Which family members Are Eligible for UsU Benefits?Eligible family members are:• Spouse• Domestic partner• Children under age 26 • Natural children • Step children • Legally adopted children • Children for whom you have legal guardianship • Foster children • Children placed with you for adoption • Children of your domestic partner who depend on you for support and live with you in a parent/child relationship • Unmarried children to whom you are legally responsible to provide health coverage under the terms of a Qualified Medical Child Support Order • Unmarried children over the age limit who depend on you for primary financial support and maintenance due to physical or mental disability

Does your spouse or partner Work at UsU?If your spouse works at USU, you may elect Dual Coverage (see page 11 for details). Both employees will pay premiums for the same plan, one at family or couple rates, the other at single rate.

Waiving medical CoverageIf you have medical coverage elsewhere, you may choose to waive coverage through USU by filling out the Medical Opt-out form. Please contact the Office of Human Resources at (435) 797-0122 to obtain this form.

Steps for Medical and Dental Enrollment/Changes: 1. Between May 2 - June 3, 2016, go to ServiceNow.usu.edu and log in with your A number and password.

2. Click on the Service Catalogs link and select the Human Resources tile.

3. Click on Medical and Dental Enrollment.

4. Select the appropriate form.

5. Select your desired medical and dental insurance plans and coverage levels.

6. Verify your family members. (This is a list of all your family members on Banner Access.) If someone needs to be updated or added click Update Family Members, this will direct you to Banner Access.

Note: If adding a family member for the purpose of insurance you are required to attach proof of the relationship via ServiceNow. (e.g marriage certificate, birth certificate, or adoption document).

7. Verify that all eligible dependents are to be added to the insurances. When selecting No, please include the details in the coverage comments box.

8. Select “yes” to the agreement and click Submit Now.

VRx — UsU’s pharmacy Benefit managerUSING YOUR ID CARD

To use your prescription benefit the pharmacy must have the information on your ID card with the VRx logo. Be sure to show them the card the first time you fill a prescription (for each member of the family). Also, it’s a good idea to have the card available each time you fill a prescription. The RxBIN (610245) and RxPCN (05780000) provided on your ID card is required from the pharmacy in order for claims to process.

RETAIL PHARMACY

The retail pharmacies you used last year are still available. Most retail pharmacies, including, Walgreens, Smith’s, Target and Walmart are contracted to allow you to fill a 30-day supply and up to a 90-day supply of maintenance medications.

MAIL ORDER PHARMACY

VRx offers the convenience of home delivery through our partnership with WelldyneRx. You will be able to enroll online through the VRx website as soon as your coverage begins. You may also contact a VRx Benefit Advocate for additional details at (855) 586-2569.

SPECIALTY PHARMACY

VRx has contracted with specialty pharmacies to meet the needs of members using specialty medications. If you are using a specialty medication, please contact a VRx Benefit Advocate for additional details at (855) 586-2569.

The “Choice” HDHP medical plan with a Health Savings Account is not like the traditional High Premium or Wellness health insurance plans offered at USU. However, it may be a plan that works for you.

additional Information about the “Choice” High Deductible Health plan (HDHp) with a Health Savings account (HSa)The “Choice” High Deductible Health Plan (HDHP) is a zero premium (when choosing the Preferred ValueCare network), high-deductible plan combined with a Health Savings Account (HSA). The two go hand-in-hand. An HSA is a triple-tax-advantaged account where your contributions, account earnings, and qualified withdrawals are tax-free. Utah State University partners with HealthEquity, an industry leader in HSA administration.

If you select the “Choice” HDHP, there is no monthly premium when choosing the Preferred ValueCare network. The premium you save may be contributed to your HSA*. Not only will you save on premiums, but the money you put into your HSA account is controlled by you, and earns interest. HSA money belongs to you if you leave USU. You can make tax-free contributions to your HSA up to the HSA contribution limit of $3,350/single or $6,650/family. If you do not use the money, it continues to roll over each year.

The “Choice” HDHP encourages wellness, wise spending, and health-related savings.

Note: If you have “other coverage” such as coverage as a dependent on your partner’s or parent’s group policy, or if you are on Medicare, there are restrictions to opening an account and contributing to an HSA.

The $1,500 deductible is only for a single person on the HDHP plan. Once you have 2 or more persons on the medical plan your deductible is $3,000. All of the family members medical costs will add together to reach the deductible.

flexible spending Account - is it Right for you?*You Must Enroll Every Year*Utah State University offers employees a Flexible Spending Account (FSA) for both health expenses and dependent care expenses that are incurred during the plan year. Employees allocate a portion of monthly earnings, pre-tax, that can be used for health or dependent care expenses, which can result in substantial tax savings to employees who participate in the FSA.

• FlexMinder: FlexMinder can monitor your health plan expenses through Regence BCBS to identify claims with qualified out-of-pocket health care expenses that can be reimbursed from your health care FSA. You can then direct FlexMinder to prepare and submit FSA claims for the identified claim amounts. This can be done as frequently as you wish. You can also allow FlexMinder to “shoe-box” the identified claims throughout the year and then submit only once at the end of the plan year. To learn more and to sign up, log into your FSA account at ASIflex.com and click on the FlexMinder icon. • ASI Flex Mobile App: You can check your FSA account balance and file claims from your mobile device. The ASI Flex Mobile App is available at ASIflex.com, Google Play Store, or the App Store.

Note: If you elect the “Choice” HDHP, you are not eligible for the Medical Flex Spending Account, but you may sign up for the Dependent Care Account.

For a list of all eligible expenses, please go to ASIflex.com.

The current maximum amount you may set aside for health expenses is $2,550. Dependent care maximum amount is $5,000. A monthly administrative fee will be charged for all Flex spending accounts in the amount of $3.

To participate, you must sign up ANNUALLY online during open enrollment. To enroll go to enroll.asiflex.com and use the employer code USU. Remember that the money you set aside in your flexible spending account must be used within the same plan year.

MEDICAL AND DEPENDENT CARE FLEX SPENDING: Expenses may be incurred from July 1, 2016 through September 15, 2017.

Medical and dependent care receipts must be submitted for reimbursement by December 15, 2017.

the Hartford - life & Disability insurancesSUPPLEMENTAL LIFE INSURANCE: The open enrollment period is the time to purchase additional Supplemental Life Insurance for yourself or your dependents. If you are interested in Supplemental Life Insurance, you may purchase it from The Hartford Insurance Company.

SHORT-TERM DISABILITY INSURANCE: If you have not already purchased Short-term Disability Insurance, you have the opportunity to apply for it during open enrollment. This is a very inexpensive way to protect your income if you experience a short-term disability.

Retirement Healthcare savings plan (RHsp)The retirement health care savings plan allows you to contribute to a trust that can help you pay for qualified health care expenses in retirement or when you leave USU. The plan is administered by TIAA. You can choose to put your money into lifecycle or money market funds.

Because of its many tax-advantaged features, a retirement health care savings plan is valuable to anyone concerned about health care costs in retirement. You can contribute as much as you like to the RHSP using post-tax money, and your contributions grow tax-free. Then, when you leave USU, the funds you use for qualified health care expenses can be withdrawn tax-free.
