Statement of NondiscriminationGila River Telecommunications, Inc., is the recipient of federal financial assistance from the Rural Utilities Service, an agency of the United States Department of Agriculture, and is subject to the provisions of Title VI of the Civil Rights Act of 1964, as amended, Section 504 of the Rehabilitation Act of 1973, as amended, and the rules and regulations of the United States Department of Agriculture which provides that no person in the United States on the basis of race, color, national origin, age or disability shall be excluded from participation in, admission or access to, denied the benefits or, otherwise be subjected to discrimination under any of this organization’s program or activities.

The laboratory offers a complete spectrum of tests using up-to-date analytical equipment. Most tests are completed in-house; an outside laboratory handles those that are not. Current computer technology is used to monitor tests and report results. The laboratory has an extensive internal quality control program, in addition to participating in proficiency testing programs. The laboratories are accredited by The Joint Commission (TJC) and certified under the Clinical Laboratory Improvement Act (CLIA 1988).

Pharmacy and public health nursing services can be obtained through the different Gila River Care locations. In addition, certain services can be accessed at Hu Hu Kam Memorial Hospital, which includes laboratory, x-ray, imaging, physical therapy, optometry, dental, behavioral health, the Life Center, and dialysis services.

GRTI was established on July 6, 1988 for the purpose of providing the Gila River Indian Community with telephone service and other telecommunications services.

Today GRTI provides telephone and high-speed internet to both residential and business customers. GRTI offers a Computer Purchase Program to bring affordable technology into the homes of Indian Country. It is projected to have all of Gila River serviced with Fiber-To-The-Home (FTTH) by 2019.

Gila River Telecommunications, Inc. is also a 25% owner of the Gila River Cellular General Partnership with Verizon Wireless, serving the Rural Service Area 5 in Arizona. The service area includes a portion of the Interstate 10 corridor between Phoenix and Tucson , as well as all of Pinal and Gila counties.

Since 2006, GRTI has created several subsidiaries, Native Technology Solutions, Alluvion Communications, and the first Low-Power Television station in Indian Country, Gila River Broadcasting Corporation. Numbers to keep on hand!You may not know about these numbers, but they are great to keep on hand in case you need a little assistance.

Directory Assistance provides any listing in the United States. Every

month the first 2 calls are free, after that 45¢ per call.

AZ 511 System gives up-to-date weather and road conditions for

transit, airport, tourism and metro regions.

GRTI Customer Service can be reached for service repairs,

disconnects & reconnects, relocation service orders and general information. This toll free call only applies within the GRTI telephone exchange.

Deaf, hard of hearing, or speech impaired telephone users can call to

be connected to TTY services. The operator then relays the conversation for the two callers.

AZ Blue Stake should be used for locating underground facilities such

as telephone wires, gas lines, pipelines. You MUST call in before you dig. Utilities will be located and marked for you at no cost by the utility companies within 48 hours. This is the National Call Before You Dig number.

Emergencies happen. 911 works from any landline, cell phone or pay phone.

If you are not sure your situation is an emergency, call 911 and the dispatcher will give you assistance.

Billing DisputesGRTI retains the right to disconnect service for non-payment of telephone charges. A customer may dispute a bill, or any charge on a bill, by calling, writing or visiting a Customer Service & Sales Representative at GRTI offices. The Customer Service & Sales Representative will investigate the question or dispute and report the findings to the customer. GRTI will not disconnect service for non-payment of a disputed charge while GRTI investigates, provided that all other undisputed charges are paid when due.

If the customer is dissatisfied with the results of the investigation, he or she may request a meeting with the

Customer Service & Sales Supervisor. If still dissatisfied, the dispute may be taken to the General Manager, who will have the final authority to resolve the dispute. Ay any time in the process the customer will have the opportunity to present any facts or documents relating to the disputed bill.

Only after this process is completed, and if the matter is resolved in GRTI’s favor, will service be disconnected.

The Gila River Indian Community Council has the authority to hear and resolve billing disputes after the procedure described above has been completed.
