1. CEO and senior management support for worksite wellness
A case for worksite wellness has been clearly articulated.
A Worksite Wellness champion has been appointed.
2. Establish a Workplace Wellness Team
Seek membership from all departments as well as management and support staff. The composition of the wellness team should reflect the diversity of the em-ployer’s workforce.
3. Collect data to inform employer wellness efforts
Gather demographic data, such as age, gender, etc on your workforce.
Contact your health insurance provider and broker to determine what support they may provide you.
Conduct a workplace audit to determine current and past efforts taken to support employee wellness.
Conduct an employee interest survey to determine employee needs and interests.
Ask employees to complete a confidential Health Risk Assessment to determine to gather information about your employee population.
Examine existing company data such as use of sick leave, health insurance claims data, disability insurance claims, worker’s compensation claims, etc.

Collect de-identified employee biomet-ric screenings such as blood pressure, body mass index, etc.
4. Develop an operating plan
Prepare vision statement.
Establish goals that describe the desired outcome of the program.
Identify objectives that are Specific, Measurable, Achievable, Realistic, and Time specific.
Include an implementation timeline with detailed information concerning when wellness activities will occur and individual responsible for executing the plan.
Marketing or Communication Plan that describes methods that will be used to inform employees about the plan.
Detailed Budget describing the revenue needed to execute the plan.
Measurement and Evaluation Plan that describes how success will be measured.
Review legal implications of proposed workplace wellness program with legal attorney to insure compliance with federal (ADA, HIPAA, ADEA, NLRA) and state laws.
Describe how the wellness program fits into the company’s health benefit and compensation strategy as well as busi-ness goals.

6. Create a supportive environment for em-ployees in their efforts to lead healthier lifestyles
Revise personnel policies and procedures if needed.
Offer rewards and incentives.
Make renovations to the worksite to pro-vide a supportive environment (e.g. stair-case renovation to support walking pro-gram).
7. Carefully Evaluate Outcomes
Evaluate outcomes for each intervention to determine which interventions are ef-fective and provide the business case for continuing.
SMOKING CESSATION
Policy
Develop and implement a written smok-ing and tobacco-free workplace policy that addresses tobacco use in worksite buildings, on the worksite grounds, as well as riding in company vehicles.
Supportive environment
Ban tobacco vending machines, tobacco promotions, tobacco promotional offers, or tobacco prizes from the worksite.
