When nursing care comes close to you, you should first understand long-term care insurance.What is long-term care insurance?

What is long-term

care insurance?

The long-term care insurance program was implemented in Apri l 2000. This program is operated by the municipal ity whe re t he pe r son conce rned l i v e s i n . When a pe r son becomes 40 years of age, he/she wi l l be enro l led in the long-term care insurance program as an insured person, and a person certified by his/her municipality as a person that is under the condition of need for long-term care and is the age of 65 or older may receive services at any t ime. A person aged 40 to 64 i s a l so e l i g i b l e t o r ece i ve nu rs i ng ca re services, if the person is certified as a person that is under the condition of need for long-term care due to the specified diseases covered by long-term care insurance.

1. Persons aged 65 or over (Category1 insured persons)

2. Persons aged 40 to 64 that are insured by medical insurance (Category2 insured persons)

●Cancer at terminal stage

●Rheumatoid arthritis

●Amyotrophic lateral sclerosis

●Ossification of posterior longitudinal ligament

●Osteoporosis following a bone fracture

●Dementia in middle age

●Spinocerebellar degeneration

●Spinal carnal stenosis

●Progeria

●Multiple system atrophy

●Diabetic neuropathy, diabetic nephropathy and diabetic retinopathy

●Cerebral vascular disease

●Parkinson's disease, Progressive supranuclear palsy and corticobasal degeneration

●Arteriosclerosis obliterans

●Chronic obstructive pulmonary disease

●Osteoarthritis following significant deformity of both knee joints, or the hip joint

Insurance benefits under long-term care insurance are paid directly to the facilities and service providers used by elderly persons, and users pay only a portion of the cost payable by users.

To use facilities and services under the long-term insurance program, you must undergo a review to obtain a certification of needed long-term care.

Community General Support Center is a support center established by each municipality to provide community support for elderly persons for their daily living. Each Center has public health nurses, certified social workers, senior care managers and other staff. The Center serves as a contact desk for elderly persons and their family members in the community, and works with other organizations to help resolve issues and provide consultation.

The applicant’s attending doctor is requested by the municipality to submit a statement of opinion on the conditions of disease of the applicant, special medical treatment needed, the level of dementia or

disability and other relevant matters.

Partial nursing care is needed due to unstable mental and/or physical condition, or dementia or other similar condition.

●To make inquiries about long-term care insurance or to use services under long-term care insurance, you can consult with long-term care insurance desk in municipalities and 23 special wards in Tokyo, community general support centers, in-home care support providers and other support organizations.●If you wish to use services, you must submit a “certification of needed long-term care (needed support)” together with a long-term care insurance certification to the long-term care insurance desk of your municipality.●You may request a community general support center, an in-home care support provider or a long-term care insurance facility to prepare and submit an application for you.

Certification Committee of Needed Long-Terms Care reviews the case to determine what level of care is needed based on the result of initial assessment, investigation of situation, statement of opinion of attending

doctor and other relevant facts. (The decision is NOT based on the level of severity of illness, etc.)

A certification investigator from your municipality will visit the applicant’s home for interview.The investigator will conduct an interview to assess mental and physical conditions of the applicant using a certification

investigation sheet that is commonly used throughout Japan.

Should you have questions concerning certification of needed long-term care or wish to obtain an application form, please refer to each municipality’s website.

● There are 8 types of results: Care levels 1 to 5; Support levels 1 and 2; and Not applicable (Self-support).● A person who is found to be “not applicable” may not use long-term care services.● The effective period of certification generally expires at the lapse of 6 months in the case of new application and amended application, and at the lapse of 12 months in the case of renewal. However, if the mental or physical condition of the person concerned is expected to drastically deteriorate, the effective period may be shortened up to 3 months, and if the mental and physical condition of the person concerned is deemed to be stable for a long period of time, the effective period may be extended up to 24 months.

Partial social support is needed. The person to whom none of care levels 1-5 and support levels 1 to 2 is applicable.

Lectures on promotion of sound health for the elderly, nursing care prevention courses, such as a course for the improvement of motor functions, for those whose vital functions are deteriorating and nursing care will be needed in the near future are available.

The person is not demented or does not have any similar symptoms and mental and physical condition is stable, but social support is needed.

The highest level of nursing care is needed.

Procedures for long-term care services

Care manager is formally called “long-term care support specialist” and prepares a care plan, communicates with relevant organizations and makes arrangements upon consultation or request of a person who received a certification of needed long-term care or his/her family members for the person requiring care to be able to receive appropriate care services under the long-term care insurance.

Use services under the long-term care insurance wisely to achieve a better work-life balance

Kyushu University has in place a system of leave of absence for nursing care. Family care leave (unpaid)Care levels 1~5 Support levels 1 and 2You may take family care leave when you need to provide nursing care for a period of 2 weeks or longer to a person (eligible family members) who requires constant care due to injury, illness or physical or mental disability.・Up to 6 months in total (or up to 93 days for fixed-term faculty members, etc.*1 and fixed-term faculty members (indefinite change )*2), and up to three times for each eligible family member.

Support services for daily life, such as meals and bathing, or for the maintenance or improvement of vital functions are provided through a day visit to a day service center.

Outpatient day long-term care (day service) Outpatient preventive long-term careCommon services, such as support for daily life, and se l ec t i ve se rv i ces based on t he pe rson’s goa l (improvement of motor functions, nutrition and oral functions) are provided.

