Dear Friends in Christ,

Our 2015 Annual Report theme reflects the “growth” Lutheran Homes Society (LHS) has experienced in the last year. It also embraces the sense of “community” the organization strives to build inside its walls as well as throughout the larger communities we serve. Finally, our theme celebrates the “hope” our ministries create for residents, patients, and families every day.

For LHS, the year 2015 was marked by significant transitions in leadership, including the installation of Rick Marshall as the new President/CEO. The year also saw the expansion of its caring ministry to even more people in need. Beyond the familiar long-term care facilities, affordable housing communities, and family and youth services, we now serve a lower income community in Toledo through the LHS Adult Day Center. LHS Family & Youth Services programming is expanding from its traditional focus on troubled youth to include adults with disabilities and mental health counseling. A pulmonology program was developed and is underway at Lutheran Village at Wolf Creek, and a Parkinson’s disease management program is proving successful at Lutheran Home at Toledo. Furthermore, in an effort to enable those served by LHS to remain in their own homes, outpatient rehabilitation is growing at our four care campuses, and we celebrated the first anniversary of our home health venture.

This ministry growth is not stopping, either. A new Strategic Plan for the next five years was developed and is already helping move the organization forward. We are working on telehealth, both for our long-term care facilities and elders living in their own homes, as a means to help reduce medical care costs. To be sure, good stewardship has always been an LHS Core Value, but in the past year, refinancing has made an additional $5 million available to improve our facilities and to better positon us for the future.

God blessed LHS with the opportunity to serve 3,354 residents, patients, and clients during the year, through 1,005 staff. In addition, 959 volunteers provided 25,234 hours of service to enhance the lives of our elders and youth. LHS also provided in excess of $4.7 million in charitable care and community benefit.

We are grateful to the congregations and individuals who have shared this ministry through their volunteering, contributions, and prayers of support. God has richly blessed us, enabling us to grow, strengthen our community, and create hope.

Highlights

W. Richard “Rick” Marshall became the President and Chief Executive Officer of Lutheran Homes Society on February 1, 2015. Mr. Marshall came to LHS from Christian Care Communities, Inc. in Louisville, Kentucky, where he had most recently served as Executive Vice President and Chief Operating Officer. He has over 20 years experience in health care finance and operations, with direct experience in operations, strategic planning, information systems implementation, bond debt financing, and banking relationships. Soon after his arrival, he began developing a strategic plan to direct the organization’s activities for the next five years. He was installed as President/CEO during the 2015 Annual Meeting.

Highlights

Lutheran Homes Society expanded its ministry outreach in 2015 to include adult day services. The LHS Adult Day Center (ADC) opened at the beginning of September and is located on the campus of the Warren AME Church, adjacent to the J. Frank Troy Senior Center, near downtown Toledo. The ADC provides cost-effective care for elders during the day and follows a medical model that combines expert health services with social and therapeutic activities. Care is individualized for each participant. The ADC provides services that include nutritious meals, care management by a registered nurse, monthly health screens, nursing care, medication assistance, personal care, therapy, and a variety of activities. Community support for the Adult Day Center has been very positive, with participation growing much quicker than anticipated.

Music & Memory Program Benefits ResidentsLutheran Home at Toledo and Lutheran Home at Napoleon became Music & Memory SM certified care facilities. This personalized program enables those struggling with Alzheimer’s, dementia, and other cognitive and physical challenges to reconnect with the world through music-triggered memories. According to research, music not only provides comfort and enjoyment, but it also increases cooperation and attention, reduces agitation, enhances engagement and socialization, and helps reduce reliance on medications for residents. Activity staff, trained in the Music & Memory program, talk with residents and their families to determine the artists, songs, and genres that will be most meaningful and enjoyable for them. A personalized playlist is then created and downloaded onto an iPod for resident listening and enjoyment.

Henry, Wood, and Ottawa counties, but the agency has since expanded its service area to include Sandusky and Erie counties. The joint venture, an equal partnership of Lutheran Homes Society and Alternate Solutions Health Network, serves as a vehicle to deliver care at the lowest cost and in the most independent setting.

A Pulmonary Rehabilitation Program, under the direction of Dr. Hesham H. Elgamal, a prominent local pulmonologist, has been implemented at Lutheran Village at Wolf Creek. The program consists of a multi-disciplinary team of specialists who provide evidence-based rehabilitative care to people with chronic breathing issues. To increase the program’s effectiveness, Lutheran Village at Wolf Creek hired a full-time Respiratory Therapist to oversee daily operations. The comprehensive program provides an individualized treatment plan that focuses on patient education, as well as clinical evaluation, exercise training, nutritional counseling, pain management, and psychological counseling and support.

