Memorial Staff, Medical Staff, Board of Trustees and Community: Introductory Remarks We are celebrating our company-wide achievements in improving quality and safety in patient care. In 2011, LifePoint was selected as one of 26 organizations to participate in a Centers for Medicare and Medicaid Services (CMS) 2-year program called Partners for Patients as a Hospital Engagement Network (HEN). The goals of the program were to reduce healthcare-acquired conditions (“harms”), reduce readmissions, and create an enduring culture of safety. At the end of September this year, that contract concluded.

LifePoint achieved outstanding results. Some highlights of our success include at 49.6% reduction in aggregate harms, compared to the baseline from 2010, and a 14% reduction in readmissions. 97% of LifePoint hospitals now utilize a bedside shift report and include the patient and family as a part of the team discussion during shift changes. Additionally, 50 LifePoint leaders earned Certified Professional in Patient Safety (CPPS) designation through the National Patient Safety Foundation. Julie Howard, Director of Quality and Safety from Memorial, was one of these.

We are incredibly proud of our results as a company and as an individual hospital. We especially thank and recognize Memorial’s hospital leadership team, clinicians and staff for working tirelessly on behalf of our patients to make quality and safe care a top priority. Gov. Honors Dr. Andazola with Distinguished Public Service Award The 2016 Governor’s New Mexico Distinguished Public Service Awards has selected 11 individuals from throughout the state plus a lifetime achievement family to celebrate at a gala on Nov. 16 in Albuquerque. This year’s honorees include our own Dr. John Andazola, Director of the Southern New Mexico Family Medicine Residency Program and physician at Memorial Family Medicine Center. Dr. Andazola has been a champion of public health causes in our region and our state. One of these initiatives is the nationally acclaimed Nuestra Vida program which has taught better eating and healthier lifestyles to underserved areas of Dona Ana County where diabetes is exceptionally high. Memorial is very fortunate to have a physician of his caliber on our team. Congratulations on this well-deserved honor Dr. Andazola!

Scott Schlaud to Lead Cardiovascular and Cath Lab Services We couldn’t be more excited to introduce and welcome our new Director of Cath Lab and Cardiovascular Services, Scott Schlaud. Scott has an extensive clinical and management background in cardiovascular services in hospitals across the country where he has helped orchestrate significant improvements in patient quality and safety. We are so proud that he chose to work at Memorial and look forward to enhancing and improving the cardiovascular services we bring to this region under his leadership. MMC Cancer Center Team Recognized for Outstanding Performance in Research ACoS Accredited Cancer Center, MMC Cancer Center is able to offer clinical trials to our patients. Patients can benefit from using the latest treatment techniques while our Center contributes to nationwide cancer research. Recently, the Memorial Cancer Team was recognized for outstanding patient participation in a National Cancer Institute (NCI) research program. We are proud to offer patients in our region the highest standard of cancer care from a team of highly trained oncologists right here at home. Memorial Leads in Support of Cancer Charities As the only Cancer Center in our region with ACoS and ACR Accreditation, MMC Cancer Center and Memorial Medical Center are proud to step up as key sponsors of Cowboys For Cancer Research and Tough Enough To Wear Pink (TETWP). For years, Memorial has been a premier sponsor of TETWP, offering not only a generous cash donation, but also 100 Free Mammograms to underserved, uninsured women in our region. If you know of a woman who would qualify for a free mammogram, please ask her to call 575-521-5270 to set her appointment with Memorial Imaging Services. For the past two years, Memorial has been the title sponsor for the Cowboys for Cancer Research Dinner and Silent Auction. Together, these events raise close to a million dollars last year to fund cancer research in New Mexico, both at NMSU and the University of New Mexico. Although we strive constantly to offer the highest standard of care and the latest techniques to help patients beat cancer, our real hope is that one day we will be instrumental in helping find a cure for this disease. Our very own Lynn Fletcher, Director of MMC Cancer Center, has been the President of Cowboys for Cancer Research for three years. We respect and admire her hard work, both here at our MMC Cancer Center and as a leader of this wonderful organization. Fight Breast Cancer – Schedule Your Mammogram Today Through the month of October - Breast Cancer Awareness Month - Memorial Imaging Center at 160 Roadrunner is open every Saturday from 8:00AM until 2:00PM. Come get your (alcohol-free) Mammosas and a wonderful pink tote bag during Saturday hours. Call our Imaging Scheduling Line at 575-521-5270 to make your Mammogram appointment. BOO TO THE FLU! – Memorial Free Flu Shot is National Best Practice Clinic Under the leadership of our Infection Control Manager, Twyla Anderson, and in conjunction with the local New Mexico Department of Health, we will once again be holding a free Flu Shot Clinic for adults and children. The “Boo to The Flu” clinic will run from 8am until all shots are given out, or until 2pm, on Saturday, October 29th. This is the 20th year that Memorial has hosted this free clinic as a benefit to the public. In 20 years of running this clinic, we have administered more than 60,000 doses of flu vaccine to adults and children in Southwest New Mexico. On Saturday, October 29th, adults can go through a drive-thru area in Memorial’s south parking lot, or they may come into the Memorial West Annex Conference Room area. Children must come inside for their vaccine. This year the Department of Health is offering only flu shots for children as the Center for Disease Control (CDC) has determined the mist is

not as effective. This clinic has been so successful under Twyla’s supervision that it has been used by the CDC as a Best Practice model for mass immunizations in other parts of the country. Congratulations, and a Big Thanks, to Twyla for organizing this important event! Our mission is to keep people healthy. Julie Howard One of 50 LifePoint Leaders to Earn Quality Certification

Julie Howard, RN, BSN, CPPS, our Director of Patient Quality and Safety, just became the first member of the Memorial team to become a Certified Professional in Patient Safety. Julie moved into her current position at the beginning of 2016, with a focus on improving patient care throughout the organization. She has made great strides toward that goal, contributing to our 99% Bedside Shift Report, significant improvements in Patient Satisfaction scores and overall reduction in harms across every department in the hospital. Congratulations on earning this significant certification, Julie. Everyone benefits from your accomplishments – especially our patients!

Joseph Armijo Earns HBO Certification Join me in congratulating Joseph Armijo for earning his Hyperbaric Technologist Certification! Joseph is with the Memorial Wound Care Center serving as our Hyperbaric Oxygen (HBO) Safety Director. Hyperbaric Technologist Certification is administered through the National Board of Diving and Hyperbaric Medical Technology (NBDHMT). It certifies the participants’ knowledge of hyperbaric oxygen therapy, physiology, and safety. This national certification increases our staff’s wound care competencies and creates an environment where patients and referring physicians can be assured that Memorial Wound Care Center personnel have the most up to date education and training. In Closing… The subject of quality and safety in patient care is of utmost importance to LifePoint Health and to Memorial Medical Center. That’s why I felt it was important enough to take three paragraphs in the introductory comments this month to share the exciting news about how we are improving quality and care. In everything we do at Memorial, we focus on improving safety and the overall patient experience. We support education and certification for our nurses and providers that translate into improved service to our patients. It is evident that our efforts are paying off. In addition to the significant HEN achievements and many certifications and awards reported in the newsletter, we have received more than a dozen kudos from patients this month. These compliments have come in the form of social media posts as well as many handwritten notes to our CCU and other nurses, thanking them for exceptional and compassionate care in the most difficult times. I can’t tell you how proud and happy it makes me to read these notes and realize what an outstanding and caring team of nurses and doctors we have. Just as our patients thank you, I thank these very special caregivers for your warm heart and hard work each and every day. You make us great!
