Transdermal Glyceryl Trinitrate Significantly Improves Exercise Capacity But there Is no difference between low and high doses in patients with angina pectoris In a double-blind crossover study, 15 men with stable effort angina randomly received placebo patches or single applications of transderma/ glycery/ Irinitrate [nitroglycerin] 2Ocm2 or 4Ocm 2 (releasing 10 and 20 mg/24 hours, respectively). Patches remained in place IOf 24 hours and there was a 24-hour washout period between each application. During bicycle exercise 4 and 24 hours after patch application, both glyceryt trrnitrate doses significantly increased heart rate and rate-pressure product compared with placebo. The 2 active doses also significantly prolonged exercise duration and time to 1mm ST-segment depression and improved maximal workload and total work performed 4 and 24 hours after patch application. There was no significant difference between the 2 glyceryl trinitrate doses in any of the variables measured. Headache was reported in 11 patients (8 after both active doses and 3 only after the higher dose) an average of 2.6 hours after patch application, and lasting 14.9 (range 1·24) hours. Thus, single applications of either transdermal glyceryl trinitrate 200m 2 or 400m 2 prolong exercise duration and time to 1 mm S1 depression for 24 hours, in patients with stable eflorl angina.
