8th Grade Field Trip

Dear Parents/Guardians:

On June 8, 2009, the eighth grade students will take a field trip to the Water Park at Bohrer Park inGaithersburg, MD. The students and staff are very excited about this opportunity to celebrate their successes at Crestwood Middle School.

Frederick County Public Schools make every effort to assure student and staff safety on field trips. Inorder to keep all students safe, we are requiring that all students return the permission form below.

Students are asked to pay a small amount toward the cost of this trip to help cover the cost of transportation. Please complete the form below and return it along with a payment of $5.00 byWednesday, June 3. Checks may be made out to Crestwood Middle School. Students that do not

submit a signed permission form and payment will not be allowed to go on the field trip.   Late

permission slips will not be accepted. All students planning to attend must hand in forms to their home

 base teacher by June 3rd. If you have any questions please do not hesitate to call Mrs. Patterson at (240)566-9101.

Students may pack a lunch, bring money to buy a lunch at the park, or the cafeteria can pack a lunch for your child. Please check the box at the bottom of the slip if you would like the cafeteria to provide alunch on the day of the field trip.

Crestwood Middle School is not responsible for damage to, or loss of, any items students bring on thefield trip. Students who misbehave on this trip will be disciplined accordingly and may lose the privilegeto participate in other eighth grade events planned for the school-year.

We are looking for parents/guardians who are willing to chaperone this trip. Please check the appropriate

 box below if you are able to chaperone.
