RtI Problem Solving Team Meeting

AGENDA

Tier I Review of Data Meeting: (Note: Held ONLY for students prioritized for review and whose

required documentation is complete.)

Δ RtI Coordinator chairs meeting, states purpose of meeting, and allows time for introductions. Δ Teacher presents a description of the problem and supporting documentation including health

information. Δ Team selects strategies and progress monitoring instrument/s from menu link OR district website

to IMPLEMENT or CONTINUE in core instruction (10-12 weeks/6-8 weeks speech/language). Δ Specific plan for instructional services, programs, and recommendations are reviewed and agreed

upon and documented. Δ Coordinator schedules follow-up meeting, if appropriate, and ensures parent is provided a copy of

RtI documentation to date.

Tier II Meeting: (Historical and current data indicators guide movement to Tier II.) Diagnostician,

LSSP, or SLP must review documentation upon movement to Tier II (RtI Consultation Form may be attached in lieu of signature.) Dyslexia-504 Coordinator should be present if recommending dyslexia assessment.

Δ Coordinator chairs meeting, states purpose of meeting, and allows time for introductions. Δ Teacher shares progress monitoring reports and other available data. Δ Team selects Tier II intervention/s and progress monitoring instrument/s from menu link OR

district website. Specific plan for intervention and recommendations are reviewed and agreed upon. Coordinator documents recommendations.

Δ Coordinator schedules follow-up meeting, if appropriate, and ensures parent is provided a copy of RtI documentation to date.

Tier III Meeting: (Data showing no progress guides movement to Tier III.) Diagnostician, LSSP, or

SLP, must review documentation upon movement to Tier III (Presence is required for testing recommendation.)

Δ Coordinator chairs meeting, shares purpose of meeting, and allows time for introductions. Δ Team selects Tier III intervention/s and progress monitoring instrument/s from menu link OR

district website. Specific plan for intervention and recommendations are reviewed and agreed upon. Coordinator documents recommendations.

Δ Coordinator schedules follow-up meeting, if appropriate, and ensures parent is provided a copy of RtI documentation to date.

Reminders: Δ All RtI process documentation needs to be entered on the RtI Interactive Online System.

Supporting documentation should be scanned and attached. Δ For Section 504 referral, document medical or physical impairment that “substantially limits” a

major life activity at Tier I and move to Tier III to recommend 504 evaluation. Δ Dyslexia assessment may be recommended at Tier II. Student remains in Tier II pending results.

Dyslexia-504 Coordinator schedules appointment with Dyslexia Department to submit referral, along with required documentation.

Δ For special education referral, evaluation staff is responsible for ensuring all required documentation is secured.
