SPIROMETRIAlat untuk mengukur ventilasi yaitu mengukur volume statik dan volume dinamik paru
VOLUME STATIK Volume tidal (VT) Volume Cadangan Inspirasi (VCI) Volume Cadangan Ekspirasi (VCE) Volume Residu (VR) Kapasiti Vital (KV) Kapasiti Vital Paksa (KVP) Kapasiti Residu Fungsional (KRF) Kapasiti Paru Total (KPT)
• Pada orang dewasa normal jumlah maksimum udara di kedua paru sekitar 5,7 liter(pria), wanita sekitar 4,2 liter
• Alat untuk mengukur volume paru adalah spirometer• Beberapa volume paru dan kapasitas paru :
1. Tidal volume (TV) : volume udara yang keluar masuk paru dalam pernapasan biasa. Nilainya sekitar 500 ml
2. Volume cadangan inspirasi ( Inspiratory reserve volume ) : volume udara yang masih dapat masuk keparu setelah inspirasi biasa
3. Volume cadangan ekspirasi(expiratory reserve volume,ERV) : jumlah udara yang dapat dikeluarkan secara aktif dari dalam paru , setelah ekspirasi biasa
4. Residual volume : volume paru yang masih tersisa didalam paru setelah ekspirasi maksimumResidual volume ada 2 : kolaps volume dan minimal volume
5. Volume ekspirasi paksa dalam 1 detik ( Forced expiratory volume, FEV1.0): volume udara yang dapat dikeluarkan secara paksa selama detik pertama. Normalnya > 70%-80%
6. Kapasitas vital (Vital capacity ,VC) : volume udara maksimal yang dapat atau keluar paru selama satu siklus pernapasan , yaitu setelah inspirasi maksimal sampai ekspirasi maksimal, KV= TV+VCI+VCE. Kapasitas vital menggambarkan kemampuan paru mengembang
7. Kapasitas inspirasi : KI =TV+VCI8. Kapasitas residual fungsional : KRF = VCE+RV, jumlah udara dalam paru pada akhir
ekspirasi biasa. 9. Kapasitas paru total ( Total lung capacity ): KPT = KV+RV
• Volume paru seperti : Residual volume, FRV, TLC tidak dapat diukur dengan spirometer, tapi menggunakan dilusi helium atau pletismografi tubuh
A. FVC (pr) (Forced Vital Capacity Predicted ) : the maximum volume of air forcedly exhaled from the point of maximum inspiration, by an average healthy person. The FVC is dependent on age, sex and height.
B. FVC ( Forced Vital Capacity ) : Vital capacity performed with a maximally forced expiratory effort. Forced vital capacity is often reduced in patients with chronic lung disorders.
C. % FVC( Percent Forced Vital Capacity ) : forced vital capacity expressed as percentage of the predicted FVC X 100%

D. FEV1 % ( Percent Forced Expiratory Volume In One Second ) : Forced Expiratory Volume in the first second of FVC expressed as a percentage of the total FVC.

INDIKASI PEMERIKSAAN Setiap keluhan sesak Penderita asma stabil Penderita PPOK stabil Evaluasi penderita asma tiap tahun dan penderita PPOK tiap 6 bulan Penderita yang akan dianestesi umum Pemeriksaan berkala pekerja yang terpajan zat Pemeriksaan berkala pada perokok
CARA PEMERIKSAAN Subjek berdiri / duduk Melakukan manuver setelah keadaan steady state Pemeriksaan dilakukan sampai didapat minimal 3 hasil yang dapat diterima dan dua
diantaranya reproduksibel
HASIL YANG DAPAT DITERIMA Permulaan uji harus baik Pemeriksaan selesai Waktu ekspirasi minimal 3 detik Grafik flow – volume mempunyai puncak
PEMERIKSAAN YANG TIDAK BAIK Permulaan ekspirasi ragu-ragu/ lambat Batuk selama ekspirasi Manuver valsava Ekspirasi tidak selesai Terdapat kebocoran Mouth piece tersumbat Meniup lebih dari 1 kali
REPRODUKSIBILITIDitentukan setelah didapat 3 manuver yang dapat diterima reproduksibiliti bila nilai terbesar perbedaannya kurang dari 5% atau kurang dari 100 ml untuk nilai KVP dan VEP1

• Penyakit paru obstruksi :– Peningkatan tahanan jalan napas– Asma , COPD
• Penyakit paru restriktif :– Ketidakmampuan / berkurangnya paru untuk mengembang – disebabkan : penyakit parenkim paru ( fibrosis,edema paru), penyakit dinding dada
( kifoskoliosis , dll), otot-otot respirasi lemah, tumor paru, pleura efusi,

Measuring preparation for subject1. Insert the filter holder firmly into the exhaling side of transducer (to the side where the diameter is
large), and then insert the mouse filter into the head firmly.– The attachment of the mouth filter to transducer is recommended for the prevention of
infection– When the disposable paper mouthpiece (option) is used, insert the mouthpiece into the filter
holder instead of the mouth filter.2. Explain the measurement that will be performed from now to the subject.3. Close the nose's holes with nose clips
VC Measurement1. Tap VC tab2. Tap START button to start measurement3. Breathe normally holding mouth filter in subject's mouth. When breathing is stabilized, the
message "Exhale to the maximum" is displayed on LCD4. When breathing is stabilized, instruct the subject to exhale to the maximum. After confirming that
volume curve never changed, instruct the subject to inhale to the maximum5. When the subject inhales to the maximum, after confirming that volume curve never changed,
instruct the subject to exhale to the maximum. After confirming that volume curve never changed, instruct the subject to return to normal breathing
FVC measurement :1. Tap VC tab2. Tap START button to start measurement3. Breathe normally several time 4. Inhale to the maximum5. Exhale to the maximum as quickly as possible and more to exhale maximum.
MVV (maximal voluntary ventilation), MV Measurement1. Display MVV measurement data screen by tapping MVV tab (or MV tab). Confirm that MVV
tab is shown as "MVVPs" (Reversing display) when the post-test is performed2. Let the subject be ready for the measurement3. Tap START button. MVV measurement screen is displayed.4. The message of "Ventilate fully for 12 seconds" is displayed. Let the subject breathe as deeply and
quickly as possible.5. The measurement will end automatically in 12 seconds after the measurement starts. After
calculating the data, it returns to the measurement data screen, and measured result is displayed. When it is required to stop the measurement for less than 12 sec, tap STOP button. The calculation processing is carried out based on the ventilatory volume till the ending point, it returns to the measurement data screen, and the measured result is displayed.
