Introduction

TheIsleofWightRegistrationServicehasproducedthisbereavementguidetohelpyouthroughthisdifficulttime.

It provides guidance, support and reassurance and will help you to access all the information you will need to helpyou through your bereavement.

Losing a loved one presents many challenges and our bereavement guide gives some suggestions of organisations you might like to contact for support after registration.

We provide a sympathetic, helpful and considerate service to help you make the necessary decisions and arrangements when someone dies. It is important to us that you are provided with clear and concise information and guidance throughout the registration process.

In case of emergency, please contact Wightcare on (01983) 821105 who will have up-to-date out of hours contact details for the Registrars.

The First Steps

What must you do when someone dies?

When someone dies, you will need to inform a number of people and organisations and complete certain documents needed by law. If you are a relative or friend you can do some of these things yourself. Others will need to be done by the executor or administrator of the estate. There is plenty of support availabletohelpyouthroughthisdifficulttime.

Some of the first steps include:lObtainamedicalcertificateofcauseofdeath signed by a doctor or, if the Coroner is involved, takeinstructionsfromCoroner’sofficersregarding registration of the deathlRegisterthedeathattheRegisterOffice. To make an appointment telephone 01983 823233lContacttheBenefitsAgencyusingformBD8(given by the Registrar when you register the death.) l Contact a funeral director, if you intend to use onel Begin arrangements for the funeral – if the deceased made a will you should check the will for any special requests.

Registration

How and where can you register a death, and what information will you need to provide?

Adeathmustberegisteredwithinfivedaysfromwhenit occurred. This period can be extended in exceptional circumstances and if the Coroner is involved. The registration must take place in the district where the death occurred, although the information can be given atanyRegisterOfficewithinEnglandandWales.You can only register a death once you have theMedicalCertificateofCauseofDeathfromthe doctor, or in the case of a death reported to the Coroner,confirmationfromtheCoroner’sofficethattherelevant paperwork has been issued. Contact us once youhavethecertificatetomakeanappointment.Thecertificatewillusuallybeinawhiteenvelopeprintedwith our contact details including the appointment line phone number.

Who can register a death?

The death should be registered by a close relative.If there is no close relative please contact the Register Officeforadvice.

What information will you need to give theRegistrar about the deceased?l The date and place of deathl Their full name and any other names they are known by, or have been known by, including a married woman’s maiden surnamel Their date and place of birthlTheirlastoccupation(ifthedeceasedismarried, widowed or has a formal civil partnership, the full name and occupation of their spouse or civil partner)l Their usual addressl The date of birth of a surviving spouse or civil partnerlDetailsofanypublicsectorpension,e.g.civil service, teacher or armed forces

What information will you need to give theRegistrar about yourself as the person registering?l Your relationship to the deceased for example, son, daughter, widow, widower, niece, nephew, surviving civil partnerl Your full namel Your usual address

What documents will you need when registering a death?

When you go to register the death you must take with you:Medicalcertificateofthecauseofdeathsignedbyadoctor, unless the Coroner is issuing all the paperwork.Birthand/ormarriagecertificateinrespectofthe deceasedtorefertoasrequired.PleasealsobringIDfor yourself.

What documents will you receive from theRegistrar?

Aftertheinformationhasbeenrecordedintothereg-ister, the Registrar will issue the necessary forms and certificates.IfaCoroner’spost-mortemhasnotbeenheld, the Registrar will give you:lACertificateforBurialorCremation(calledthe ‘green form’), giving permission for the body to be buried or for an application for cremation to be madelACertificateofRegistrationofDeath(formBD8)to notifytheBenefitsAgencylCertifiedcopiesoftheregisterentrymaybeobtained from the Registrar at the time of registration at the currentstatutoryfee.Thesecertificatesarewhat many organisations and agencies refer to as ‘OriginalDeathCertificates’andwillservealllegal purposes. AllcertificatespurchasedareCrownCopyright protected and cannot be photocopied. Certifiedcopieswillcontinuetobeavailablefromthe RegisterOffice.

What happens if the death is referred to a Coroner?

If a death is reported to the Coroner by either the doctor, the Registrar or the family, he/she is required to enquire into the circumstances and the cause of the death, and he/she will consider whether it is necessary to hold a post mortem. In due course he/she will issue his/hercertificatedirectlytotheRegistrarwhichwillpermit registration to take place. Appointmenttodothisoncetheregistrationhastakenplace. In a very small number of cases the Coroner will be required to hold an Inquest. In this case registration of the death will be delayed until the inquest is closed, althoughinterimdeathcertificateswillbeavailablefromtheCoroner’sOffice.Aftertheinquestthedeathwillberegistered upon information provided by the Coroner. TheRegisterOfficewillcontactyoubypostsothatyoumayapplyforanycertificatesyourequire.

