These trials culminated at the end of the 1990s in ap- proval of several pump designs for circulatory support pending recovery of the patient's own heart or pending cardiac transplantation. The evolution from temporary to permanent pump implantation destination therapy) is even more exciting [2, 3]. Permanent implants have been reported in Europe and the United States, and multicenter trials of destination VAD therapy are now underway [4, 5, 6, 7]. The data generated by multicenter trials of VADs describe the performance of individual brands of assist devices for relatively common indications i.e., bridge to transplantation) but do not address uncommon and difficult situations such as mechanical circulatory sup- port for patients who have already had cardiac trans- plantation. Furthermore, they do not readily allow users to compare the indications, strengths, and weakness of one device to another. In their paper, Minev and associates [8] describe 15 patients from a population of 848 transplant recipi- ents who required mechanical circulatory assistance with a variety of assist devices following a cardiac trans- plant procedure. Although this is not a large group, it is the largest reported experience to date and is an impor- tant step in defining how to optimally care for these challenging patients. Previous case reports show that success is possible in individual instances. The collected cases in the Minev et al. contribution indicate that this is true, but with the proviso that many patients will not survive. The paper also tells us that there are several reasons for profound circulatory failure in cardiac trans- plant recipients, and that each situation poses its own difficulties. Moreover, it is likely that each situation is best treated with a certain type of support device. Right heart failure and primary graft failure are usu- ally apparent in the operating room or soon thereafter in the intensive care unit. Right heart failure is frequent- ly related to high pulmonary vascular resistance faced by the donor heart in the recipient, and right heart fail- ure is often transitory. Prescribing a less expensive, short-term support device such as a centrifugal pump is reasonable. Primary graft failure may or may not recover [9], and therefore a device capable of long-term support is indi- cated. Rejection that presents as rapidly progressive heart failure, sudden death syndrome, or medically re- fractory rejection with shock is a particularly difficult problem. The heart may be surrounded by dense adhe- sions, the patient may present in cardiac arrest, and sev- eral days or weeks of immunosuppressive drugs may have seriously compromised the patient's ability to combat infection. The survival to hospital discharge of one in five patients reported by Minev et al. [8] is there- fore noteworthy, as is their innovative use of the Thora- tec VAD as a TAH following removal of the transplant- ed heart. Their one success serves as an impetus to con- tinue treating these patients, with an eye towards im- proving their chances of survival. Effectively treating transplant patients with pro- found cardiac failure requires that the team managing the patient meet several challenges. One such challenge W.
