    QUEEN CREEK BARBERING SCHOOL

    STATEMENT OF COURSE CURRICULUM AND FEES

    To graduate a student must complete 1500 hours of school work which includes 300 hours oftheory and 1200 hours of clinical. If a student goes 40 hours a week it takes approximately40 weeks to finish the course. (If a student goes 48 hours a week it takes approximately 33weeks to finish the course.)

    Theory of BarberingHistory of BarberingTheory of MassageUse, care, and Sterilization of ImplementsTheory on Personal ImageryAnatomy and Physiology of the human body as pertains to barbering

    Practical hands on training on all areas of barbering includes:

    Regular Haircuts, Flat tops, style cuts, fades, perms, coloring, tinting, highlighting, stylingtechniques, shaving, facial treatments, scalp manipulation, and many other aspect of thebarbering skill needed to become a successful Barber/Stylist.

    Reference and Placement Assistance

    The school will provide charter or academic achievement references for graduates toprospective employers. All students graduating satisfactorily will receive a Diploma statingsuch. The school encourages students to avail themselves of this service. Queen CreekBarbering School and Supplies, keep in close contact with barber shops for job placement.However, the school DOES NOT guarantees employment.

    Cancellation and Refund Policies

    A. An applicant whose application for enrollment is rejected by this school for any reasonwill be refunded 100% of monies paid.B. An applicant may cancel enrollment of the course. An applicant not requestingcancellation by their scheduled starting date will be considered a student.

    1. All monies paid by an applicant will be refunded if requested within 3 businessdays after signing an enrollment agreement and making a payment.2. An applicant subsequently requesting cancellation will be refunded all moniespaid to the school minus the registration fee of $200.00.

    C. A student termination will be considered to have occurred the last day of actualattendance scheduled for computing refunds.1. For a student terminating his training after having entered school and started the

    course of training, but within the first week of the course, the tuition charges made bythe school to the student will e 10% of the contract price of the course plus $200.00,but no event more than $350.00.

    2. For a student terminating training after one week, but within the first 25% of the

    course, the tuition charges made by the school shall not exceed 25% of the contractprice of the course plus $200.00.3. For a contract terminating after 25% but within 50% of the course, tuition charges

    made by school shall not exceed 50% of the contract price of the course plus $200.00.4. If attendance is more than 50% of the course, there will be no refund.

    For students terminated by the school:1. If a student is terminated by the school for misconduct, tuition is as above, orstudents terminated from misconduct may at the discretion of the director be allowedto reenter.2. A student who fails to maintain an 80% grade average or minimummonths/hours for two consecutive months will be interrupted; re-admittance is at the

    discretion of the school. If re-admitted, minimum standards must be kept monthly.3. A student who does not make timely tuition payments due to the school will beterminated for unsatisfactory conduct.4. A student determined to be unattainable by the school is given a partial refund.

    E. Students Termination; in case of student prolonged illness, accident,death in the family, or other circumstances that make it impractical to complete thecourse, the school shall make a settlement which is reasonable and fair to both.F. Veterans: a veteran who does not begin the course will be refunded allmonies paid to the school minus a $25.00 registration fee.G. Percentage of course completion shall be computed on the basis of theamount of time the student could have gone up to the last day of attendance.

    Tool Kit Policy

    The cool kit is disbursed when the student begins their training program, providedthat the necessary down payment and payment requirements have been met. If thestudent withdraws from the course or is terminated there is a full refund of the unusedtools which are returned. All students accounts must be paid up before the tools mayleave the school.

    The school herein for monetary consideration as stated herein contracts with the student to provide thestudent a course of study to include 300 hours of Theory and 1200 hours of Practical Study asmandated by the Arizona State Board of Barber Examiners, and as set forth in the school of CourseCurriculum Fees as attached to and incorporated into this contract.

    The school also agrees to provide instructional and practical study from completion of the 1500 hoursbasic course up until the first scheduled Arizona State Board of Examiners exam date following thestudents completion of the contracted course. However, no credit will be given for these additionalhours of instruction unless they are paid for in accordance with School Statement of CourseCurriculum and Fees. After the first of the contracted hours, there will be additional charges as set

    forth in the Schools Statements of Course Curriculums and Fees.

    The School will be flexible in the scheduling of students days of school, but the student is required tobe at school for the hours he/she is scheduled for, or face disciplinary action. All changes in schedulemust be approved at least one week in advance of the first change.

    Student agrees to abide by the Rules and Regulations as posted by the School and attached to, andincorporated into this contract as part of this contract, and any other additional rules that the schoolmay deem necessary to implement in the future.

    Student agrees, if deemed necessary, to present himself/herself at any of our satellite shop required

    hours while at the satellites will be credited. Compensation other than student tips will be property ofthe school.

    THE BARBERING COURSE PURCHASED SHALL BE AS FOLLOWS:

    Payment Arrangements are to be as follows:

    Down payment to be $1,500.00 (in cash) due upon entering collegeMonthly payment of $500.00 to be made on the 1st day of each month. Remaining balance due prior

    At any time, we may ask you to provide a co-signer for your payment arrangementsPayments made five days after due date will be assessed a $5.00 per day late charge.Interest will be charges at the going national rate for anyone faltering on payment arrangementsIf allowed to graduate prior to balance being paid in full, an automatic 10% increase in Contract

    price will occur. Please make sure that you understand this clearly.Please note that your entire contract amount must be paid in full (with no exceptions!!!) prior to you being allowed to take your State Exam

    unless approved in writing by Owner.
