Is Knee Replacement Surgery the Only Option? DEAR DR. ROACH: I usually walk 45 minutes daily for a cardio workout. However, the Florida summer heat can be brutal, so instead of walking, I swim and walk one hour in the pool. Is that as good a cardio workout as walking? -- R.G.

ANSWER: I think that having several different ways to exercise is a good idea, since the muscles used will always be at least somewhat different and it can keep you from being bored when doing the same thing day after day. In your situation, I think that it makes a great deal of sense to avoid the heat by getting in the pool.

The intensity of a cardiovascular workout can be measured simply by your pulse rate. A rough but commonly used guide is that moderate cardiovascular exercise is 50 percent to 70 percent of your age-predicted maximum of 220 minus your age. So for age 50, your maximum is 170, and moderate exercise is a pulse of 85 to 119. You can check your pulse at the wrist with a clock, or wear a heart monitor.

Swimming almost always is a more-intense workout than walking (race walkers certainly can get a very fast pulse rate), and walking in the pool is a good exercise for your heart and many muscles.

If the pool is outdoors, as is usual in warm, sunny places, you need to wear and reapply plenty of sunscreen to protect your skin.

DEAR DR. ROACH: In 1978, I was diagnosed with a benign brain tumor. It was on the pituitary gland and pushing on the optic nerve. I had surgery, but only half was removed because of the optic nerve. The other half was shrunk with chemo and radiation. I am concerned about the long-term effects of radiation, because I had a stroke in 2012 and the doctors said this stroke might have been a link to long-term effects of radiation. But 36 years later? Can you confirm or deny that the long-term effects of radiation cause stroke? -- B.R.

ANSWER: I can’t tell you whether your stroke was caused by the radiation you received; however, I can confirm that high- dose radiation (from radiation therapy for your tumor, not from a diagnostic CT scan) increases the overall risk of transient ischemic attack and stroke. A well-done study from Australia in 2011 showed that radiation treatment approximately doubles the risk of TIA and stroke.

This does not mean that the treatment you had in 1978 was inappropriate. Treatment of a brain tumor, even a noncancerous one, is critical, because there is very little room in your head for a tumor in that position. The medical treatment we have now for these kinds of tumors was not available in 1978.

What this means for people with a history of radiation treatment to the head is that they should do everything to reduce their risk of stroke, which means no smoking, careful control of blood pressure and cholesterol, careful diet including low sodium and low sugar, and discussion with a doctor about aspirin therapy.

Caring for grandchildren In the not-so-distant past, it was quite common for

various generations of a single family to live under one roof and for many different members of the family to play a role in raising the children.

But that dynamic slowly changed as families spread out geographically. However, when the economy faltered and parents of young children realized they needed help, many returned to the old way of doing things. Grandparents stepped up to look after their grandkids, and adults moved back home with their aging parents. Some seniors needed to move in with their children to make ends meet.

A study of data from the Rand Corporation found that, of the four million children living with their grandparents in the United States, 2.5 million live in three-generation households. Nearly 1.5 million live in split-generation households or ones in which grandparents are raising their grandchildren. The proportion of all grandchildren living in three-generation households, 3.6 percent, has been steady in recent years.

Research suggests that split-generation households are usually formed when parents are no longer able to take care of their children because of physical or mental illness, substance abuse, or economic problems. Three-generation households, on the other hand, are generally formed because of problems parents encounter living independently, such as through separation or divorce or due to unemployment or economic need.

Caring for their grandchildren can elicit many feelings in grandparents, from nervousness to excitement about a fresh face around the house. Raising grandkids can be overwhelming for elderly men and women, but the following are a few tips that can make the process easier.

· Explore your feelings. When you acknowledge your feelings, you are on the right path to making things work and recognizing possible obstacles.

· Expect mixed feelings from others. Grandchildren and your own children also may be apprehensive about this new living

situation. Encourage everyone to share their thoughts and come to a consensus on how things will be done. Expect it to take some time to establish a schedule, and don’t be discouraged by any initial behavioral problems.

· Take care of yourself. Grandchildren, particularly young ones, can have a lot of energy and may require constant attention. Caring for such lively youngsters can be taxing on grandparents, who must make their own health and nutrition a priority. Give yourself some time for recreation and rest. Have grandchildren help out where they can. Don’t feel you have to spend every moment entertaining them.

