  • CAUSES AND SOURCES OF ACCIDENT 1. Unsafe ActAn unsafe act is a violation of an accepted work practice or method. Examples: Using equipment without having received proper training in its use. Using tools or equipment the wrong way. Failing to use personal protective equipment Fooling around and playing about. Hurrying and taking dangerous shortcuts Distracting other from their work or allowing yourself to be distracted.

  • 2. Unsafe ConditionsAn unsafe condition is created by improper control of the physical, mechanical, and environmental exposures of the workplace. Examples:.5.6.Lack of Instruction in safe methods. Lack of training.Unsuitable clothing for the task to be done.Physical defects, like poor eyesight or defective hearing. Long hair in the proximity of rotating machines.Unguarded machinerys.