LHS Home Health Agency Celebrates One-Year AnniversaryIn March 2015, Northwest Ohio Home Care Partners celebrated one year of providing skilled nursing and therapy services to clients in northwest Ohio. Through December, 357 clients had received care in the comfort of their own homes. Nurses and Therapists provide clinical care, as well as motivation, encouragement, and health education. When the home health agency began its operations, the coverage area was Lucas,

LHS Honors Legacy of the Rev. Gerald LabuhnLutheran Homes Society lost an iconic figure in its history when the Rev. Gerald H. Labuhn, Executive Director from 1964 to 1994 and Executive Director Emeritus, passed away on September 23, 2015. Pastor Labuhn faithfully served LHS for more than 60 years. His relationship with the Society began as a seminary student, while he served two years as the “Big Boys” attendant in the Lutheran Orphans’ Home and as student pastor at First St. John Lutheran Church next door. Pastor Labuhn was also a volunteer member of several Board of Trustee committees. He was part of the 1962 committee that planned the original building now housing The Labuhn Center, the short-term skilled nursing and rehabilitation center on the Lutheran Home at Toledo campus named in his honor. Upon his retirement as a full-time staff member, Pastor Labuhn served as the LHS Archivist and Museum Curator until 2012.

Lutheran Home at Napoleon Celebrates 30-Year AnniversaryLutheran Home at Napoleon commemorated a significant milestone in 2015 when the campus celebrated 30 years of service in the Napoleon community. The 99-bed nursing home opened its doors in January 1985. This was the first time Lutheran Homes Society had expanded its elderly care services outward from its original campus in east Toledo. The nursing home quickly filled to capacity and developed a reputation for care and services unsurpassed by other nursing homes in the area. An anniversary reception was held in September to celebrate the years of service.

At the dedication of The Labuhn Center in 2012, Pastor Labuhn (center) was joined by his wife, Ilean, and the Rev. Dr. Marcus Lohrmann, Bishop of the NW Ohio Synod, ELCA.

While serving as Executive Director, Pastor Labuhn was responsible for expanding youth and elderly services beyond the original Toledo campus.

LHS Refinances Debt and Generates Funds for RenovationsLutheran Homes Society closed on the refinancing of the majority of its long-term debt in October. Due to the organization’s strong operating performance and financial stability, LHS was able to take advantage of a favorable interest rate environment to lock in significant debt service savings and borrow additional money to fund capital improvements. Working with Citizens Bank, LHS refinanced existing debt and borrowed an additional $5 million, while still lowering its debt service payments. LHS will use the new $5 million for a variety of projects. Lutheran Home at Napoleon and Lutheran Home at Toledo will undergo extensive renovations to their skilled nursing units to

provide warmer, more home-like environments with more amenities that residents and patients have come to expect in today’s health care market. In addition, the therapy gym on the Napoleon campus will be significantly expanded. Many other projects will be less visible, but they will ensure that LHS buildings remain viable for years to come.

The mission of Lutheran Homes Society has always included providing care to the underserved – historically youth and elders. As the scope of whom we serve has broadened, our commitment to serving those in need has not diminished. We spend a significant amount of LHS funds to ensure individuals receive necessary care, housing and services.

In 2015, LHS provided in excess of $4.7 million in charitable care and community benefit. Through charitable care, LHS subsidizes the cost of resident care. Charity care can be defined as the difference between Lutheran Homes Society’s cost of services and the rates paid by third party insurers, including private pay and governmental agencies.

Through community benefit, LHS provides free or discounted care and services to the uninsured, underinsured, low-income, and indigent residents. As a non-profit organization, we also provide or participate in numerous educational and community-based programs that improve the overall health and wellness of the communities we serve. LHS is a committed to improving the lives of individuals, families, and communities.

Tanya - Family & Youth Services

LHS Foundation manages designated endowments established by individuals for Family & Youth program needs. Based upon a percentage of the annual endowment earnings in 2015, $42,779 was made available from five endowments for Family & Youth operational and capital needs. The endowment distributions are available for such services as: education and living expenses for individuals who continue to receive services beyond the state-mandated age limit, tutoring costs for individuals needing extra assistance, and capital expenditure needs for the facilities.

