Cerebrale malaria
- 2,7 % van de p. falciparum malaria
- insulten, psychose, agitatie, bewustzijnsdaling en coma
- moeilijk onderscheid symptomen / bijwerkingen

Voorschrift o.a. afhankelijk van - reisgebied (= soort malariaparasiet en resistentie) - reisduur - individuele risicofactoren / contraïndicaties - autoriteit die adviseert

Bijwerkingen Lariam
Veel: cardiaal, gastrointestinaal, neurologisch, psychiatrisch, dermatologisch
(Neuro)psychiatrisch:Frequent: duizeligheid, hoofdpijn, slaperigheid, slaapstoornissenVerder: visusstoornissen, oorsuizen, evenwichtsstoornissen (evt.
gepaard gaand met gehoorstoornissen) Zelden: encefalopathie, sensorische en motorische neuropathieën
(incl. paresthesie, tremor, ataxie, coördinatiestoornissen), voorbijgaande emotionele stoornissen, neuropsychiatrische en neurovegetatieve stoornissen zoals angst, verwardheid, rusteloosheid, concentratiestoornissen, depressie, stemmings-veranderingen, hallucinaties, psychotische of paranoïde reacties, convulsies

Conclusies
Cochrane review, 2009:
- Malarone lijkt beter te worden verdragen en geeft een lager risico op
neuropsychiatric adverse effects
- Kwaliteit van beschikbare evidence is echter beperkt
- Andere factoren, zoals kostprijs, gebruiksgemak, type reiziger en
type reis moeten worden meegenomen in de keuze
Advies
Advies CDC:
- Mogelijke nadelen van profylaxe wegen niet op tegen de voordelen - Patiënten met depressie, angst of psychose in de voorgeschiedenis
moet een alternatief middel worden voorgeschreven
Farmacotherapeutisch kompas:- Profylactisch gebruik is gecontraïndiceerd bij psychiatrische
afwijkingen (incl. endogene depressie) of convulsies in de
anamnese

Mechanisme?
Exacte effect op gedrag in mensen grotendeels onbekend
Motorisch leergedrag blijkt gestoord tijdens Lariam-gebruik, waarschijnlijk o.b.v. blokkade van connexins, wat leidt tot problemen met synchronisatie in de oliva inferior.
