Self Carding Coaches Through US Club

Required Documents

1. Completed background screening.

2. Proof of completed concussion course.

3. Completed Lindsay's Law form.

4. Proof of completed SafeSport course.

5. Proof of completed SideLine Sports course.

6. OCL Code of Conduct Form

7. 1x1 Photo of coach

Once you have completed all the required documents and online courses, you may enter the coach into the system by clicking STAFF then selecting Add New Stuff from the drop down menu.

Fill in the information for the coach and click submit once finished.

You do not need to fill the Background Check text field to request the pass. Just know that the pass will not be approved until the results come back.

Requesting the cards for the coaches once you have all the proper information entered is the same as requesting cards for the players. Refer to the power point for carding players for any questions about this process.