In addition, the Foundation dedicates its annual Christmas Appeal to Family & Youth Services to pay for gifts for youth in our care. Each year, generous donors help ensure that our residents receive gifts to help them enjoy the Christmas season and feel a part of the Family & Youth family. Youth in our care have been preparing the artwork for that appeal. Each year, their talents are displayed on the front of the Foundation’s Christmas appeal and on the Thanksgiving cards sent to LHS donors.

“Tanya” came to LHS Family & Youth Services’ Maumee Youth Center at the age of 16. She was removed from a very abusive home and, prior to coming to LHS, had spent years in several other residential placements. Tanya was impulsive, depressed, and prone to running away. She had also cycled through several children's psychiatric hospitals. Her caseworker wanted a place where Tanya could complete school without having to move again. Although Tanya often appeared happy and outgoing, she was anxious and distrustful of others. She was wary of making friends and developing relationships with staff, since they had always ended abruptly in the past. Tanya was assured by staff members that they were not going

anywhere. Her therapist worked with Tanya and helped her develop insight into her thoughts and behaviors. She began to see things differently and develop new tools and skills. Tanya has an older sister, and a plan was developed to see if Tanya could live with her sister after graduation. The two sisters had not seen each other in years and spoke on the phone only occasionally. Phone conversations were increased, and Tanya was taken by LHS staff to visit with her sister several hours away. The visit went well and soon other visits followed. Tanya graduated from high school in May, a triumphant occasion made possible by the diligent efforts of caregivers, therapists, teachers, and administrators. Her sister and caseworker attended the ceremony and graduation party. After the party and pictures, Tanya’s sister helped her pack up her belongings. Before leaving for the next chapter in her life, though, Tanya expressed gratitude to her therapist and all the staff who helped her onto the path of success.

Marsha - Housing & Community Services“Marsha” has always been very independent and able to provide and care for herself and her family. Recently, she had some medical issues and experienced financial difficulty. Lutheran Homes Society was asked to provide benefits counseling services through the PASSPORT program to help Marsha qualify for programs to help pay her medical and household bills. Marsha and the LHS Service Coordinator set up a monthly budget and were able to pay down her bills. Unfortunately, Marsha had several overdraft fees, and the bank garnished her entire Social Security check for two months to pay them. The Service Coordinator worked with the bank to forgive one of the overdraft fees.

Due to the bank garnishing her check, Marsha was unable to pay her utility bills for the month of January and was in jeopardy of having the utilities to her home shut off. The Service Coordinator and Marsha spoke with several community agencies and requested assistance. The Salvation Army was able to provide partial assistance. The Service Coordinator then arranged to have the remaining portion of the utilities paid by an LHS Foundation fund set up to help community clients in need.

Marsha explained that she had never had this type of financial hardship before, and that her main concern was for her eight-year-old grandchild, who is in her care. She did not want to cause her grandchild any more confusion or stress, which would have been created by packing them both up and finding somewhere else to stay until she could pay her utilities. Marsha was very thankful for help and support provided by LHS.

LHS Foundation manages a designated fund called the Resident Assistance Fund. This fund helps pay for immediate expenses of both LHS housing residents and community service clients receiving in-home assistance. In one community case, a mother, who was being transitioned out of a long-term care facility, wanted to reunite with her two sons. Dollars from the Resident Assistance Fund enabled them to live as a family. To obtain custody of the children, the mother had to have a livable, safe home that addressed the needs of the two boys. The fund helped supply items, such as bunk beds, that kept the family together. In addition, there are times when an LHS Service Coordinator notices there is not enough food in a home, a new housing resident does not have the

LHS Foundation Supportfinancial means to pay for an apartment’s security deposit until a Social Security check arrives, or a housing resident has applied for Medicaid and related benefits but needs prescription medication right away. These immediate needs could all be covered by the Resident Assistance Fund.

David - Lutheran Home at Toledo“David” came to The Labuhn Center on the Lutheran Home at Toledo campus after hip surgery. He was flat on his back, totally dependent, and dealing with Parkinson’s disease. In his younger days, he had been a basketball player, a coach, and an educator. Now, according to one of his therapists, he was one of the most physically impaired patients she had ever worked with. His therapy started slowly, with him lying on a tilt table and working toward weight-bearing status. It took weeks of physical and occupational therapy to get him where he could sit up in a chair. During some of his treatments, David would sing Christian hymns. As the weeks went by, he gradually became stronger. Eventually, he was able to walk 20 feet with a walker and assistance, and was able to feed himself. He and his wife, “Donna,” worked as a team with the therapists, as she learned to assist him with transferring from his bed to his wheelchair. She would push his wheelchair, and they would go for walks around the campus. Many times, David would be sporting his Ohio State shirts and pants.

