I just got married! How do I add my spouse to my health plan? You need to submit the Family Status Change form to the Bureau of Human Resources (BHR) within 30 days after your wedding. If you miss the deadline, you will not be able to add your spouse until FY19 Annual Enrollment.
Why is an on-site health screening reservation required two weeks in advance? The two week reservation period will save you time at the on-site health screening. Your paperwork will be ready and waiting at the event. It also allows us to priovide health screenings in a more cost effective manner.
The Family Status Change form is due within 30 days of the qualifying event for birth, adop-tion, marriage, divorce, or job loss. Please con-tact BHR for more information at 605.773.3148, or [email protected].
