Stand by includes all time in excess of one-half (1/2)hour during which Telephone Company personnel stand by tomake installation acceptance tests or cooperative testswith a customer.

Testing and Maintenance with Other Telephone Companies

Additional labor charges apply for additional testing.maintenance or repair of facilities which connect tofacilities of other telephone companies. This is inaddition to the normal effort required to test, maintainor repair facilities provided solely by the TelephoneCompany .

Testing Services

Testing Services other than those described in otherparts of this tariff will be provided on a time andmaterial basis, if requested by the customer. Testingwill be provided subject to the availability of equipmentand qualified personnel.

Other Labor

Other labor is that additional labor incurred to accommo-date a specific customer request that involves laborwhich is not covered by any other section of this tariff.It also covers additional labor necessary to meet cust-omer requests as described in Section 5. Other Laborincludes, but is not limited to, the provisioning to thecustomer, additional or duplicate reports or documentspreviously provided to the customer. Charges will bedeveloped on a time and material basis .

9.3 Miscellaneous Services

9.3.1 Testing Services

Testing Services offered under this section of the tariffare optional and will be billed to the customer on a timeand material basis by the Telephone Company. Othertesting services, as described in 6.6.8 preceding, areprovided by the Telephone Company in association withAccess Services and are furnished at no additionalcharge.

•Testing services are normally provided by TelephoneCompany personnel at Telephone Company locations.However, provisions are made in (B)(2) following for acustomer to request Telephone Company personnel toperform testing services at the customer designatedpremises.

The offering of Testing Services under this section ofthe tariff is made subject to the availability of thenecessary qualified personnel and test equipment at thevarious test locations mentioned in (A) and (B)following.

(A) Switched Access Service

Testing Services for Switched Access are comprisedof (a) tests which are performed during theinstallation of a Switched Access Service, i.e.Acceptance Tests, (b) tests which are performedafter customer acceptance of such access servicesand which are without charge i.e., routine testingand (c) additional tests which are performed duringor after customer acceptance of such access servicesand for which additional charges apply, i.e.,Additional Cooperative Acceptance Tests andin-service tests.

Routine tests are those tests performed by theTelephone Company on a regular basis, as set forthin 6.6.8 preceding which are required to maintainSwitched Access Service. Additional in-servicetests may be done on an automatic basis (noTelephone Company or customer technicians involved),on a manual basis (Telephone company technician(s)involved at Telephone Company office(s) andTelephone company or customer technician(s) involvedat the customer designated premises).

Testing services are ordered to the Dial Tone Officefor FGA, to the access tandem or end office for FGB(wherever the FGB service is ordered) and to the endoffice for FGs C and D. Testing Services forDirectory Assistance Service not routed through anaccess tandem is ordered to a Directory AssistanceLocation for each NPA.

(1) Additional Cooperative Acceptance Testing

Additional Cooperative Acceptance Testing ofSwitched Access Service involves the TelephoneCompany provision of a technician at itsoffice(s) and the customer provision of atechnician at its premises, with suitable testequipment to perform the required tests.

Additional Cooperative Acceptance Tests may,for example, consist of the following tests:

2. Additional Automatic Testing

Additional Automatic Testing (AAT) of Switched Access Services (Feature Groups B, C and D), is a service where the customer provides remote office test lines and lOS test lines with associated responders or their functional equivalent. The customer may order, at additional charges, gain-slope and C­notched noise testing and may order the routine tests (1004 Hz loss, C­Message Noise and Balance) on an as needed or more than routine schedule.

The Telephone Company will provide an AA T repmi that lists the test results for each trunk tested. Trunk test failures requiring customer participation for trouble resolution will be provided to the customer on an as-occurs basis.

The Additional Tests, (i.e., gain slope, C-notched noise, 1004 Hz loss, C­message noise and balance) may be ordered by the customer at additional charges, 60 days prior to the start of the customer prescribed schedule. The (e) (0) rates for Additional Automatic Tests are as set forth in 13.S.3(B) following. (e)(o)

Additional Manual Testing (AMT) of SwitchedAccess Services (Feature Groups A, B, C, and Dand Directory Access Service not routed throughan access tandem), is a service where theTelephone Company provided a technician at itsoffice(s) and the Telephone Company or cust-omer provides a technician at the customerdesignated premises, with suitable test equip-ment to perform the required tests. (B) Special Access Service

The Telephone Company will provide assistance inperforming specific tests requested by the customer.

(1) Additional Cooperative Acceptance Testing

When a customer provides a technician at itspremises or at an end user's premises, withsuitable test equipment to perform therequested tests, the Telephone Company willprovide a technician at its office for thepurpose of conducting Additional CooperativeAcceptance Testing on Voice Grade Services. Atthe customer's request, the Telephone Companymay provide a technician at the customer'spremises or at the end user premises. Thesetests may, for example, consist of thefollowing:

9.3.2 Maintenance of Service

(A) When a customer reports a trouble to theTelephone company for clearance and no trouble isfound in the Telephone Company's facilities, thecustomer shall be responsible for payment of aMaintenance of Service charge developed on a timeand material basis for the period of time fromwhen Telephone company personnel are dispatched,at the request of the customer, to the customerdesignated premises to when the work iscompleted. Failure of Telephone Companypersonnel to find trouble in Telephone Companyfacilities will result in no charge if thetrouble is actually in those facilities, but notdiscovered at the time.