During David’s stay, he and Donna bonded with the staff, and David found a kindred spirit in the resident across the hall who had also played basketball. They enjoyed many spirited conversations. When the time came to leave, David and Donna went home - ready to meet the challenges of daily life. He had worked very hard to progress as far as he had, and she was at his side every step of the way. David’s accomplishments while at The Labuhn Center were an inspiration to those who watched him battle and overcome so many obstacles.

Following the passing in 2015 of the Rev. Gerald Labuhn, who served many years as Lutheran Homes Society Executive Director and then its Archivist and Museum Curator, the Labuhn family decided to designate memorial gifts to purchase crosses for each patient room at The Labuhn Center. Pastor Labuhn had always wanted to place crosses in each resident’s room, as a visible reminder of the religious-based founding of LHS and its continuing mission to enrich the spiritual as well as physical healing of residents. However, due to declining health, he was unable to fulfill that dream. His wife wanted to honor her husband’s years of LHS service, and his daughters wanted to show their respect and admiration for their father. They decided that the best way to honor him would be to purchase the crosses for each room with the memorial funds from friends and family, designated for this purpose. Now, each room will have a cross honoring both husband and father for his many years of loving, faithful service.

“Kathy” came to Lutheran Home at Napoleon with a fractured hip. She was familiar with the Napoleon campus because she had been in and out visiting friends, neighbors and families over the years. Even though going to a rehabilitation center was a difficult choice for Kathy, she knew the high caliber of care she would receive from the staff. She worked hard in therapy and found

Lutheran Homes Society has generous, faithful donors who realize that sometimes the everyday needs of the residents cannot be met through their financial resources. To assist with providing these resources, designated gifts have been received from donors for the Lutheran Home at Napoleon campus, including resources for a Bible study group, resident activities in assisted living, barber or beauty services at the nursing care center, renovations to the east wing, other capital improvements, and nurses’ chairs. Lutheran Home at Napoleon also received a bequest for renovations to the Chapel.

A Lutheran Home at Napoleon resident and donor explained:

… because of the compassion shown to me by the staff and their good care, I feel I need to thank them by supporting what they need to help others like myself.

herself a bit surprised to discover how weak her upper body had become. This was aggravated by her lower extremity weight-bearing restrictions. Kathy realized how much she needed upper body strength to maneuver her wheelchair and to transfer herself to a bed or another chair. When the time approached for her discharge, a home assessment was completed by therapy staff so that everything would be ready for Kathy to transition home successfully.

After Kathy returned home, members of the therapy team, who worked with her as an inpatient, were able to continue working with her through an arrangement with the LHS home health agency, Northwest Ohio Home Care Partners. Kathy is well on her way to a complete recovery. She wrote thank you notes expressing how much she appreciated the Lutheran Home staff. From arranging for her card clubs to come and play at the facility while she was there to making her feel comfortable and in charge of her own decisions, Kathy was grateful for the wonderful staff.

“James” was admitted to Lutheran Memorial Home after a short hospital stay related to a chronic breathing problem. He hoped to receive therapy and return home. After his rehabilitation was completed, and he was physically and medically stable, James found out that he did not have any place to call home. The Life Enrichment Coordinator assisted him with making sure that insurance would cover his continued stay until safe discharge plans were put in place. She also helped James apply for the HOME Choice Program. This program would help him find an apartment and obtain needed furnishings and personal items.

James’ HOME Choice caseworker turned out to be another Lutheran Home Society employee, who also works as the Service Coordinator at Luther Oaks senior apartment community in Norwalk, Ohio. The LHS team worked together to make James’ transition back into the community a smooth, safe one. He will be going home to a new apartment with the personal items, furnishings, and home health services that he needs to successfully move from a nursing home setting to community living. “My stay at Lutheran Memorial Home was good, including the physical and occupational therapy, the doctors, nurses, and aides,” James explained. He is very grateful for all the support he received from the Lutheran Home Society family of services.

