The following information may be obtained from the above reports: A. Surveillance Reports: These surveillance reports generate the information at various level i.e. National Level/State Level/District Level/Block Level/ Reporting Unit Level

cases of selected disease for the selected year as per selected form (S/P/L) at various levels. Please refer Figure 1 for selecting the parameters and Figure 2 for output.

2. Cases of Selected Diseases for Selected Period (National/State/District): This report will generate total number of cases of particular selected disease for the selected weeks as per selected form (S/P/L) at various levels. Please refer Figure 3 for selecting the parameters and Figure 4 for output.

3. Cases of All Diseases for Selected Period (National/State/District) : This report will generate all disease of selected form (S/P/L) for the selected week. In case more than one week is selected than it will generate cumulative no of cases at various levels. Please refer Figure 5 for selecting the parameters and Figure 6 for output.

4. Proportion of The Samples Found +ve for Selected Diseases for Selected Period(National/State/District): This report will generate the detailed laboratory surveillance data for the selected diseases (multiple) for a selected period with the break up of sample tested and found positive at various levels. Please refer Figure 7 for selecting the parameters and Figure 8 for output.

5. Time Based Submission Status: This report will generate S/P/L form wise submission status for the selected period for district/block/RU level. This report shows submission in time, delay and not submitted Figure 9 for selecting the parameters and Figure 10 for output.

6. RU wise/Form wise Submission Status: This report will generate form

wise (S/P/L) with different categories of RUs submission status for the selected period at various levels with total no of RUs in selected category. (a) RU Type wise submission status: this report will generate RU wise

submission status for S/P/L for a selected period. Please refer Figure 11 for selecting the parameters and Figure 12 for output at RU TYPE WISE

7. State/District/Block Wise Submission Status: This report will

generate form wise submission status for the selected period for reported and not reported facilities (State/Districts/Blocks).

8. Week wise SPL - Forms Submission Status for Selected Period

(National/State/District) : Status of submission for weekly surveillance information will invoke the below reports:

a) Weekly Data Received: For accessing number of districts/blocks/RUs reporting

b) Weekly Reporting percentage: For accessing %age of districts/blocks/RUs reporting

c) Weekly Summary Report: This is very useful for monitoring purpose regarding the total number of RUs in a particular State/District and the RUs reported. It is similar to Covering Sheet of RUs in State/District/Block Select the Reporting Level (State/District/Block) In case of Weekly Summary Report -> select the Form Type for two

reports (a & b) above -> select the State/District, year and weeks. (Please refer to the Figure 9 below)

9. Consistency Report (Please refer to 8.0 section for details)

10. IDSP User's Login Status Report: This is for users monitoring purpose, which users have ever logged in or used previously. From this report one can also view the date and time of login for a particular user.

C. Disease Outbreak Report:

11. Disease Outbreak Report: This report will generate outbreak reported for selected National/State level for the selected period.

12. Disease Outbreak Submission Status: This report will generate state wise no of outbreak reported for the selected period.

13. Search for Disease Outbreak: This report will generate disease syndrome wise outbreak reported for the selected period.

14. Disease Outbreak History: This report will generate disease wise outbreak report for the selected state and disease.

D. Quarterly FMR Report:

15. FMR Report Quarterly: This report will generate Quarterly FMR report for state/district wise for the selected financial year.

16. FMR Submission Status: This report will generate submission status of Quarterly FMR report for State/Districts.

E. Feedback Report:

17. This report will generate the feedback given by the various users for the selected period.

F. Reporting Unit Report:

18. This report is useful to detect the garbage master data; this shows all the garbage/non-visible master data which might have been created by mistake or for demonstration purposes but has not been deleted due to some or other reasons. This will show the exact number of Blocks/RUs in any particular district for further corrections.

8. CONSISTENCY REPORT Timeliness & Consistency: - Timeliness may be defined as reports obtained within one week after the last date of every reporting week. Minimum expectation is that each reporting unit reports for at least 40 weeks (or 80% of week at any given time under consideration) in a year (52 weeks). All identified reporting units must report. However, for programme purpose, the following definition of consistency may be followed:

A reporting unit is consistent, if it has reported for at least 40 weeks (or >= 80% of week at any given time).

A district is consistent for a week if at least 80% of RUs under the district report for a particular week.

A state is consistent for a week if at least 80% of the district (having consistent RUs) report for a particular week.

Grouping for Consistency Report: - Reports should have desegregated collated forms of

P {i. PHCs, ii Other Govt. Hospitals (CHC/ID Hospitals/Medical Colleges/Other Premier Institutes/Public Sector Hospitals) and iii Private hospitals separately}

L (Government and Private Laboratory) and S reporting units ( Subcentres)

How does a reporting unit/district become consistent Suppose we have 5 reporting units i.e. A, B, C, D, E in a district and we want to check the consistency of this district during 10 weeks of the current year. The denominator in this case is 10 and any unit that has reported in time for 8 weeks or more will be considered as consistent. Now, RU A has reported for 9 weeks, B for 8 weeks, C for 7 weeks, D for 8 weeks and E only 5 weeks as table below. (Reporting unit, reported is mentioned as “Y” and not reported mentioned as “N”) S. Reporting units C and E are not consistent because they are reporting for less than 80% of weeks. In the above example the given district is not consistent because only 60% [3 out of 5] reporting units are reporting consistently. Steps for viewing Consistency Report based on P form on Portal:-

Step 1:- For viewing the consistency report click ‘CONSISTENCY REPORT’. Step 2:- After clicking on consistency report a window will open as under and fill up the desired information as required.

A. Select Reporting format (State/District/Block Level); Say for a State.

B. Select States (Priority/Non Priority/Entire State) C. Select Form Type (S/P/L form) D. Select year (2008/2009/2010). E. Enter week no. in from and to check box as required.

After completing steps A to E the following window will appear.

If we select in Step 4-A:-A reporting format on District level and complete steps B to E the following window will open.