· Ask for help when needed. Reach out to friends or community members if you are feeling overwhelmed. There are a number of resources available to you, and many organizations, including AARP, have their own tips for assisting three-generation households.

Multi-generational households are common once again. Families who work together can make the most of such living arrangements.

Prepare your home to sell

Every morning, Dr. Gerald Benjamin, or Dr. Ben, as he is more widely known stands at the front door of St. Agnes School - rain, snow or sunshine - and greets the children by name. Doling out hugs, encouragement, and genuine smiles, Dr. Ben is the living embodiment of the school’s core valuesórespectfulness of others, kindness, and a sense of appreciation for all people. As the children line up to say their eager “hellos,” it is difficult for them to imagine beginning the day at their beloved school without taking a moment with this gentle man, but that day is fast approaching. After nineteen years at St. Agnes School, Dr. Ben has announced his retirement. Dr. Ben began his career 57 years ago as an intern teacher for the disabled at Massachusetts Hospital School, and demonstrated his commitment to quality education for all in a variety of roles including Assistant Professor of Education at Boston University and the University of Virginia and Principal at Geneseo Central, Cambridge Central, and Caledonia- Mumford Central schools. He established both the scholarship and summer education programs at St. Agnes School, improving educational outcomes and access to resources for its students. Dr. Ben has created a curriculum that educates the whole child, adding Spanish, Technology, Art and Page Turners. A dedicated supporter of the arts and the greater community, Dr. Ben has long been involved with many area organizations, including the Paralyzed Veterans of America, the Open Door Mission, Teresa House, and Camp Good Days. As Principal of St. Agnes School, he has organized cultural programs for school and community that range from rock cello performances to RPO classical brass to Chinese

A beloved member of the community, Dr. Ben is the recipient of multiple awards, including the American Legion Americanism medal, the Genesee Valley Council of the Arts award, and the New York State Distinguished Leader of a Private School Award, and has been honored by the Rotary Foundation of Rotary International as a Paul Harris Fellow in recognition of his efforts to further better understanding among diverse people and his philanthropic efforts. In addition to a student celebration, a community Open House will be held in Dr. Ben’s honor on Sunday, July 17th, from 1-4 p.m. at the Riviera Theater on Center Street in Geneseo. What’s with these men nowadays who get women interested in them, then drop them cold? It had gone so far as to saying “Hello Darling” on the phone, hugging me many times whenever he saw me, etc. Just because I’m as old as I am doesn’t mean that I don’t have feelings and can’t be hurt. If you have no intention of following through on your first interest don’t start anything in the first place. Stay in the situation that you’re in at that time and forget anything else.

You WERE being loud and rude on your phone when the women nicely asked you to please take it out into the hallway. I applaud her for sticking up for us who are tired of this behavior. Yet this is why many of us won’t say anything to people like you. The excuse you had?

You were making a call for work regarding a disabled person you work for? Lady, with that mouth and attitude I would be ashamed of having you represent an agency that violates persons’ HIPPA rights in front of others and uses the language you did! If this is how you act when you don’t have an individual with disabilities with you, I would seriously be concerned how you treat them when no one else is around.

I live in [local town] and the other night a neighbor veteran had called the VA with severe issues. All living in the area were not allowed to go to our homes as police handled the situation. When the local news stations showed up some of them were, let’s say, very unprofessional in their language. I understand this is their job but they were working in our neighborhood. The man the police were dealing with has served our country! My hope and prayers go out to all our veterans in hopes they can receive the help they need. I am so grateful to live in this country.

Last Wednesday I was turning lef t on a green arrow that had just turned yellow when an oncoming car came straight at me out of nowhere. I was very shaken but thanks to my airbag and seat belt not badly injured. My car, however, was totaled. A young woman who had been walking or jogging when the accident occurred came up to my car immediately after the crash to see if I was okay. She told me I had had a green arrow and she stayed with me until the paramedics and police arrived. I believe she gave a statement to the police, but I did not get her name. I asked one of the firemen to thank her for me as she was walking away, but he said he could not catch her. Thank you for your caring and kindness. It was a blessing!

Honest people do exist. Just a thank you to the person who found the Arcticloth that I had dropped while getting into my car on May 28th, a ninety degree day. I noticed I did not have it when I got to the other side of town. I called Brockport Rite Aid where I had purchased it and was told that someone had found it in the parking lot and turned it in.
