What is Transforming Clinical Practice Initiative (TCPI)?The Transforming Clinical Practice Initiative (TCPI) is one of the newest programs initiated by the Centers for Medicare & Medicaid Services (CMS) uniquely designed to support clinician practices through nationwide, collaborative, and peer-based learning networks that facilitate practice transformation.

Who is the HCDI TCPI Support and Alignment Network (SAN)?HCD International, National Council of Asian Pacific Islander Physicians (NCAPIP), National Hispanic Medical Association (NHMA), National Medical Association (NMA), and National Minority Quality Forum (NMQF) are the HCDI Support Alignment Network (SAN) committed to ensuring that clinicians—especially clinicians serving vulnerable populations, receive free Transforming Clinical Practice Initiative (TCPI) training.

What are Support and Alignment Networks (SANs)?The Support and Alignment Networks provide a system for workforce development utilizing national and regional professional associations and public-private partnerships that are currently working in practice transformation efforts. Utilizing existing and emerging tools (e.g., continuing medical education, maintenance of certification, core competency development) these networks help ensure sustainability of these efforts.

What are Practice Transformation Networks (PTNs)?The Practice Transformation Networks will provide “boots on the ground” support to coach, mentor and assist clinicians in developing core competencies to assist them in moving through various phases of health care transformation.

Clinicians supported by TCPI include primary and specialty physicians, nurse practitioners, physician assistants, clinical pharmacists, and their practices. They need to be on the Phy-sician Quality Reporting System (PQRS) list of eligible partici-pants. (If not currently enrolled in PQRS, the HCDI TCPI SAN can assist in facilitating enrollment).

Physicians: Doctors of Medicine, Doctors of Osteopathy, Doctors of Podiatric Medicine, Doctors of Optometry, Doctors of Oral Surgery, Doctors of Dental Medicine, and Doctors of Chiropractic.

Practitioners: Physician Assistants, Nurse Practitioners, Clinical Nurse Specialists, Certified Registered Nurse Anesthetists, and Anesthesiologist Assistants, Certified Nurse Midwives, Clinical Social Workers, Clinical Psychologists, Registered Dieticians, Nutrition Professionals, Audiologists, Advanced Practice Regis-tered Nurses or APRNs.

Therapists: Physical Therapists, Occupational Therapists, and Qualified Speech-Language Therapists.

You cannot participate if you are enrolled in Medicare Shared Savings Programs, Pioneer ACOs, Comprehensive Primary Care Initiative or Multi-Payer Advanced Primary Care Practice programs.

» Receive FREE sensitivity training on cultural and linguistic delivery of care, when requested

As clinicians progress through the TCPI five Phases of Transformation, they will be better positioned to meet and succeed under delivery system reform efforts such as those that are part of the Medicare Access and CHIP Reauthorization Act (MACRA), such as the Merit-Based Incentive Payment System and qualified Alternative Payment Model provisions. WHO IS ELIGIBLE TO JOIN TCPI?
