About ten distinct IFNs have been identified in mammals; seven of these have been described for humans. Based on the type of receptor through which they signal, human interferons have been classified into three major types: Type I IFN, Type II IFN, and Type III IFN. Type I or nonimmune interferons consist chiefly of interferons alpha produced by leukocytes and of interferon beta produced by fibroblasts, although there are several other less important variants. Type II or immune interferon is interferon gamma, which is mainly produced by NK cells and T cells. Type III consists of the lambda interferons. Each type is characterized by a specific receptor and signal transduction pathway [Pestka 2004, Meyer 2009, Borden 2007],[19].

Interferons can stimulate or inhibit up to 300 different genes encoding proteins involved in antiviral defense mechanisms, inflammation, adaptive immunity, angiogenesis, and other processes. The properties of interferons are used to treat a number of viral infections (e.g., hepatitis B and hepatitis C), inflammatory diseases (interferon beta for multiple sclerosis and interferon gamma for systemic sclerosis), and malignancies [19]. Overactivation of the interferon pathways has been demonstrated in patients with systemic lupus erythematosus [Meyer 2009]. The result is a characteristic pattern of mRNA expression known as the interferon signature.

Although interferons are very similar they affect the body differently. Therefore, different interferons are used for different conditions. Interferon alphas are used for treating cancers and viral infections; interferon betas are used for treating multiple sclerosis; and interferon gamma is used for treating chronic granulomatous disease [19].

Interferon-alpha group

The main type I interferons present in humans are IFN-α and IFN-β (others are IFN-ε, IFN-κ, IFN-ω) [Pestka, 2004]. In humans there are multiple IFN-α genes and a single gene of each of

IFN-β, IFN-ε, IFN-κ, IFN-ω. A total of 14 human non-allelic IFN-α genes are located on the short arm of chromosome 9, given rise to 12 distinct proteins (IFN-α1, IFN-α2a, IFN-α2b,IFN-α4a, IFN-α4b, IFN-α5, IFN-α6, IFN-α7, IFN-α8, IFN-α10, IFN-α14, IFN-α16, IFN-α21) [REF]. At least 13 IFN-α genes are transcribed. The coding sequences of those genes diverge up to 8% giving rise to 12 different functional subtype proteins (proteins IFN-α1 and IFN-α13 are identical) [Pestka 2004, Hilkens 2003].

All type I IFNs are structurally similar and share a common receptor IFNAR, consisting of two known subunits, IFNAR1 and IFNAR2. Binding and activation of the receptor stimulates Janus protein kinases, which in turn phosphorylate several proteins, including STAT1 and STAT2 and leads to downstream signaling events. Activated ISGF3 (IFN-stimulated gene factor 3) consisting of STAT1, STAT2 and the IRF-9 (IFN regulatory factor 9) binds to IFN-stimulated response elements (ISREs). The result is transcriptional activation of numerous IFN-stimulated genes (ISGs). The resulting proteins mediate the biological responses generally attributed to type I IFN, including antiviral, antiproliferative and immuneregulatory activities [REF] [Pestka 2004, Hilkens 2003].

Despite the fact, that IFN-α proteins share the same receptor, the specific activity of individual IFN-alpha proteins differs. It might be due to distinct receptor affinity and\or different interaction sites and it can lead to the variability of signals transduced. Regarding the signaling events triggered by IFN-α subtypes there are indication for differential receptor interaction and distinct activation of STAT molecules, which may result in divergent ISG regulation and functional impact on the IFN-α family members. With respect to cell responses triggered by IFN-β versus IFN-α, a distinct aminoacid composition conserved among IFN-α subtypes seems to result in a weaker affinity for IFNAR1 and a generally lower

biological activity of the IFN-α subtypes. However, differences in gene regulation by IFN-β versus IFN-α are only detectable at subsaturating concentrations, indicating that responses comparable to IFN-β may be elicited by high levels of IFN-α [Hilkens 2003, Molla 20111].

