Instructions to Supplier for Completing form EMS-0196 Supplier Disposition Request

Responsible Person Block Title Description

Supplier 1 SDR No. & Revision

The SDR number consists of two parts. The first part is the GA-EMS PO number, and the second part is a three or more digit sequential number (for each PO) that is assigned by the Supplier, who manages an SDR number log to avoid duplication or gaps. A letter is used to indicate subsequent revisions of the SDR.

Supplier 2 Page 1 of __ Fill in the total number of pages in the SDR, including continuation pages. Include the attachment pages documented in Block (23) as part of the total page count.

Supplier 3 Part Nomenclature Part Nomenclature from the drawing or specification.

Supplier 4 DWG/Part No.

Enter the Drawing Number of the Part that is nonconforming, requiring information, or design change suggestion. List the lowest level part number that the Supplier has. If no drawing number exists, then list the applicable specification number.

Supplier 5 Rev. Proper configuration of the Drawing/Part per the Purchase Order (PO) (i.e., drawing revision + applicable Change Notice[s]).

Supplier 6 Location Current physical location of the part.

Disposition & Technical Justification

The Engineer/Liaison enters the disposition and justification. Standard dispositions and explanations are included in REF-0035, Nonconformance Disposition Definitions. The disposition addresses each condition listed on the document and the actions necessary to correct the situation, where applicable. Specific instructions addresses rework or repair, where required, and include in-process and/or final inspections and acceptance criteria. For hardware requiring source inspection, specific inspection sheets with locations for applicable sign off and stamps are included. If additional pages are required for a disposition with additional instructions, continuation sheets can be added. The technical justification explains why the disposition was selected and must provide a complete explanation of the condition that is understood by someone not familiar with the condition of the hardware. The justification must discuss all impacts of the disposition, including, but not limited to, effectivity, safety, performance, service life, site installation, maintainability, interfacing items. Where the disposition results in a nonconforming condition, supporting analysis is required to justify the condition. The analysis varies depending on the complexity and extent of the condition, ranging from hand calculations to a full-on finite element analysis. All analysis requires engineering review and approval prior to final document approval. If further investigation is required before final disposition, the request for additional information will be identified as “Preliminary Disposition”. For multiple items, number them with the corresponding item numbers used in Block (20).
