Inflammation: A unifying theory of disease?Research is showing that chronic inflammation may be the common factor in many diseases.

hen it comes to risk reduc-tion, it often seems as thoughwe need to keep a running tabof things to avoid or embrace.

Lay off saturated fats to keep our arteriesclean. Steer clear of carbs to stave offdiabetes. Stay away from smoking toprevent lung cancer. Solve the sudokupuzzle to outwit dementia. And exercise,exercise, exercise! Wouldn't it be nice ifthere were a one-stop approach to healthjust as there is to shopping?

Although there may never be such asingle path, mounting evidence suggestsa common underlying cause of majordegenerative diseases. The four horsemenof the medical apocalypse— coronaryartery disease, diabetes, cancer, andAlzheimer's—may be riding the samesteed: inflammation.

New research on inflammation hascreated a shift in medical thinking. Fortwo millennia it has been viewed mainlyas a necessary, even beneficial, responseto illness or injury. But now both obser-vational studies and laboratory researchare indicating that inflammation can bemore of a bane than boon, the common,causative factor in many diseases.

What is inflammation?Inflammation is part of the immuneresponse. It's a process that depends bothon the physical actions of white bloodcells and the chemicals that they produce:antibodies, cytokines, and the like. Overthe last several decades, scientists haveidentified dozens of new immunologicaland inflammatory molecules and thepathways through which they interact.The loops and feedbacks of thosepathways mean inflammation can beturned on and off in any number of ways.The problem comes when it is left on, forno apparent reason.

For simplicity's sake, immunologistsstill describe inflammation as depen denton two basic processes. The first, innateimmunity, relies on granule-

cytes and complement. Granulocytes areshort-lived white blood cells containingenzyme-filled granules that dissolveforeign substances. Complement is anarray of circulating proteins produced ina cascade of enzymatic activity in thepresence of microbes.

The second process, adaptive immu-nity, is directed specifically at microbesthat have invaded the body previously. Itslargely the responsibility of white bloodcells called lymphocytes. T lymphocytes,or T cells, are the master strategists of theprocess, directing cells and chemicals toeradicate the invader. B lymphocytes, orB cells, produce antibodies, which attachto specific pathogens and call in thecomplement to help dispatch the invader.Macrophages (literally "big eaters") arethe scavengers, swooping in to eliminatethe remains of microbes, deadgranulocytes, and cellular debris createdin the skirmish. While they clean up,macrophages also process informationabout individual pathogens and transmitit to the lymphocytes, which store theinformation for future reference.

As pathogens are eliminated, sup-pressor T cells turn down the inflamma-tory response, so the regeneration oftissue injured either by the invader or by"friendly fire" from the immune systemcan begin. Fibroblasts, cells that producecollagen and fibrin, arrive to create ascaffold for new tissue cells. If thedamage is extensive, fibrin and collagenmay become thick enough to re place theoriginal tissue and form a scar.

Chronic inflammationThe first-century Roman physicianAurelius Cornelius Celsus described thefour cardinal signs of inflammation—color (heat), dolor (pain), rubor (red-ness), and tumor (swelling)—a Latinlitany still learned by medical studentstoday. But these symptoms are mainlytip-offs for acute inflammation.

Chronic inflammation often stays

under the patient's—and doctor's— radar.It occurs when the triggering agent isn'tentirely eliminated or the suppressor Tcells don't call off the immune systemafter the body has repulsed the invasion.

All of us adults have some level ofchronic inflammation slowly waging awar of attrition on tissues and organs, itsactivity often evidenced only in bloodtests. But if it's turned up a notch or two,chronic inflammation can wear away atthe body so that the damage isdevastating.

We've some evidence of inflamma-tions sweeping, "cross-platform" effects.Observational studies, like the Framing-ham Heart Study and the Nurses' HealthStudy, have found lower rates of anumber of degenerative diseases inpeople who take nonsteroidal anti-inflammatory drugs (NSAIDs)regularly—usually for pain or arthritis.High levels of C-reactive protein (CRP),a marker for inflammation, are associatedwith several illnesses.

But for the most part, our ideas aboutdisease are organized by organ system, soscientists have tended to focus on theinflammatory process in particular organsor tissues. They are beginning to workout how inflammation lays thegroundwork for the following:

Coronary artery disease. Cardio-vascular research indicates that inflam-mation acts in concert with an excess of"bad" LDL cholesterol to create ath-erosclerosis. At high blood levels, LDLcholesterol becomes oxidized. Thatmakes it recognizable to the immunesystem and marks it for ingestion bymacrophages. The lipid-loaded macro-phages trigger complement activity thatdamages the vascular endothelium— thelayer of cells that lines the inside of bloodvessels. Macrophages and their fattycargo slip through the resulting cracksand lodge next to the arterial wall, wherethey are encased in a shell of fibrin andform arterial plaque.

As the plaque grows and its fibrin coat isstressed, it may rupture, forming a clot thatblocks a coronary artery supplying oxygento the heart muscle. Heart tissue nourishedby the artery then dies, caus ing a heartattack.

