FIELD TRIP PERMISSION FORM Your child is going on a field trip . . .

Location The Pittsburgh Zoo and Aquarium
Cost $10.00 for each student (and $10.00 per adult chaperone if you are willing)
Transportation Included in the cost mentioned above.
Notes
• Parents are needed. If you would like to go, please indicate that in writing on the bottom of this form.
• Parents will be asked to chaperone small groups of children, including their own.
• Students should either bring money to buy lunch or pack a lunch in a plastic bag or appropriate container. Please do not pack lunch in a paper bag.
• Students will not necessarily be visiting the gift shop, although sometimes there is an opportunity to browse. Please discuss with your child your policy about spending money on souvenirs.
• Students should dress appropriately for the weather and in accordance with standard school rules. As Deer Valley veterans, we should all know the importance of being prepared for all sorts of weather.