Producers of interferon-alphaAn important consequence of having several type I IFN might lie in the different temporal or spatial regulation of their expression. It seems probable that all cells produce IFNs I after viral infections. Similarly, cells of epithelial, fibroblast or hematopoietic origin produce IFNs I in response to bacterial infection. In general interferons alpha are produced by leukocytes and interferon beta is produced by fibroblasts [Borden 2007],[19]. However, an important distinction must be made between those cells and produce just enough type I IFNs to affect local environment and the production of IFNs I by the IFN-producing cells, which secrete vast amount of type I INFs and therefore generate a systemic response [Liu 2005]. IPCs are thought to be immature plasmacytoid dendritic cell population. The plasmacytoid dendritic cells accumulate in peripheral lymphoid tissues during an infection and can secrete up to 1000 times more interferon than other cell types. They were studied in [Liu 2005]

Type 1 interferon-(alpha, beta, omega)-producing cells (IPCs), also known as plasmacytoid dendritic cell precursors (pDCs), represent 0.2%-0.8% of peripheral blood mononuclear cells in both humans and mice and are specialized in rapidly secreting massive amounts of type 1 interferon following viral stimulation. pDC/IPCs is a separate hematopoietic lineage, which appears to be closer to B lineage than to myeloid lineage. These cells are continuously produced from HSC within the bone marrow and then released into the peripheral blood stream. Unlike myeloid cells that enter the secondary lymphoid nodes from afferent lymphatics, pDC/IPCs enter the lymph nodes through HEV (like the T and B lymphocytes) and then colonize the T cell–rich areas. Under steady state, pDC/IPCs appear to play a critical role in maintaining peripheral immune tolerance. This may be due to the ability of resting pDC/IPCs to prime naive T cells to produce IL-10. Unlike monocytes and mDCs that preferentially express TLR2, -4, -5, and -6, pDC/IPCs selectively express TLR7 and -9 within the endosomal compartment and thus display plasma cell morphology,

Upon viral infection, pDC/IPCs rapidly produce large amounts of type 1 IFNs, which not only have direct inhibitory effects on viral replication but also contribute to the activation of NK cells, B cells, T cells, and mDCs, leading to the induction and expansion of an antiviral immune response. The ability of activated pDC/IPCs to activate myeloid immature DC through type 1 IFN appears to be critical for the induction of T cell–mediated antiviral immunity.

After producing large amounts of type 1 IFN at a later stage of viral infection pDC/IPCs rapidly differentiate into unique type of mature DCs through an autocrine mechanism mediated by type 1 IFNs and TNF-α. At this stage, pDC/IPC-derived DCs may contribute to the contraction of the effector phase of T cell responses and at the same time to the establishment of T cell memory. These mature dendritic cells regulate the function of T cells and thus links innate and adaptive immune responses. IPCs are considered as the most important cell type in antiviral innate immunity.

Differences in IFN-alpha subtypesA mentioned before, the specific activity of individual IFN-alpha proteins differs. But a good correlation was found between their antiviral and antiproliferative activities. Also there are large differences in the ability of individual IFN-alpha proteins to activate NK cells [Hilkens 2003, Molla 2011].

The existence of so many different IFN-α subtypes raises the question of their biological relevance. It is known that different viruses induce different patterns of IFN-α subtype expression and that the production of the individual subtypes is regulated by different IRFs (interferon-regulating genes). Distinct relative potencies have been shown, however, all IFN-

α subtypes likely have the abitility to exert the classical IFN-αβ activities (e.g. antiviral effects) as they act through the common receptor. However, there is substantial biochemical evidence that individual IFN-α subtypes bind to different sites of IFN-αβR (IFN-αβ receptor) and have different binding affinities. This leads to the formation of distinct signaling complexes, which might be expected to differentially recruit downstream signaling pathways [Hilkens 2003, Molla 2011].

In the study [Hilkens, 2003] for the first time was investigated the specificity of IFN-alpha responses in immune cells (T cells and Dendritic cells in particular. Signaling through the Jak/STAT pathways and gene expression profiles were examined for human T cells and DC treated with different IFN-α subtypes. The authors supposed that super-imposed upon widely overlapping classical functions such subtype specific signaling complexes may be capable of differentially activating ancillary responses peculiar to that subtype. It was assumed that the differences between IFN-α subtype signaling vary with cellular background and will be manifested by differentials in gene expression profiles. These cells were chosen because it seems most obvious to analyze IFN-α subtype properties in primary cells of the immune system. The following subtypes of IFN-α were chosen for the analysis: IFN-α1, IFN-α2 and IFN-α2 because IFN-α1 and IFN-α2 are generally the major IFN-species produced in response to many viruses and are widely used therapeutically. Also IFN-α21 exhibits enhanced growth inhibitory activity compare with IFN-α2. In addition both recombinant and natural IFN-α1 have a ~10 fold lower specific antiviral activity that the majority of subtypes, including IFN-α21 and IFN-α2.