Support services for daily life, such as meals and bathing, and rehabilitation to maintain or improve vital functions are provided through a day visit to a long-term care health facility for the elderly or medical institution.

Outpatient rehabilitation (day care) Outpatient rehabilitation for preventive long-term careIn addi t ion to support serv ices for da i ly l i fe and rehabilitation as common services, selective services to meet the person’s goal (improvement of motor functions, nutrition and oral functions) are provided at a long-term care health facility for the elderly or medical institution.

A home helper visits the person’s residence and provides support for physical activities, such as bathing, using a restroom and having meals, and other daily life, such as cooking and laundry. Transportation services (care taxi), which offers support for getting on and off, to visit hospital are also available.

Home-visit long-term care (home help) Home-visit service for preventive long-term careWhen no family support or community support is available for activities which the user is unable to do alone, home visit services are provided by a home helper to support him/her to become independent.

A person who requires constant nursing care that is difficult for him/her to receive at home is admitted and receives support and care for daily life.(Only for those at care level 3 or higher. However, those at care level 1 or 2 may be eligible if he/she is recognized as living at home is difficult for unavoidable reasons (exceptional admission).)

A person with stable condition receives care, primarily rehabilitation, to be able to go back home and live at home. (Care level 1~5)

Nursing care, long-term care and rehabilitation, etc. are provided based on the medical management to a person who finished receiving medical treatment for an acute phase and requires long-term treatment. (Care levels 1~5)

A certified care worker and nurse visit the person’s residence and provide bathing service using a mobile bath.

Home-visit bathing service Home-visit bathing service for preventive long-term careAs far as the person does not have a bathing facility or using a bathing room of other facilities is difficult due to infectious diseases, etc., home-visit bathing services are provided.

Under the instruction of a doctor, a physiotherapist, occupational therapist or speech therapist visits a home of a user who has difficulty visiting hospital, and assists rehabilitation.

Home-visit rehabilitation Home-visit rehabilitation for preventive long-term care

Under the instruction of a doctor, a nurse or any other care provider visits a home of a user who has difficulty visiting hospital and provides nursing care or assistance for medical treatment.

Restrictions on overtime work or work late at night for family careYou may request that you do not work overtime beyond 24 hours per month and 150 hours per year to provide care to an eligible family member.You may request that you do not work between 10:00 pm to 5:00 am to provide care to an eligible family member.

Early start or late start for family careYou may change the starting time of work and ending time of work up to one hour to provide care to an eligible family member, to the extent that your daily working hours remain the same.

Day off for family care (paid)You may take a day off to provide necessary care to an eligible family member, including nursing care, accompanying a family member to visit hospital and making an arrangement for an eligible family member to receive long-term care services.・Up to 5 days per calendar year if you have only one eligible family member, and up to 10 days per calendar year if you have 2 or more eligible family members.

The objective of Support for the Employment of Research Assistants (short-term) is to enhance the overall research capability of Kyushu University (“the University”) by taking full advantage of the diversity of human resources. This is achieved by developing an environment where researchers at the University can devote themselves to their research without facing delays or interruptions in their research when they attend meetings to make contributions to society or go through life events such as giving birth, raising children, or caring for aging parents.

Objective

EligibilityThe University’s female faculty members or research fellows. (those who engage in research for at least 20 hours a week)◆For further details, contact the Scientific Research Promotion Division.

This program helps researchers cover the expenses of hiring research assistants who work under their supervision to directly assist with their research work. The program is mainly designed for female researchers who are recognized as having difficulty securing research time.

●Cases where benefits are not payable・If an insured person starts family care leave on or after the day his/her family member reaches 65 years of age, the allowance is not payable, as a person becomes a “continuously insured elderly person” after he/she reaches 65 years of age.・If, at the time of start of family care leave, the person is expected to leave the University after the expiration of family care leave period, the allowance is not payable. In addition, if a person is employed for a fixed term, it is required that, at the time of start of family care leave, the person has been continuously employed by the University (and insured by the employment insurance) at least one year, and is expected to continue to be employed by the University more than 93 days from the starting day of family care leave.

①Leave must be taken to provide care for 2 weeks or more to a family member who requires constant care (walking, using a restroom, meals and other activities that are necessary accommodations for daily life) due to injury, illness or physical or mental disability.②Leave must be actually taken by an insured person of the employment insurance after he/she applies to his/her employer for the leave clearly indicating the first day and the last day of the period of leave.

The insured of employment insurance who is on a family care leave to provide nursing care to a family member under the condition of need for long-term care and whose basic wage payment days in a month were 11 days or more for at least 12 months in 2 years immediately prior to the day the family care leave started

◆Family care leave may not start during maternity leave. If family care leave for a different family member, maternity leave or child care leave starts during family care leave period, the original family care leave ends on the day prior to the starting day of such new leave, and family care leave benefits are not payable for the days after such day.

The allowance will be paid for family care leave which satisfies both 1 and 2 below, during the period of family care leave (up to 3 months from the starting day of family care leave), provided that the allowance is paid only once for family care leave to provide care to the same eligible family member for the same condition of need for long-term care.

The family care leave period is divided into one month periods staring from the starting day of family care leave (if the ending day of family care leave occurs during such one month, the period up to the ending day of family care leave), and the amount of benefits is calculated for each period, and the total amount for the entire periods is paid by lump sum.

The amount paid for each payment period is generally the monthly wage of the month during which the leave started x 67%.◆There is a limit to the amount of the allowance.
