Medieval Times, located at the Arundel Mills Mall in Hanover, MD on Thursday, March 27, 2014. I understand the following …

The field trip includes a live horse/knight show in an enclosed area and a meal. Persons with respiratory conditions, allergies, or asthma should attend at own risk. Medieval Times uses stroboscopic and pyrotechnic effects. Frederick County Public Schools / Board of Education and its employees, agents and

volunteers, cannot be held responsible for events or conditions beyond their control. All Board of Education Student Conduct Policies are in effect for this activity. (Students will

be held to the same expectations and rules as for any other normal school day.) Students may not use devices to take photographs of other students. (County Policy) Due to the arrangements of tickets and the finances associated with the trip, money is not

refundable if my child is absent on the day of the trip. Students should not bring extra money on the trip as there will not be time to visit the Gift

Shop. (Additionally, they will not have to worry about losing it or other related items.)

Chaperones (Parents or Guardians, not older siblings) will be needed for the trip. (Emergency chaperone replacements must be on the school’s emergency contact list.) Unfortunately, due to the cost, CMS cannot provide tickets for chaperones. Chaperone tickets are $40. If you would like to chaperone a group of approximately eight students, check the box below.

I would like to chaperone.

If a vegetarian meal is preferred, check the box to the right. Student (Chaperone)