Differences between kinetics of expression of IFN-stimulated genes and in the requirements of individual IFN-stimulated genes for additional signaling pathways were observed. In particular, striking differences between subtypes were found in the induction of IFN-γ inducible protein-10 (IP10, a chemokine which plays an essential role in the recruitment of Th1 cells and NK cells to inflammatory sites. IP-10 was highly induced by IFN-α2 and IFN-α21 but to a much lower extent by IFN-1 in DC. In contrast it was poorly induced by all of these IFN-α in T cells. In addition to a protective role for IP-10 in infections that require strong cell-mediated immune responses, it may also contribute to progression of some diseases. The expression of IP-10 has been observed in many Th1-type inflammatory /autoimmune diseases including psoriases, multiple sclerosis, rheumatoid arthritis and type1 diabetes [Meyer 2009], where the subsequent recruitment of Th1 cells to inflammatory sites are not desirable. It is of interest to note that the therapeutical application of IFN-α is contraidicated in patients with a history of Th1-related disorders. There are clinical examples where administration of IFN-αβ has caused the induction of auto-immune diseases and has led to exacerbation of psoriatic lesions [Meyer 2009]. Accepting the essential role of IP-10 in Th1-type inflammatory diseases the observation of its differential regulation by IFN-α subtypes may be clinically relevant. The IFN-α1 subtype may be more suitable candidate for therapeutic application in cases where Th1 inflammatory responses are not desirable as induction of IP-10 is much less in response to IFN-α1. Also it is tantalizing that IFN-α1 caused less side effects in patients with malignant diseases.

Using global gene expression profiling it was shown previously that at a given concentration more genes are induced by IFN-β that IFN-α2, but the patterns of expression of IFN-stimulated genes becomes identical upon increasing the IFN-α2 concentration. In this study a similar effect was found for the majority of IFN-stimulated genes. It is not surprising, accepting that IFN-αβ act through a common receptor. Although induction of IFN-αβ may be high in response to at least some virus infections, it can be relatively low in response to other pathogens. Thus, differences in IFN-αβ actions at lower concentrations may also be biologically relevant [Hilkens 2003].

Substantial quantitative differences between IFN-α1, IFN-α2 and IFN-α21 in the ability to activate STAT1-5 and a spectrum of IFN-stimulated genes were found. No significant differences in the kinetics of STAT activation by the different subtypes over a range of concentrations were observed. The less efficient phosphorylation/activation of STAT1,-3 by IFN-α1 and IFN-α2 correlates with the known lower binding affinity for the IFN-αβ receptor and lower specific antiviral activity, depending on cell type. Quantitative differential between IFN-α1 and IFN-α2 and IFN-α21 appears smaller for the induction of IFN-stimulated genes than for the activation of STAT.

kinetics of accumulation of mRNAs for different subsets of IFN-stimulated genes add to the complexity of the IFN-α response.

The study [Moll, 2011] addressed to the diversity of IFN-α subtypes in the context of cell-type specific responses in a comprehensive manner. It was shown that differential activity of interferon-alpha subtypes is consistent among distinct target genes and cell types. ISG regulation at the mRNA and protein level as well as anti-viral activity were investigated in lymphatic endothelial cells (LECs), in blood vessel endothelial cells (BECs) and in primary fibroblasts isolated from the same donor to minimize the role of biological variation.

Since ISG regulation by type I IFN occurs primary at the transcript level, first ISG mRNA expression levels in response to 13 different IFN-α were investigated. Five target genes were selected, involved in antiviral and immunoregulatory IFG functions (IFIT1, ISG15, CSCL10, CSCL11, CCL8). The selection of ISGs was based on a previous analysis of genes differentially regulated in endothelial cells and fibroblasts by IFN I. ISG expression values were normalized against those in untreated conditions. With respect to the induction profile of these five target genes in response to various IFN-α subtypes a reoccurring pattern of stimulation was observed. Individual IFN-αproteins showed consistently high, intermediate or low activity irrespective of target gene or cellular background.

different cells types yielded an induction profile for the investigated IFN-α proteins. It was shown, that according to their potency in gene induction, IFN-α subtypes can be classified in three clusters with low (IFN-α1), intermediate (IFN-α2a, IFN-α4a, IFN-α4b, IFN-α5, IFN-α16, IFN-α21) and high (IFN-α2b, IFN-α6, IFN-α7, IFN-α8, IFN-α10, IFN-α14) activity.

