension, LV restrictive pattern(dip and plateau pattern, with end-diastolicpressure of 16 mmHg), and RV with atypicalmorphology wrapped round the apex of the LV(Fig. 3).
To determine the etiology of thecardiomyopathy, cardiac magnetic resonanceimaging was performed, which confirmed LVdilatation with spherical geometry, preservedglobal function and apical septal and apicalanterior hypokinesia; moderate mitral 1339

do VE com morfologia esférica, função globalmantida e hipocinésia septal apical e anteriorapical; insuficiência mitral moderada; ventrículodireito com posicionamento anómalo anterior aoVE na sua porção apical (imagem em “dedo deluva”) (Fig. 4); na sequência de realce tardio(Fig. 5) observou-se hipercaptação localizada decontraste no apex e no subendocárdio dossegmentos septal mediano e apical, semenvolvimento miocárdico. Estes aspectosimagiológicos são compatíveis com fibroseendomiocárdica.
CONCLUSÕES
No caso em análise, há a salientar a ausênciade sintomatologia tendo a ecocardiografia
regurgitation; and the RV positioned abnormallyanterior to the LV at its apex (shaped like thefinger of a glove) (Fig. 4). Delayed enhancement(Fig. 5) revealed localized hyperfixation ofcontrast in the apex and the subendocardium ofthe mid and apical segments of the septum, withno myocardial involvement. These imagingfeatures are compatible with endomyocardialfibrosis.
CONCLUSIONS
Of note in the case presented is the absence ofsymptoms, with echocardiography revealingstructural heart disease and a pattern ofrestrictive cardiomyopathy. Cardiac magneticresonance imaging to investigate the etiology ofthe cardiomyopathy led to a diagnosis ofendomyocardial fibrosis, an uncommonpathology. The case also highlights theimportance of using gadolinium contrast fordelayed enhancement study to assess the extentof fibrosis at the endocardial level.

revelado cardiopatia estrutural e padrão demiocardiopatia restritiva. A ressonânciamagnética cardíaca realizada no âmbito dainvestigação etiológica de miocardiopatia,permitiu fazer o diagnóstico de uma patologiapouco frequente como é a fibrose endo-miocárdica. Destaca-se ainda a importância douso de gadolínio para estudo de realce tardio naavaliação da extensão da processo fibrótico anível endocárdico.
