Dear Teachers!

Collectively, we've managed to keep undergraduate GP learning alive through the pandemic. We're looking now to what we can offer for 2021-22. From next year, GP teachers will be providing over 20% of clinical undergraduate teaching for Bristol Medical School and as a result our whole enterprise is moving onto a more professionalised footing. We really want you to feel part of a university teaching community.

Our fifth year placement, GP5, is expanding from four to nine weeks and all eyes are on Primary Care to make this a success. This follows the old-school apprenticeship model with the students making a tangible contribution to service delivery.

The other big innovation for next year is our Academies Packages. These signal a new style of recruitment. Instead of practices signing up for single elements they commit to a package of teaching across Years 3, 4 and 5. Packages come with substantial financial compensation, and with monthly payment guaranteed, open a new era in workforce planning.

We continue to receive startlingly good feedback from our placement students. For instance, with GP3, 94% of students rated their enjoyment at 4 or 5 out of 5. Our students consistently appreciate your efforts. Let's hope that 2021-22 will see our biggest contribution yet to the formation of well-rounded, generalist, graduates for mother NHS.

How to use this document

The first page is aimed at those who are new to teaching. The next page covers Teaching Packages, followed by a flow chart to help with your decisions around teaching. Then there is an introduction to each year from the year leads followed by the infographic that summarises teaching in that year. The introduction contains the link to complete the teaching survey. The deadline for responses is 30th June. If you wish to take on one of our teaching packages, please still complete the survey for each year and email [email protected] to let us know that you wish to be considered for the package.

Covid-19

We are aiming for a return to face to face teaching for all years in September. Each year has a contingency plan should the pandemic situation deteriorate with the later clinical years being prioritised for face to face clinical teaching.

Payment

This year we are pleased to offer another increase in the sessional rate to £63 per student per session, reflecting a substantial increase from £54.40 over the last four years. For some practices not on a teaching package, we may need to provide fewer students than the standard placement number, which would result in a proportionately lower payment rate. E.g. if you had 5 students for Year 1 instead of 6, we would only be able to pay you for 5 students.

Why teach? • GPs find teaching students fulfilling and enjoyable

• It helps GPs and practice staff stay on their toes and up to date

• Being a teaching practice helps recruit new doctors

• It provides an opportunity to promote a career in general practice

• Medical students give the practice professional kudos in the eyes of patients

What sorts of teaching can I do? There are different styles of teaching across the five years including small group tutorials, directly observing students consulting, and having students observe you consult. There are workplace based assessments that some students will need to complete and if you are interested, you can help with end of year exams.

When can I teach? Each year we recruit around May for the following September. The academic year usually runs from September to June. Don’t worry if you’ve missed the recruitment deadline, though, as opportunities to teach arise throughout the year. If you’re interested, email [email protected]

How am I supported? Each year has accessible GP year leads. We run a programme of workshops for new and experienced teachers as well as providing detailed handbooks and regular email updates. Welcome to Years 1 and 2

We warmly invite you to teach Years 1 and/or 2 of the medical undergraduate programme. Students’ feedback on their early patient contact and exposure to general practice continues to be overwhelmingly positive. We know that many of you really enjoy this teaching too. The infographics on the following pages provide an overview of what’s involved, including the dates. Students are taught in groups of 6. Detailed teaching plans are provided, and we will be running a teacher workshop in September. We are planning to return to face-to-face teaching in practice from October 2021. However, we have contingency plans for a mix of online and face to face teaching if this is not possible. If necessary, we can revert to short-term teaching online via MS Teams as we did in 20-21. We are happy to answer any questions and discuss concerns or teaching options now. Equally, we

will communicate regularly, and support you throughout the teaching block.

In Year 1, students have 7 or 8 sessions in GP on a

Thursday morning or afternoon. These fortnightly

sessions form part of a course called Effective Consulting

which explores clinical reasoning, clinical communication

and clinical skills. Effective Consulting is integrated into

students’ case-based learning cycles (these are fortnightly

cycles covering Human Health and Wellbeing).