To investigate whether the distinct potency of IFN-α subtypes to induce ISG transcripts was also reflected in the amount of generated protein a member of each cluster was chosen for subsequent experiments: IFN-α1 with low activity, IFN-α4b with intermediate and IFN-α2b with high activity. Since IFN-α subtypes displayed a consistent induction pattern in all cell types, the investigations were limited to LECs. After stimulation with IFNs, cells were analyzed for mRNA and protein expression. It was shown, that protein levels were closely correlated with transcript levels (for example, IFIT1 protein and corresponding transcript).

The distinct potency of IFN-α subtypes was further evaluated with respect to biological functions, in this case to prevent viral replication. The antiviral activity of the investigated subtypes correlated with their capacity to regulate ISG mRNA and protein. IFN-α1 showed the weakest potency to protect LECs against influenza infection and propagation, IFN-α4b and IFN-α2b showed intermediate and strong anti-viral activity respectively.

Finally, comparison of time course and dose response to IFN-α subtypes was performed. The maximum level of transcript expression was comparable between subtypes but was achieved at distinct ligand concentrations. The kinetics of ISG15 mRNA induction was comparable between IFN-α subtypes. Differences in IFN activity were only observed at subsaturating levels of IFN-α proteins and didn’t affect the time course of ISG regulation.

Cell-type specific responses were apparent for distinct target genes independent of IFN-α subtype and were based on different levels of basal versus inducible gene expression. For

example, CXCl10 and CXCL11 transcript levels were generally higher in endothelial cells despite a pronounced induction by IFN-α in fibroblasts.

To sum up, with respect to ISG regulation IFN-α proteins can be split into three categories of high, intermediate and low. Differences in ISG expression are further determined by cell-type specific factors directing basal and inducible gene regulation.

Double-stranded RNA forms the genome of some viruses and can be made as part of the infectious cycle of all viruses. These long double-stranded RNA are potential inducers of IFN alpha and beta production.

Long double-stranded RNA are recognized by Toll-like receptor TLR-3 in the endosomes. TLR-3 signals through and adaptor molecule TRIF to activate the interferon-regulatory factors IRF3 and IRF7, transcription factors that induce the production of IFN-alpha and IFN-beta.Long double-stranded viral RNA can also induce the expression of interferons by activating the cytoplasmic proteins RIG-I and MDA-5, which interact with adaptor protein CARDIF and that leads to the activation of the transcription factors IRF3 and IRF7. Activated

IRF3 and IRF7 can form both homodimers and IRF3-IRF7 heterodimers that enter the nucleus and activate the transcription of a number of genes, including IFN-alpha and IFN-beta [Platanias 2009].

IFN-alpha and IFN-beta secreted by the infected cell bind to a common cell-surface receptor (interferon receptor), on both the infected cell and nearby uninfected cells. The interferon receptor, like many other cytokine receptors, is coupled to a Janus-family tyrosine kinase, through which it signals. Janus-family kinases directly phosphorylate STAT proteins (signal-transducting activators of transcription). The phosphorylated STAT proteins enter the nucleus, where they activate the transcription of several different genes, including those encoding proteins that help to inhibit viral replication [Platanias 2005, Meyer 2009]. Examples of those proteins:

TLR-4 can also induce IFN-alpha and IFN-beta in response to the recognition of bacterial cell-wall components. Interferons in turn induce the expression of costimulatory molecules on macrophages and dendritic cells, which enables them to act as antigen-presenting cells that can fully activate T cells. Thus, a macrophage or a dendritic cell that becomes activated when its TLRs bind pathogens is in turn able to signal to other macrophages and dendritic cells and recruit them to initiate an adaptive immune response [Platanias 2008, Meyer 2009, Decker 2005].

IFN alpha and beta also stimulate the increased expression of MHC class I molecules on all types of cells and to lesser extent MHC II molecules. The cytotoxic T lymphocytes recognize virus-infected cells by the complexes of viral antigens, presented on MHC class I on cell membrane. So, indirectly interferons promote the killing of virus-infected cells by cytotoxic T cells (CD8+). Also they may up or downregulate delayed-type hypersensitivity and stimulate macrophage activity [Decker 2008, Meyer 2009].