(B) The customer shall be responsible for payment ofa Maintenance of Service charge when theTelephone Company dispatches personnel to thecustomer designated premises, and the trouble isin equipment or communications systems providedby other than the Telephone company or indetariffed CPE provided by the Telephone Company.

In either (A) or (B) preceding, no creditallowance will be applicable for the interruptioninvolved if the Maintenance of Service Chargeapplies.

9.3.3 Telecommunications Service Priority - TSP (N)

(A) Priority installation and/or restoration ofNational Security Emergency Preparedness (NSEP)telecommunications service shall be provided inaccordance with Part 64.401, Appendix A, of theFederal Communications Commission's (FCC's) Rulesand Regulations.

In addition, TSP System service shall be providedin accordance with the guidelines set forth in"Telecommunications Service priority (TSP) Systemfor National Security Emergency preparedness(NSEP) Service Vendor Handbook" (NCSH 3-1-2)dated July 9, 1990, and "TelecommunicationService Priority System for National SecurityEmergency Preparedness Service User Manual" (NCSM3-1-1).

The TSP System is a service, developed to meetthe requirements of the Federal Government, asspecified in the Service Vendor's Handbook andService User's Manual which provides theregulatory, administrative and operationalframework for the priority installation and/orrestoration of NSEP telecommunications services.These include both switched and Special AccessServices. The TSP system applies only to NSEPtelecommunications services, and requires andauthorizes priority action by the TelephoneCompany providing such services.

When the Customer requests an audit or a reconciliation of the Telephone Company's Telecommunications Service Priority records, a Miscellaneous Service Order charge as set forth in Section l3 .2.1 and Section 14, following, will be applied. Local exchange carriers will provide equal access interconnec- (C)tion to intrastate interexchange carriers according to theprocedures and schedules identified in this section. Presub-scription and access rules applicable to end users are alsoexplained in this section. Presubscription is the process bywhich end users may select and designate to the TelephoneCompany an IC to access, without an access code, forinterexchange calls. Depending upon availability, the end usermay presubscribe to different ICs for interstate and intrastatecalls. (~)

A. A local exchange carrier providing service to an exchange (L)(N)in a location identified in 3 AAC 52.355(a) (1) shallinstall and maintain plant, equipment, and facilitiesnecessary to provide 2-PIC dialing in that exchange uponreceipt of a bona fide request for interconnection.

B. If a local exchange carrier receives a bona fide requestfor interconnection in a location identified in3 AAC 52.355(a) (1) in an exchange Where interstate equalaccess is currently provided, it shall implement 2-PICdialing in that exchange within 90 days after receipt ofthat request. Upon implementation of 2-PIC dialing, theaccess line or lines of each end user will bepresubscribed to the incumbent carrier until the localexchange carrier receives authorization from the end userchanging presubscription for an access line or lines toanother certificated intrastate interexchange carrier. C. If a local exchange carrier desires a waiver of Part B. of (N)this section, it must file a motion with APUC for thatwaiver within 30 days after receipt of the bona fide requestfor interconnection and include a schedule that states when2-PIC dialing can be provided, the financial and technicalbasis for the waiver request, and a proposed public noticeof the interim dialing arrangements. The proposed publicnotice must explain the 1-PIC dialing arrangements, listcarriers available to provide both interstate and intra-state interexchange service, and describe the actionsnecessary to change a presubscribed access line to adifferent interstate interexchange carrier.

D. If a waiver of Part B. of this section is granted by theAPUC then the local exchange carrier shall implementI-PIC dialing for the period prior to 2-PIC implementationin accordance with the following rules:

1. Within 14 days after Commission approval of a waiverand associated pUblic notice, the local exchangecarrier shall mail copies of the approved notice to itsend users. Each intrastate interexchange carrier listedon the notice will be billed by the local exchangecarrier for an equal share of the cost of noticing.

2. The local exchange carrier shall implement I-PIC dialing45 days after Commission approval of the waiver.

3. Unless otherwise requested by the end user, uponconversion to I-PIC, an end user's interstate carrierwill automatically become the end user's primary carrierprocessing both interstate and intrastate calls.

E. If a local exchange carrier receives a bona fide request (N)for interconnection in a location identified in3 ACC 52.355(a) (1) but in which interstate equal accessis not currently available, it shall provide 2-PIC dialingat the same time and on the same schedule as requiredby the Federal Communications Commission for theimplementation of interstate equal access. In this event,presubscription balloting for intrastate interexchangecarriers will be conducted in coordination with ballotingfor interstate interexchange carriers.

1. At least 45 days in advance of the mailing ofinterstate interexchange presubscription ballots, alocal exchange carrier shall file with the APUC forreview and approval a proposed intrastate interexchangepresubscription ballot that lists the available intra-state interexchange carriers. The ballot must clearlystate that an end user may select different inter-exchange carriers for intrastate and interstateinterexchange services.