Typical session

How the placement works

6 students per placement. You can choose whether they

come out on a Thursday morning or afternoon. In Year 1,

students are Bristol based and so their placements tend to

be at GP practices in, or very close to, Bristol. The course is

split into two parts: Foundations of Medicine and Human

Health and Wellbeing, although the structure of the

sessions in GP remains the same.

Foundations of Medicine

In October/November, students attend fortnightly on four

occasions as part of an introductory course called

Foundations of Medicine which introduces them to the

healthcare environment and talking with patients and

clinicians. Usually two of the students sit in on a reduced

surgery whilst the other students meet a patient (home visit

or in practice) to talk about their healthcare experiences. The

session ends with a 45 minute debrief of the students’

clinical encounters.

Human Health and Wellbeing

After Foundations of Medicine, the students come out for a

further 3 or 4 sessions on a monthly basis, focussing on a

different case or consultation skill each time. You can choose

whether you take students for 3 sessions (CBL3) or 4

sessions (CBL4). See overleaf for details of the themes and

dates. If possible, we would like the home visits to be to

patients relating to the particular theme; however the

reduced surgery can still be ‘undifferentiated’ patients.

of Year 1 and Year 2 students, as well as providing

Can more than one GP deliver the teaching? Yes, although we would prefer no more than two regular teachers per block.

I am a locum GP, can I teach year 1? Yes. However, we are only able to pay money to practices, not individual GPs. You would

also need to be able to arrange suitable patients for the students to visit.

In Year 2, students have 5 or 6 sessions in General Practice as a group of 6 students on a Thursday. This is on different dates to the Year 1 student placements.

At the start of Year 2, students have an introductory week to clinical learning, building on the early history and examination skills that were introduced in Year 1. The rest of Year 2 develops these.

Each session in General Practice is themed around a system or symptom.


GP tutors should invite two patients with conditions

relevant to the systems or symptoms the students are

covering, and help students practice gathering clinical

information through history and examination using the

Bristol derived COGConnect consultation model. There are

two streams: Stream A and Stream B which cover different

topics and come on different dates (see overleaf). Stream

B is now only 5 sessions as students are going to psychiatry

for the ‘Low Mood’ session.

Your choices

Ideally a practice will take one group in Stream A and one in

Stream B, covering all 11 dates, either in the morning or the

afternoon. Or you can opt to take one group (Stream A or B).

Hands on Practice

This is a key time for students to be hands on and really

practice their skills. They love to be actively talking to

patients and having a go at examination and clinical skills.

It’s important to set up the sessions at the beginning to

make it a “safe” space where they know it’s okay to make

a mistake, ask questions, and have another go.

Welcome to Year 3

We are delighted to offer you the opportunity to teach GP3 for the next academic year. Our

aspiration at the time of writing is to deliver this face to face in practices and the details

overleaf are designed to show you how this would work.

However, we are also pragmatic and we know that as the pandemic plays out there may be

a need to revert to online teaching as we have done this year. This would run via Microsoft

Teams and has proved a popular alternative with both students and teachers.

In GP3 we really strive to offer a very supportive teaching environment and as well as

fortnightly email updates, we also offer an online drop in session on the mornings of

teaching where the year leads are available to troubleshoot any questions or just have a

chat!

In Year 3, students have 16 days in General Practice on

Tuesdays. This placement consists of two blocks, each of

eight days. Seven of these are in practice. The final day in

each block is the Academy Day which comprises a

simulated surgery and an outside speaker.


Workshop with 6

students

Consulting Online Sway How the placement works

Each day in practice is split into a workshop in the morning

with 6 students, facilitated by the GP teacher. In the

afternoon, the students split into pairs A, B and C and

rotate between consulting with patients, observing

consulting and an online tutorial on Microsoft Sway.