We can help you tell the people who to know

When someone has died, there are lots of things that need to be done, at a time when you probably least feel like doing them. One of these is contacting Government departments and local council services that need to be told. The Isle of Wight Council is providing a service which we hope will make things easier. This service means that you can just tell us and these organisations will be contacted.

How the service can help you?

When someone has died, their death needs to be registered with the Registrar.Once that’s done, several other organisations may have to be contacted and given the same information.We can help you to give the information to the DepartmentforWorkandPensionsandtheycanpasson this information to a number of other government departments and local council services for you.

What happens to the information?

Next of Kin

Ifyouarethenextofkin(closestrelativebybloodormarriagetothedeceased),yourentitlementtobenefitsmay change, so please have your own National Insurance number and / or date of birth with you when you contact us.If you are not the next of kin, or the person who is dealingwiththedeceased’sestate(thepersonsortingout their property, belongings and money), you can still use the service if you have their permission to provide their details and act on their behalf.

How we will treat the information you give us to use any of these services.

The information you give will be treated securely and confidentially.Theorganisationswhoarecontactedwillusetheinformationtoupdatebenefits,creditsorhelpto start up services. They may use this information in other ways, but only as the law allows.

How can you contact us to use the service?

If you decide not to use the Tell Us Once service, the following information may be useful.

Pensions/Social Security Benefits/New Tax Creditsl InformtheDepartmentofWorkandPensionto ensure any state pension payments are stopped, telephone 0345 6060 265l Inform the deceased’s personal pension provider, if a policy was in placel InformtheJobcentrePlusorSocialSecurityOffice ifbenefitwasinpayment,forexample,Income Support,JobseekersAllowance,ChildBenefit, AttendanceAllowancetelephone0345 604 3719l Inform the HM Revenue and Customs, if tax credits wereinpayment(WorkingTaxorChildTaxCredits) telephone 0345 300 3900l Inform the deceased’s employer or current occupational pension provider who will advise if the deceased’s occupational pension can be passed to another family member or surviving civil partner.

Health Servicesl Tell the deceased’s doctor and any other health service provider they may have used, such as local hospital, dentist and chiropodist.l Return any equipment the deceased may have borrowed from a hospital, clinic or Social Services.l Cancel any services received from Social Services, such as meals-on-wheels, transport or home help.

Money

Thereareanumberoffinancialorganisationswho may need to be informed. These organisations will probablyrequesttoseeacopyofthedeathcertificateand include:-l The Inland Revenue as there may be an Income Tax refund duel Banks, building societies, credit card companies, insurers,PostOfficesavings,premiumbonds,stocks and shares.

Houses and Household BillslAskthePostOfficetoredirectthedeceased’smail, orspeaktotheRegistraraboutusingtheDeceased Preference Service or the Bereavement Register to stop any unwanted junk mail or direct mailings and the distress they bring with them, and reduce the risk of identity fraudl The law states that the Registrar who registers the deathmusttelltheCouncilTaxDepartmentofa person’s death. However, you may wish to contact them directly to discuss any outstanding monies owed and any refunds or reductions in Council Tax chargeslDependingonthedeceased’scircumstances,you may need to speak to a council housing department, housing association, mortgage company or private landlordl Tell the deceased’s gas, electricity, water and telephone line providerl Cancel any deliveries of heating oill Inform the insurance company that provides the household insurance.

Transport and Travell Tell the car insurance company. Please note that anyone who is a named driver on the deceased’s car insurance will no longer be insured to drive the vehicle. This is because the person who took out the insurance is not alive to give permission for someone else to drivelReturnthedeceased’sdrivinglicencetotheDriver& VehicleLicensingAgency(DVLA)l Return the deceased’s car registration documents totheDVLAtoallowthechangeofownershiptobe recordedl Cancel or return any travel passes or season tickets. Claim any refunds duel Return the deceased’s passport to the UK Passport Agency.

Personal Itemsl Return any library books and cancel membershipl Send back any membership cards to clubs or associations and claim any refunds that may be due.

As early as possible:

Contact the executor as soon as you can to enable him/her to start the process of obtaining probate if necessary. The executor is usually nominated in the will. If there is no will, decide who will apply to sort out the deceased’s affairs and contact the Probate Registry to apply for ‘letters of administration’ if necessary. What do you need to do to register a stillbirth?

When a child is stillborn, a doctor or midwife will is-sueamedicalcertificateofstillbirth.ThepersonwhoregistersthestillbirthmustpresentthiscertificatetotheRegistrar.EverystillbirthinEnglandorWalesmustberegistered in the district in which it takes place.Atpresentastillbirthcanberegisteredupto12monthsafter it occurred.Where a stillbirth occurs in hospital on the Isle of Wight, the Registrar is willing to attend at the hospital and carry out the registration there subject to the availability of the mother or the mother and father, and the consent of the hospital or alternatively at a home address.

Who can register a stillbirth?