2. Pending the conversion to 2-PIC access, I-plus andno-plus intrastate interexchange calls must be routedto the incumbent carrier. Other intrastateinterexchange carriers shall obtain access throughFeature Group A or Feature Group B without discount, orthrough the incumbent carrier's 2-PIC equal accesstandem. (N)

3. On the effective date of this tariff, all existing end user$ have access to intrastate MTS/WATS. No later than 85 days prior to conversion to Feature Group D in a serving end office, the Telephone Company will notify end users of the availability of equal access in their particular area. The notification will include the intrastate ballot identified in section 9.3 A( e)(1). This notification will be sent via U.S. Mail to each end user of record served by the end office to be converted.

4. End users may select one of the following options at no charge:

- a primary interstate IC or No PIC for all of its lines,

- a primary intrastate IC or No PIC for all of its lines,

- a different interstate IC or No PIC for each of its lines,

- a different intrastate IC or No PIC for each of its lines.

Only one interstate or intrastate IC may be selected for each line or lines terminating in the same hunt group

End users may designate that they do not want to presubscribe to any IC. The end user must arrange this designation by directly notifying the local exchange carrier's business office. This choice will require the end user to dial an access code (1 o XXX) for all interstate or intrastate calls.

After the end user's initial selection of a predesignated IC or the designation that they do not want to presubscribe to any IC, for any change in selection after conversion to Equal Access in the serving end office, a nonrecurring charge, as set forth in 13.2.4 following applies.

5. End users not responding to the initial notification will be sent a second notification for the selection of a predesignated IC no earlier than 40 days prior to or no later than 90 days after the conversion to Equal Access in a serving end office. This second notification will indicate the default option of (C) No-PIC will be assigned to them if they fail to respond to the second notification.

End users assigned to a No-PIC by default may change their No-PIC one time (N) within six months after conversion to Equal Access in the serving end office (D) at no charge.

Following the six month period after conversion to Equal Access for any change in selection, a nonrecurring charge as set forth in 13.2.4 following, applies.

6. When an end user indicates more than one IC selection (C)on the return notification or returns an illegiblereturn notification, the Telephone Company willcontact the end user for clarification. If the enduser indicates an IC selection on the returnnotification that does not match with informationprovided by an IC and both notifications indicate thesame authorization date, the end user's notificationtakes precedence and the Telephone Company willprocess the end user's selection. In the event thattwo or more ICs provide to the Telephone companynotifications with the same authorization date andneither notification has been processed, theTelephone Company will contact the end user forclarification. A list of these customers in conflictmust be sent to the affected IC by the Telephonecompany.

In the event that two or more rcs have provided tothe Telephone Company notifications with the sameauthorization date(s), and one IC notification hasalready been processed by the Telephone Company,those IC notifications not yet processed would bereturned to the ICs.

H. If an IC elects to discontinue its Feature Group D Serviceoffering prior to or within 2 years of the conversion, the ICwill notify the local exchange carrier of the cancellation.The IC will also notify all end users which selected them thatthey are cancelling their service and that they should contactthe Telephone Company to select a new primary IC. The IC willalso inform the end user that it will pay the presubscriptionchange charge. The cancelling IC will then be billed by thelocal exchange carrier the appropriate charge for each end userfor a period of two years from the discontinuance of FeatureGroup D service.

I. Customer Notice of Access Options and Presubscription

For a period of three years after a local exchangetelephone carrier initially provides 2-PIC dialing, itmust annually include in its periodic billings or in aseparate mailing a notice to end users listing availableintrastate interexchange carriers and explaining theactions that must be taken to change carriers and the costof making the change. Each intrastate interexchangecarrier listed on the notice will be billed by the localexchange carrier for an equal share of the cost ofnoticing.

2. A local exchange carrier may not reassign an end (N)user's access line or lines to a different intrastatecarrier unless either the end user has signed awritten request for the change or the local exchangecarrier has verifiable direct contact with the enduser. A copy of the written request will suffice asa basis for reassigning an end user, so long as theLEC is immediately provided with the original writtenrequest in the event of a disputed reassignment. Blocking Service

900 Blocking Service

The Telephone company will provide 900 BlockingService to customers who obtain intrastate FeatureGroup A switched Access service under this tariff.This service is only provided at appropriatelyequipped end offices. Those offices providing 900Blocking Service are identified in NATIONAL EXCHANGECARRIERS ASSOCIATION, INC. TARIFF F.C.C. NO.4.

On each line for which 900 Blocking Service isordered, the Telephone Company will block all directdialed calls placed to a 900 number. When capable,the Telephone company will route the blocked calls toa recorded message.

900 Blocking Service is provided free of charge toSUbscribers. However, when a subscriber requests toend the 900 Blocking Service, the Miscellaneousservice Order charge will apply. The charge does notapply when blocking is removed from a Feature GroupA switched Access line at the same time it isdisconnected. (N)