Workshops

Each workshop is themed and has a detailed tutorial guide.

The main focus of the workshop is consulting and there will

be scenarios each week to work through. We will email you

a copy two weeks in advance of each workshop.

Effective Consulting in Practice (Consulting)

Each pair of students will consult for an hour during the

teaching surgery. All students will get to consult every week.

We suggest booking 30 minute appointments for the student

consultations. Each student should consult at least once each

day in practice. These can be telephone, online or face to

face appointments.

Online Sway Tutorial

Each pair will also have an hour to do an online Sway

tutorial so you will need a spare computer space for this.

Academy Day

This is a biannual, fun and celebratory event at the end of

each block. There is an actor based simulated surgery and an

outside speaker. Previously this has been face to face but is

likely to be online this year. This day is facilitated by the GP

tutor so it’s important you include this in your dates when you

are planning your teaching commitment.

Health Care in Action (Observing)

Each pair will also sit in with a member of surgery staff for an

hour. This could be a GP, nurse, HCA etc. The goal here is for

students to observe how the practice works in real time. Key requirements from the practice

excellent feedback.

Will material for the workshop be provided? Yes, we will provide detailed material for running the workshop. Think of it as following a recipe,

but feel free to innovate if you have your own ideas!

Welcome to Year 4

Bristol’s MB21 curriculum reached Year 4 in the middle of the pandemic and we had to rapidly adapt the new course. Our IN&OUT system and

creative use of IT made it possible to place all Year 4 students. We are extremely grateful to all our GPs who worked miracles to make Year 4

happen in 20-21, thank you very much.

GP4-PCC is an innovative clerkship with two GP placements, one in each academy. Students will be in their practice every Wednesday

throughout the academic year. The emphasis is on experiential learning, embedding with your team, authentic tasks, follow up of patients,

integration with secondary care learning and preparing students for Finals.

What will be the same for 21-22?

GP4 has shown itself adaptable and capable of delivering complex and

comprehensive active learning. Key elements will be retained:

2 ½ hours of formal teaching time every week

Two or three pairs of students per practice

Weekly email to GP teachers and students

Fixed order for tutorials, Skill and Drug of the week

Students pre-learning content for the formally taught sessions

GP4-PCC is one clerkship with two GP placements.

Students will spend every Wednesday in Primary

Care. In total, students will be in practice for 36 (2x18)

Wednesdays.

Students will have 4 half-day central online sessions,

see dates on the next page. They will be in surgery on

the morning of those days.

The GP4-PCC clerkship integrates closely with

students’ secondary care clerkships, see schematic.

Typical day in practice - The building blocks

2.5 hours teaching time plus observation


Each Wednesday four or six students will come to your

practice. We would like students to have two to three hours’

clinic time, one to two hours’ independent study and two

hours of allied health/community time/fortnightly tutorial.

Practices will have flexibility how to organise these activities.

Consultations Each day in practice should include a surgery

(morning or afternoon). These should include observed and

student-led surgeries (in pairs).

Community/Allied Health Year 4 is a key opportunity for

students to learn from allied health professionals in a

community setting, including practice nurses, pharmacists,

midwives, health visitors, social prescribers and more.

Assigned patients During each GP placement students

should be assigned patients for follow up, tying in with their

secondary care clerkships.

Independent study time Students should have independent

study time to meet their learning outcomes, following up

patients, undertaking community and governance projects,

researching questions and reading, resources provided.

Assessment 2 reviews, 1 MiniCex, 1 CBD/student/placement

4th year students should be an asset to the practice. They

are able to assist in health assessments and authentic tasks,

e.g. phlebotomy, learning disability health checks.

How many students? 2 pairs (4) or 3 pairs (6) of students

Can more than one GP deliver the teaching? One GP needs to take responsibility, oversee the teaching and provide continuity. Other GPs, team members and allied health

professionals can contribute to teaching. It is hoped that all team members will welcome the students and help to facilitate their

learning.

