Pre-Trip/Post-Trip Form

When arriving to work, Drivers are required to fill out a Pre-Tip Form which outlines all important parts of our vehicles. As a Driver, you want to make sure that everything on the vehicle is in working order.

You should never operate a vehicle without doing a full inspection. We want to be sure that we can transport our clients in a safe and timely matter without any service or mechanical issues.

Pre-Trip/Post-Trip FormOn the back of the Pre-trip form, are vehicle diagrams where you are to notate any damages to the vehicle. Whether its scratches, dents, paint marking and any loose parts that are showing/hanging from the vehicle. This way we can get the required work done to create a safe ride for drives and our clients.

Before taking your vehicle on the road, It is very important that you check the following fluids:

Checking Fluids (Cont.) At any time while doing your

pre-trip on your vehicle and you are noticing the one or more fluids are low, notate the following on your pre-trip sheet and report to operations as soon as possible so the supervisor can take the necessary steps to get those fluids filled before you began to drive on the road.

You then will get permission to transport the vehicle to the Maintenance Department where a Service Associate will assist in topping off the fluids.

Verification Of Work Performed

Mechanic Performed• Performs work by topping off all

necessary fluids. • Performs routine oil change and

notates a sticker on the windshield of the vehicle.

• Outlines service report sheet to verify what problems were corrected.

Driver Verified with Mechanic• Verifies that all fluids are filled up. • Verifies oil change has been

performed, check oil dip stick for cleaner oil and notating sticker.

• Driver writes on Pre-Trip what was performed on the vehicle for driver documentation.

Wheelchair Lift

The Wheelchair lift is the most important piece of machinery that we have on our vehicles when it comes to handling clients that need wheelchair assistance. This is also a key part that is also involved in the Pre-Trip. You must make sure you check that you lift is able to Fold, Unfold, Up, and Down.

Each lift is also equipped with a manual system in case of failure of the lift not operating in automatic. Also when operating the lift, the emergency brake must be apply and the vehicle in park and idle in order for the lift to work.

Wheelchair Lift (Cont.) The Lift operates in two ways:

Automatic and Manual. In the event that the lift goes out while in automatic mode, the manual mode is another option. You must check both to make sure that they are operable. The lifts are also safety sensitive meaning that the front and back flat must be clear from anything blocking them while using the lift or the lift will not move

Any defects that you may see on the lift or any noise you may here that is not normal, write the information on your pre trip sheet so it can be later investigated by the maintenance department.

Special and Emergency EquipmentEach vehicle is equipped to have Sur-Loks for wheel chair securement when you have wheelchair clients aboard and also Emergency equipment which contains:

First Aid Kit Biohazard Kit Flares Triangles Fire Extinguisher Seat Belt Cutter

It is very important that you make sure your vehicle has these items during operation at all times.