In addition to antiviral activity, they activate NK cells (to kill virus-infected cells) and B cells, and down-regulates vascular endothelial growth factor expression through PI-3 kinase and MAPK kinase signaling pathways [Meyer 2009].

IFN-alpha medical applicationsInterferon-alpha has antiviral, immunomodulatory and antiproliferative properties. Interferon-alpha and beta are widely used for treatment of a wide variety of diseases, including multiple sclerosis, viral hepatitis (B and C) and melanoma [Antonelli 2008, Kirkwood 2002, Wayne 1999]. It is used in the treatment of hematologic malignancies and solid tumors. It was shown, that antiproliferative and antiviral efficacy are not correlated. Approval by FDA was granted early for treatment of patients with hairy cell leukemia, acquired immune deficiency syndrome-related Kaposi-sarcoma, condylomata acuminata. Subsequent studies have documented its activity against chronic muelogenous leukemia, B and T lymphomas, melanoma, multiple myeloma and renal cell carcinoma [Kirkwood 2002]. Although IFNs are effective as single agents in certain clinical pathologic entities, it is suggested that their greatest therapeutical potential may be realized in combination with other biological response modifiers, cytotoxics or antiviral agents. While IFN-alpha appears to be moderately effective in certain diseases, the flu-like syndrome associated with its use is a major limiting factor for its clinical application [Kirkwood 2002],[20].

Interferon-alpha drugs include many natural and recombinant interferons alpha and their mixtures. For example [20]:

Interferon alfa-2a (Roferon-A) is FDA-approved to treat hairy cell leukemia, AIDS-related Kaposi's sarcoma, and chronic myelogenous leukemia.

Interferon alfa-2b is approved for the treatment of hairy cell leukemia, malignant melanoma, condylomata acuminata, AIDS-related Kaposi's sarcoma, chronic hepatitis C, and chronic hepatitis B.

Ribavirin combined with interferon alfa-2b, interferon alfacon-1 (Infergen), pegylated interferon alfa-2b, or pegylated interferon alpha-2a, all are approved for the treatment of chronic hepatitis C.

Interferon beta-1b (Betaseron) and interferon beta-1a (Avonex) are approved for the treatment of multiple sclerosis.

Interferon alfa-n3 (Alferon-N) is approved for the treatment of genital and perianal warts caused by human papillomavirus (HPV).

The following treatment paradigms were introduced for three human malignancies that showed sensitivity to IFN-alpha treatment: melanoma (single-agent activity for durable survival benefit on an immunologic bases at high doses), lymphomas (augmented cytotoxic effects in combination with chemotherapies) and chronic myelogenous leukemia (modulation of oncogene expression at lower-moderate doses).

During the therapy, IFNα may cause severe neuropsychiatric syndromes including depression with suicidal ideation, paranoid psychoses, or confusional states [Schaefera 2002]. Antigenecity and side effects are the main drawbacks to the clinical use of IFN-alpha. Flu-like symptoms following each injection (fever, chills, headache, muscle aches and pains, malaise) occur with all of the interferons. These symptoms vary from mild to severe and occur in up to half of all patients. The symptoms tend to diminish with repeated injections and may be managed with analgesics such as acetaminophen (Tylenol) and antihistamines such as diphenhydramine (Benadryl). Dose-limiting effects usually appear a few weeks into treatment and include fatigue, anorexia, weight loss, depression, impaired cognitive function and diminished potency. These effects vary between patients. Also IFN-alpha can cause autoimmune disorders as mentioned above [Meyer 2009].

FutureIn recent decades the structure and the function of interferons (IFNs) have been elucidated and it is now clear that IFN is not only a protein but a group of proteins with parallel, but not analogous or identical, biochemical and biological properties. Among them there are at least 12 subtypes of IFN-alpha which are all involved in the control of several cellular functions and are all actively engaged in host defense mechanisms against infections. These acquisitions led to the clinical use of different types of IFN-alpha with appreciable success in several diseases. There are however several possibilities to optimize IFN-alpha treatment which can and must be addressed. For instance, as regards IFN-alpha biology and therapy issues, we need to understand more how the different subtypes can generate similar signaling outputs but also govern specific cellular responses and more in general why the body produces so many of these IFN-alphas.

Dalam beberapa dekade terakhir struktur dan fungsi interferon (IFN) telah dijelaskan dan sekarang jelas bahwa IFN tidak hanya protein tetapi sekelompok protein dengan paralel, tapi tidak analog atau identik, biokimia dan biologi sifat.