Am I required for the online GP4 sessions? No. Online GP4 sessions will be run by Primary Care and invited speakers.

What if I’m away during the 18 weeks? We appreciate you will have annual leave. Please arrange for a colleague to organise and supervise the students when you are away.

Welcome to Year 5

Bristol’s new MB21 curriculum will be reaching Year 5 this September and we are very much hoping that you will want to

be part of this exciting new teaching opportunity.

Students will still come to you in pairs for a block placement as part of their ‘Preparing for Professional Practice’ course

and the structure of the placement will be very similar to the previous few years. However, the students will now come

for a 9 week placement instead of 4, starting in November, January and March. We hope this will give the students the

opportunity to really embed into your team and contribute to the day to day running of the practice. You will need to

provide opportunities for them to consult with patients, observe them, give feedback and provide general supervision.

Students will be out of practice for the whole day every Wednesday for group teaching and private study. For practices

also wishing to host Year 4 students, please note that this means there will be no overlap with Year 5 teaching.

Payment for the placement is £7,938 per pair of students. The infographic below provides further information, and we

In Year 5 students will undertake a 9 week block placement

in primary care. They will have completed finals at the end

of Year 4 and will have just returned from an 8 week

elective.

It is an apprentice style placement as part of their

Preparing for Professional Practice (PPP) course which also

includes 9 weeks of both ward based care and acute and

critical care.

The main focus of the primary care placement is on the

student consulting with patients independently and

practising decision making.

Aim of the Placement

To prepare students for working as an F1 doctor by learning

in the Primary Care setting.


Students will come in pairs for a 9 week block. They will be

out of practice every Wednesday for small group teaching.

There are no prescribed teaching themes and the placement

can be designed flexibly to meet the individual students’

learning needs. We suggest a minimum of 4 student-led

surgeries and one observed surgery each week. The students

will also undertake a small project for the practice. Assigned Patient

We would like you to suggest at least 1 patient to the

students to follow-up during their attachment. Examples

include a recently discharged patient or a newly diagnosed

patient with diabetes.

The students will have one session a week to plan and

deliver a community based project. They will have some

central teaching from the university to support this.

Student led Surgery

Students have their own list and consult in pairs or alone

followed by a senior review after each patient.

Assessment

The students need to demonstrate satisfactory attendance

and engagement with the placement. The students will also

need to complete at least one satisfactory Mini-Cex

examination during the placement. This is an observed

consultation to the level of an F1 doctor.

Observed Surgery/Tutorial

Students consult individually with direct observation and

feedback from student colleague and supervisor.

Community/Allied Health

Students can spend one session a week with an allied

health care professional. This could include, but is not

limited to, practice nurses, district nurses, pharmacists,

midwives, health visitors, drugs workers, MSK practitioners

and social prescribers.

Year 5 Frequently Asked Questions

How many students? We would like you to take one pair of students per block. You are welcome to take a single student but the payment will be half of

that advertised. If you have capacity you are welcome to have a second pair of students per block but they will need to have their

own supervisor and consult independently to the other student pair.

Will students need their own room? Yes, we would suggest that students consult independently in pairs or alone at least 4 sessions a week. We would expect them to

have their own list of patients to consult prior to a senior review.

Can more than one GP deliver the teaching? One GP needs to take responsibility, oversee the teaching and provide continuity. Other GPs and allied health professionals can

contribute to teaching. It is hoped that all team members will welcome the students and help to facilitate their learning.

What if I’m away during the 9 weeks? We appreciate you will have annual leave. Please arrange for a colleague to organise and supervise the students when you are

away.

Can I change the timings of the day? You can arrange this with the students. We appreciate that a standard GP day is longer than an average working day. We are

happy for students to start later and finish earlier on some days as long as they complete 32 hours a week with you.