Studies have determined that peoplewhose CRP levels rank in the top third. aretwice as likely to have a heart attack asthose with CRPs in the lowest third. Therisk is even greater if a person also has highcholesterol. More doctors are adding a CRPtest to the battery of routine screening testsfor adults.

Diabetes. Several large observationalstudies have shown that people with highlevels of CRP are more likely to developinsulin resistance, a precursor to full-fledged diabetes in which cells rebuffinsulin and therefore don't properlymetabolize glucose circulating in the blood.Researchers have also found that peoplewho ultimately develop diabetes have highlevels of inflammatory molecules, includingTNF-a, a molecule produced bymacrophages, and T cells.

TNF-a seems to increase the liver'sproduction of glucose and triglycerides andinterfere with insulin's duties as a bloodsugar escort. Moreover, insulin has anti-inflammatory effects of its own. Thusinflammation not only sets the stage forinsulin resistance but accelerates as insulinresistance sets in, which may further hastenthe onset of diabetes.

Cancer. Nearly 150 years ago, thepathologist Rudolf Virchow termed cancera "wound that doesn't heal." He noticed thattissue from malignant tumors containedhigh concentrations of inflammatory cellsand hypothesized that the tumors oftenformed at sites of chronic inflammation.Recent evidence suggests that he was right.About 15% of cancers—including cancersof the liver, cervix, and stomach—areclosely linked to infectious diseases.Cigarette smoke and asbestos containinflammatory substances. Exposure tocigarette smoke is a notorious cause of lungcancer, and exposure to asbestos is linked tomesothelioma, a cancer of the tissue liningthe chest.

Moreover, laboratory research hasshown that products of inflammatoryreactions, such as reactive oxygenspecies, damage cellular DNA, creatingmutant genes that lead to cancer.Macrophages, the mop-up molecules inthe inflammatory process, churn outnumerous tumor growth factors andappear to spur on angiogenesis, thegrowth of new blood vessels that nurturetumor cells with a fresh supply of blood.In short, malignant tissues seem tocommandeer many of the inflammatoryweapons sent out to vanquish them.

Alzheimer's disease. Doctors oncethought the central nervous system wasoutside the reach of the immune system.The blood/brain barrier, formed bytightened capillaries, acts like a bouncer,screening out inflammatory cells andmolecules so they can’t enter the brain.Yet observational studies have foundlinks between NSAIDs (aspirin,ibuprofen, and naproxen), COX-2inhibitors, and other anti- inflammatorymedications and a lower risk ofAlzheimer's disease. In addition, thebrain may have its own branch of theimmune system. Cells inside the braincalled microglia, the counterparts tomacrophages, swarm and engulf foreignsubstances and release TNF-a and otherinflammatory molecules. Excessproduction of a molecule called beta-amyloid appears to play an important,and perhaps initiating, role inAlzheimer's disease, but the immuneresponse may also be involved. Oncemicroglia ingest beta-amyloid, theybecome enshrouded in fibrin and formthe plaques characteristic of the disease.

Preventing inflammationInflammation is now a fertile field for

basic research. The recently discoveredgene for selenoprotein S, a protein that

plays a pivotal role in controllinginflammation, may lead to a test

predicting who is at high risk forinflammatory disorders, and to the

development of new drugs for chronicinflammation.

There are already scores of anti-inflammatory agents on the market, ranging

from plain old aspirin to high-tech bio-engineered molecules for treating asthma,

rheumatoid arthritis, and multiple sclerosis.Long-term use of NSAIDs is a balancing act,

because while it may keep the fires ofinflammation burning low, it can also causestomach bleeding and liver and kidney

damage. The COX-2 inhibitors, especiallyrofecoxib (Vioxx), have been linked to

increased heart attack risk. Still, the daily 81-mg dose of aspirin recommended for people

at high risk for heart attack is a safe bet.Commonsense health practices may also

help. Keep regular dental appointments tospot and treat periodontal disease; theevidence is mixed, but some research hassketched a possible link between gumdisease and coronary artery disease. Take thefull prescribed dose of antibiotics to preventlingering infection and re-infection withresistant bacterial strains.

Low-calorie diets and moderate exerciseremain the surest thing for preventing

degenerative inflammatory disease. Obesityis linked to high CRP levels, as are "bad" fats

and refined carbohydrates. Saturated andtrans fats tend to stoke the immune response,while omega-3 fatty acids (in fish oils) and

monounsaturated fats (such as olive andcanola oils) may dampen it. Highly

processed carbohydrates may promoteinflammation by aiding the formation of free

radicals, so whole grains with unrefinedcarbs are more healthful. Alcohol — in

moderation only — also cools offinflammation.

So, for now, it looks as though therecognition of inflammation's role in diseasedoesn't alter the standard preventiondogma—see a doctor regularly for checkups,follow a sensible diet, exercise routinely,avoid cigarettes, imbibe but with restraint,and take a baby aspirin if you've had a heartattack or you fall into a high-risk categoryfor heart disease. Those practices may holdthe dreaded horsemen at bay by keeping theirsteeds in the stable. ®