Lutheran Memorial Home has received many donations through which family members express their support of staff. One family sent this statement in with their donation:

This gift is in appreciation for the years of service and care you gave to my mother and father. My brothers and sister found that we could not care for them at home; there were too many health issues that we did not have the resources to address. Please pass on to all the caregivers, nurses and so many other staff members our thank you that our parents’ needs were addressed in such a professional and caring manner. We owe all of you so much. You are our unsung heroes.

As with other LHS care communities, donors often give to funds designated for special purposes. Contributions to Lutheran Memorial Home have been designated for such items as popcorn for residents, plants for the garden, the employee Christmas party, bingo, an event celebrating staff, and new therapy equipment.

After much soul searching, “Sam” and “Betty” decided to sell the house in which they had lived for 50 years and move into Creekside independent living at Lutheran Village at Wolf Creek. While this was not an easy decision, they enjoyed their time together in independent living – having fun without many of the burdens that come with owning a home. After Betty passed away a couple of years ago, the condo became too much for Sam to handle and brought back too many memories of his and Betty’s life together. Sam’s health was also starting to decline. This was when the “beauty” of a continuing care retirement community came in handy. Sam moved into an assisted living apartment on the Wolf Creek campus. He was happy with his move to assisted living, and his family and he were grateful for the daily help the staff provides him.

After a while, Sam experienced health issues that required him to move to another level of care – skilled nursing. While he recovered, Sam could once again remain on the campus with which he was familiar. Because of the therapy he received and his commitment to getting stronger, Sam was eventually able to return to his assisted living apartment. The family is grateful for all the nurses, aides and therapists who made Sam’s recovery possible.

LHS Foundation frequently receives donor gifts from residents or their families, accompanied with letters thanking the ministry staff. One such letter spoke of the excellent care received by his father at Lutheran Village at Wolf Creek:

Ever since my father entered the Luther Village at Wolf Creek nursing facility, he had everything good to say about his experience there, from the nurses and aides who helped him, the activities that entertained him, the structure that housed him, the religious services that nourished his soul, and the cooks who nourished his body. Lutheran Homes Society was richly blessed by the generous volunteers who served our ministries in 2015. Volunteers play a significant role in enhancing the lives of our residents and patients. Behind the scenes, some provide leadership and expertise on the LHS Board of Directors and ministry boards, while others enable LHS to be more cost-effective through their service. Additionally, hundreds of volunteers spend valuable time with residents to provide the extra touches that make living at our ministries more like home.

Volunteers Enhance Our Residents LivesEvery day, our volunteers enrich lives through such activities as planting flowers to beautify our grounds, leading Bible studies to nourish the spirit, playing games to engage residents, hosting birthday parties to celebrate life, or simply having a conversation to show a resident someone cares. They share meaningful activities, and they create hope – for the enjoyment of a precious moment in time, for a better day, or for a more abundant life. However they choose to make an impact, LHS is grateful for our dedicated volunteers.

LHS Foundation has two programs that are accessible to all full-time staff of Lutheran Homes Society. The Foundation manages an endowment that is designated for Staff Benevolent Care, as well as a special fund designated for Staff Scholarships.

Staff Benevolent Care is used to cover emergency situations and catastrophic events that occur in a staff member’s life. It provides assistance for staff members in dealing with losses from sudden, unexpected events including: fires, floods, or other natural disasters; serious medical emergencies; or the accidental or sudden death of an immediate loved one. Since the beginning of the program in 2005, grants totaling $14,000 have been given to thirteen staff members experiencing such emergencies.

The Staff Scholarship fund is available to staff members who are attending school for degrees that are relevant to their positions at LHS. These scholarships are budgeted by each ministry and are awarded for completed work at or above certain grade standards.

Northwest Ohio Home Care Partners

Lutheran Homes Society (LHS), established in 1860, serves youth and elderly in northwest Ohio and southeast Michigan in various settings. Children and youth are served through a campus-based residential treatment center, community-based residential treatment group homes, clinical services including psychotherapy, and supportive services for families. Older adults are served in independent living, assisted living, and long-term care residential settings on four campuses. These campuses also offer both inpatient and outpatient therapy services for residents and community members. Twelve independent living apartment communities for older adults are located throughout our service area. LHS provides its housing residents and community members with service coordination and assistance programs, which help older adults access needed services that enable them to continue living independently. Northwest Ohio Home Care Partners, the LHS home health agency, extends the organization’s continuum of care by providing skilled nursing and therapy services for patients in the comfort of their own homes. The LHS Adult Day Center provides adult day services for seniors in Toledo.
