The estrogen-mimicking compound, BPA, has sent ripples through the dental industry. At the epicenter is a study published in the July 2012 issue of Pediatrics, which found a potential link between BPA in dental composites and emotional behavioral problems in children. While more research is underway to determine the study’s efficacy, the BPA controversy is not new in dentistry.

a historY oF BPa iN deNtistrYSince the first composite resin was introduced in the 1960s, composite chemistries containing BPA, bisGMA and bisDMA have been developed. Within the last 25 years, researchers have reported detectable levels of BPA in patients treated with dental sealants. In 1996, Nicolas Olea reported detectable levels of BPA (3.3 to 30 ppm) in patients’ saliva treated with dental sealants (Herman, 2010). In 1999, Arenholt-Bindslev found BPA ranging from 0.3 to 2.8 ppm immediately after sealant placement, but no BPA was found one hour and 24 hours after placement (Herman, 2010). In 2000, Fung found BPA ranging from 5.8 to 105.6 ppb in saliva samples one hour and three hours after application. No BPA was found 24 hours after application (Herman, 2010).

BPA caused widespread alarm outside of dentistry in 2010, after several studies linked the chemical to health problems, prompting many manufacturers of plastic bot-tles, particularly those used by children, to exclude the substance from their formulations. Similar findings in the dental industry emerged with a controversial study pub-lished in Pediatrics. The study concluded that dentists could use BPA-laced dental composites safely in kids if they wiped or rinsed away residue after treatment, but determined that pregnant women should postpone any dental restorative procedure until after delivery. The FDA called for more research on the chemical, explaining that the agency had “some concern about the potential ef-fects of BPA on the brain, behavior and prostate glands of fetuses, infants and children” (MSNBC, 2010).

the curreNt studYIn the U.S., depending on their age, 20% to 40% of children had been treated with composite resins between 1999 and 2004 (Cosmetic Dentistry Guide, 2012). The high

prevalence of dental composites among children inspired the research behind the controversial study published in July. Researchers analyzed 534 children between the ages of 6 and 10 who had amalgam or composite fillings, one of which contained bisGMA, a chemical that hydrolyzes to form BPA.

After five years, parents and participants answered a series of questions about behavior, performance, attitude, anxi-ety, and depression. Researchers found that children with the highest exposure to BPA-based fillings scored worse on behavioral assessment tests and had more emotional problems — like anxiety and depression — than their coun-terparts. In all, 16% of kids with the most BPA exposure had behavioral problems. Children who had fillings on the chewing surfaces of their teeth had a higher points score, which may suggest that chemicals start leaching out as a result of the teeth constantly grinding and chewing.

Lead researcher Nancy Maserejian, from the New England Research Institute in Watertown, Massachusetts, said that although a link was identified, it was important to note that the research team did not measure the level of BPA in fillings and details of other chemicals in the fillings were not taken into account. She also stressed that the effect was “small” (Cosmetic Dentistry Guide, 2012).

alterNatiVes to BPa Some toxicologists, however, suggest that current recom-mendations don’t go far enough. Fred von Saal, professor of biology at the University of Missouri at Columbia and a leading expert on BPA, stresses the importance of avoid-ing BPA, especially for children. “There are lots of sources of BPA and you want to avoid anything that adds to your body’s burden. And the younger you are, the more sensi-tive you are to this chemical” (MSNBC, 2010).

The Downfall of Reusing Disposables

In recent years, disposable products have become the gold standard of infection control protocol. These single-use items are intended to be used on one patient and discarded. They should not be sterilized in an autoclave due to the heat-intolerance of their materials. Compared to reusable (or multi-use) products, disposable products reduce the probability of patient-to-patient cross-con-tamination and potential cross-infection.

In the not-so-distant past, “reusable” was more common than “disposable.” You may even remember when proce-dural items, such as saliva ejectors and prophy cups, were cleaned and sterilized to be reused on another patient. Since then, the FDA has repeatedly stated that it is un-aware of any data that would establish safe and effec-tive cleaning/sterilization conditions necessary for reuse of any disposable device (OSAP, 2012). Similarly, the CDC advises using single-use devices on one patient only and disposing of them appropriately. However, it is still gener-ally accepted by dental professionals that many single-use products are safe to reuse if sterilized, such as… BursWhether burs are single-use or multi-use remains a con-tested issue. Cleaning can be difficult due to burs’ in-tricate physical construction, and repeated processing cycles can deteriorate the cutting surfaces enough to potentially break during patient treatment. These fac-tors, coupled with the knowledge that burs exhibit signs of wear during normal use, might make it practical to consider them single-use. A fair amount of resources are spent scrubbing burs and diamonds prior to sterilization, which equates to longer intervals between patients. From an infection control viewpoint, treating burs as single-use eliminates the risk of patient–to-patient cross–infection.

disPosaBle Mouth MirrorsPlastic mouth mirrors are great alternatives to traditional mouth mirrors in that they significantly reduce cross-con-tamination. However, they are not meant to be reused and their plastic materials are not suitable for the autoclave.

Because mouth mirrors are inserted directly into the pa-tient’s mouth, improper sterilization practices can greatly increase the risk of patient-to-patient cross-infection.

MasKsUnlike most disposable items, which are single-use in the sense that they may only be used on one patient, face masks are only as effective as their designated time limit. The length of time that a mask protects against infection and cross-contamination depends on its ASTM level*:ASTMLEVEL1 ASTMLEVEL2 ASTMLEVEL3

Why the wide discrepancy between what is regulated and what occurs in the practice? For one, complying with every OSHA and CDC guideline is easier said than done. And, because devices that were considered reusable twenty years ago are now deemed one-time use, adapting to new practices is slow to change. Plus, the misconception that disposables cost more could explain the discrepancy. On average, a dentist spends $20 in disposables to set up an operatory, which fluctuates depending on the number of patients seen per day.

However, the monetary savings gained by reusing disposables are minor when compared to the huge costs of an infection control breach. Although reuse of common disposable items may seem harmless, little things can accrue to a big problem. When it comes to infection control, loosening the slack in one area lessens the overlap and effectiveness of your total chain of infection control. Think of it as a line of dominos — once the first one falls, the rest fall along with it.

Bio-hazard aNd sharPs disPosalTHE PROBLEM: Many dental practices opt for a waste disposal pickup service that has no knowledge of dental regulations. As a result, dentists may be locked into confusing contracts without a point of contact that understands their compliance needs — leaving dental professionals responsible for understanding their requirements or risk non-compliance.

THE REGULATIONS: At least 12 states and numerous lo-calities have mandatory mercury pretreatment programs in place. At the federal level, the EPA recently announced that it would be formulating a nationwide rule regulating disposal methods for dental amalgam. This rule is expect-ed to be finalized in 2013.

THE SOLUTION: It is possible to eliminate mercury pol-lution by installing an amalgam separator. Installation is simple and allows dentists to eliminate 99% of their mer-cury waste, and maintenance usually involves replacing the canister once it’s full, generally every 6-12 months.

x-raY wasteTHE PROBLEM: Dental offices generate many different forms of waste as a result of traditional x-ray processing that can have a serious impact on wastewaters and soil.

THE REGULATIONS: The disposal of used fixer is strictly regulated nationwide due to its high toxicity level. However, many regulations can vary significantly by location.
