•By individual beneficiary we mean people
who have taken a broadband connection
from USOF . This category includes:
• Individuals (personal use at home)

• 3-Data Regarding Equipment

the purpose of using the internet services.

Broadband
•By Non-users of broadband we mean
people who are currently not using the
internet/Broadband either because they

they are not going to a kiosk for using
internet/Broadband, but who have used
the internet/Broadband in the past.

•This questionnaire is for the Sarpanch of
Village Panchayat
• In case the Sarpanch is not available
anyone from the Panchayat office who
can respond can be contacted

• 2-Basic Data-Inspection
• 3-Specific Details
Online Questionnaire Basics
• The webpage has four navigation keys-1. Next
2. Previous
3. Resume Later
4. Exit Survey
• The answers will not be submitted (recorded) till the
“submit” button is pressed at the end.
• Resume Later button allows to continue the form later.
•Once ‘submitted’ the form cannot be reopened.
•While resuming the filling of the form there are
two options-
•You will receive the link in email with user id
and password
•You may use the same link and resume filling
the form. In this case the form will show the
“Load Unfinished Survey” button in the place of
“previous” button
