Status bei Eintritt:

- 61 jähriger Patient, BD 91/73 mmHg, P 86/min, AF 16 /min,

periphere Sauerstoffsättigung 91%, GCS 14,

- Kreislauf: Herztöne leise, rein, gestaute Halsvenen, prätibiale

Oedeme, Peripherie kalt

- Lunge: VAG, keine Hinweise für Pleuraergüsse

- GIT: spärliche Darmgeräusche, Abdomen gespannt,

epigastrische Druckdolenz. Sklerenikterus

- Allgemein verlangsamt, apathisch

Beurteilung:

Rechtsführende Dekompensation mit rechtsseitiger intra- und

extravaskulärer Hypervolämie und hepatischer Stauung und low

output mit Zeichen der Organminderperfusion

Procedere Med. Intensivstation:

- Beginn mit Simdax, 0.1 ug/min/kg/KG

- RS mit Nephrologen aktuell kein Dialyse aufgrund kardialen

Komorbidität, hatte zuvor suffiziente Diurese. Versuch mit

Lasixperfusor mit einer max. Minusbilanz pro Tag von 700-1000 ml.

- optionale Möglichkeiten: HTx, Herzersatztherapie