Parents married to each otherl If the child’s parents were married to each other atthetimeofthestillbirth(orconception),eitherthe mother or the father may register the stillbirth.

Parents not married to each other

If the parents were not married to each other at the time ofthestillbirth(orconception),informationaboutthefather may be entered in the register only in the following circumstances:l If the mother and father attend together to give information for the registration and sign the stillbirth register together, orl Where the father is unable to attend with the mother - the father makes a statutory declaration acknowledging his paternity which the mother must producetotheRegistrar(thisformmaybeobtained fromanyRegistrarinEnglandorWales)l Where the mother is unable to attend with the father - the mother makes a statutory declaration acknowledging the father’s paternity which the father mustproducetotheRegistrar(thisformmaybe obtainedfromanyRegistrarinEnglandorWales).If the parents of the child cannot register the stillbirth, please contact the Register Office for advice. Information to be supplied for the registration of a stillbirth

For the child:lDateandplaceofstillbirthlTheforename(s)andsurname,iftheparentswishto name the stillborn childl Sex of the child.

For the father(wherethisinformationistobeenteredin the register):lForename(s)andsurnamelDateandplaceofbirthl Occupation at the time of the stillbirth or, if not employed at that time, the last occupation.

For the mother:lForename(s)andsurnamel Maiden surname if the mother is, or has been, marriedlDateandplaceofbirthl Occupation at the time of the stillbirth or, if not employed at that time, the last occupationl Usual address at the date of the stillbirthlDateofmarriage,ifmarriedtothestillbornchild’s father at the time of the stillbirth.

What certificates will be issued?

Certificate of registrationlAcertificateofregistrationwillbeissued,freeof charge, to the person who registers the stillbirth.

Certificate for the burial or cremation lThecertificatefortheburialorcremationwillbe given to the parents following the registration. Thecertificatewillneedtobegiventothechosen funeral director.

Funeral ServicesIsle of Wight Council Bereavement Services, who work closelywiththeRegistrarsandCoronersDepartments,are based at the Crematorium and can be contacted as above. We can facilitate all Cremation Services, in addition to arranging burials in our twelve Council Cemeteries. Therefore, we would like to just outline the role we play when someone passes away. If you would like more details, please contact us and we will assist you through this very sad and distressing time.

Cremation ServicesNormally, once you have contacted your chosen FuneralDirector,theywillliaisewithustobookadateandtimeforyourpreferredservice.Dependingontheindividual circumstances, different combinations of documents(asdescribedelsewhereinthisbooklet)will need to be completed and sent to us, therefore enabling the statutory legislation to be followed. Your FuneralDirectorwillguideyouthroughthis.Cremation services usually take about 25 minutes, but if you feel that a longer service time is required or there will be a large volume of people attending the service, pleasediscussthiswithyourFuneralDirectorwhenbooking the service. We also encourage families to discuss any individual elements of the service with the FuneralDirectororwithusaheadoftime,sothatthesecan be facilitated appropriately.Our chapel is non-denominational and many varied services take place each day. There is the provision for organ music to be played and the music system can accommodateCD’s,andUSBsetc.Cremated Remains are available from 10.30am the following working day after the service. If they are to be collected, please note that we only keep them in our careforonemonthaftertheservice.Anappointmentwill need to be made with us if you wish to scatter the cremated remains in the Gardens of Remembrance. We endeavour to keep the grounds as natural as possibleandwewelcomefreshflowers,butreserve the right to remove other memorials. In respect of memorials left at the crematorium, please see the notices in the grounds. Floral Tributes from each service are placed on the paved Tribute Plot and usually remain there for a week. BurialsBurials can take place in local churchyards, if there is still space or in any of the private woodland burial areas on the Island. For these you would need to contact the church or minister concerned or via your chosen FuneralDirector.Isle of Wight Council Cemeteries are located at Ashey,Binstead,Carisbrooke-Mountjoy,EastCowes,Newport - Fairlee, Newport - St Pauls, Northwood, Ryde, Sandown, Shanklin, Ventnor, Wroxall.Burials in these cemeteries need to be booked with BereavementServices.TheExclusiveRightofBurial(EROB)needstobepurchasedbeforeanyfullburialorthat of cremated remains takes place. If it is the owner ofthisEROBthathasdied,theycanbeburied,buttheEROBwillneedtobetransferredtothenextrightfulperson or relative, before any further burials in that grave can take place or a memorial placed. Ownership oftheEROBisalwaysconfirmedwithourrecordswhena burial is booked or a memorial requested. In many cases you’ll need to close down accounts or cancel or change insurance details, subscriptions, agreements, payments or direct debits. You should remember that if the deceased owned a vehicle then it is possible that there is no longer insurance cover for it to be driven. Many policies state that a vehicle may be driven by

someone else with the owner’s permission but as soon as the owner dies any such permission may cease. It is best to contact the car insurance company before anyone drives the vehicle to

make sure they are insured.
