Field Trip Bag Lunch Order Form  

Lunch must be available to all students on all school days, even when they are on a field trip. All students may                                           

participate. Free students will not be charged, reduced price students will be charged $.40 and paid students will be                                     

charged the price of a regular paid meal. Bag lunch includes an uncrustable, string cheese, fresh fruit, juice, baby                                     

carrots, Elf grahams and water. The cafeteria staff will mark the students’ names on the bag lunches who have                                     

allergens and then input the meals into the POS system after a copy of this order form has been returned by the                                           

teacher acknowledging receipt of meals.  

Please return this form to the cafeteria ONE WEEK PRIOR TO THE FIELD TRIP.
